Verified 06/05/23 OBSERVATION: Observed ice machine with slight accumulation of what appears
to be pink mold on front of plastic chute guard. PIC to ensure machine is
emptied, cleaned, and sanitized.
**RE-INSPECTION** OBSERVED ICE MACHINE TO BE CLEAN AT TIME OF
RE-INSPECTION.

Verified 06/05/23 OBSERVATION: Observed several knives in knife holder on side of prep table
with slight accumulation of food particles on blades. PIC to ensure
knives are thoroughly cleaned and sanitized prior to being stored in
holder.

Verified 06/05/23 OBSERVATION: Observed pepperoni pizza in table-top warmer between
127-130'F. Instructed PIC to reheat pizza to 165'F prior to placing back
in unit. Discussed hot holding requirements with PIC.
**RE-INSPECTION** OBSERVED PEPPERONI PIZZA IN TABLE-TOP WARMER AT 1430F
AT TIME OF RE-INSPECTION.
3-501 .16 (A) (1) -P REGULATION: Item 21 Proper hot holding temperature -- Holding TCS Food, Hot
Verified 06/05/23 OBSERVATION: Observed chicken salad, tuna salad, red pepper sauce, and
pickles both in top section of in-line refrigerator at 45°F and 460F
respectively. Chicken salad and tuna salads were made at 10:00am this
morning. Observed deli ham and roasted red peppers both in cabinet
section of unit at 47°F and between 45-46°F respectively. PIC states
roasted red peppers have remained in unit overnight. PIC adjusted
thermostat on unit during time of inspection. Discussed with PIC that if
temperatures in unit do not return to 41°F within an hour, call for
service and send status update or repair slip to health office if found
to be necessary.
**RE-INSPECTION** OBSERVED CHICKEN SALAD AND SLICED CAPICOLA BOTH IN TOP
SECTION OF IN-LINE REFRIGERATOR AT 41OF AT TIME OF RE-INSPECTION.

Verified 06/05/23 OBSERVATION: Observed covered container of rice pilaf and covered
containers of onions in the process of cooling in walk-in refrigerator.
Upon discussion of proper cooling methods, PIC loosely covered containers
of onions and placed rice pilaf on metal sheet.
**RE-INSPECTION** OBSERVED RICE PILAF COOLING ON SHEET PAN IN WALK-IN
REFRIGERATOR AT TIME OF RE-INSPECTION.

cardboard boxes. Discussed with PIC foods are only stored in approved
food pans and containers.

OBSERVATION: Observed several non-food contact surfaces including but not
limited to interiors and exteriors of refrigerators, walls, and side of
oven with accumulation of soil residues . PIC to ensure surfaces are
cleaned at a frequency necessary to decrease such accumulations.
**RE-INSPECTION** OBSERVED INTERIOR OF SANDWICH IN-LINE REFRIGERATOR,
WALL BEHIND GRILL AREA, AND SIDE OF OVEN WITH ACCUMULATION OF SOIL
RESIDUES . PIC TO ENSURE INTERIORS OF EQUIPMENT AND WALLS ARE CLEANED AT A
FREQUENCY NECESSARY TO KEEP THEM CLEAN TO SIGHT AND TOUCH. WILL VERIFY
COMPLIANCE AT NEXT ROUTINE INSPECTION.

Discussion Discussed with PIC to post CFPM and allergen certificates in
public view adjacent to copy of food permit.
**RE-INSPECTION** CFPM AND ALLERGEN CERTIFICATES POSTED IN PUBLIC VIEW AT
TIME OF RE-INSPECTION.
In Compliance Observed valid copies of both CFPM and allergen certificates.
Observed bulk deli ham in "Polar" branded single-door refrigerator at
41°F. Observed breaded eggplant and pepperoni both in top section of
pizza in-line refrigerator at 40°F. Observed cooked chicken in cabinet
section of unit at 39°F. Observed sliced cheese in walk-in refrigerator
at 41°F. Observed raw steak and chicken both in four drawer refrigerator
under grill at 41°F. Observed meatballs in marinara sauce in table-top
hot holding unit at 150°F. Observed bleach sanitizer in bucket in
compliance. Observed foods in freezer units to be frozen solid. Testing
strips available. Observed food thermometer available. Observed dry
storage in compliance. No pest activity observed.
Discussion Observed hood stickers in compliance, next service due in
June 2023 .
Closing Violations marked "Verified" have been corrected. Violations not marked
"Verified" remain uncorrected. Uncorrected violations are to be corrected
immediately. Uncorrected violations may result in additional
Re-inspections and fees, fines and or administrative action including
