CONSERVATION DEPARTMENT Community & Economic Development Division Millpond Townhomes 12 Millpond North Andover, MA 01845 June 21, 2023 12 Millpond, North Andover Relocation of Comeast Utility Box NACC ##308 Pursuant to section 4.4.2 of the North Andover Wetlands Protection Regulations, Civil Design.Consultants, Inc., on behalf of Millpond Townhomes filed for a Small Project Permit application for work proposed at 12 Millpond, North Andover. The proposed work includes: during previous construction, a Conicast utility box located on the side of Unit 12 was moved to a temporary location. The work under this Small Project Application involves: relocating the Conicast utility box to a permanent location back along the side of Unit 12, consisting of approximately 20 feet of trenchwork within the outer Riverfront Area associated with the Cochichewick. Brook, 10 feet of which is located within the 100-foot wetland buffer to associated BVW. At the time of completion, all grades, landscaping, patios, and other improvements will be returned to their existing condition. During the June 14, 2023 public meeting, the North Andover_Conservation.._Commission N�ACQ found this project to qualify as a Small P o'cet Permit and voted unanimously to approve this pr;oJect_�ts.pzoposed Lander C'atega�w "Ia" C thcr prQ`ei �fs of rrair7iinal impact as cleterinine(l by the NAC'C, subject to compliance with the conditions listed below. The project is approved only as it relates to the Conservation Commission and any and all other approvals should be obtained by the applicant/homeowner. All work shall conform to the following: RECORD DOCUMENT'S: Small Project Filing Including: Application Checklist with supporting narrative and attachments, Received on May 22, 2023 "Utility Relocation Plan, C-111dated 05-18-23, scale 1"=10' prepared by Civil Design Consultants, Inc. The following conditions are hereby mandated: CONDITIONS: 1. Prior to work commencing, straw wattle erosion controls must be installed along the perimeter of the work area. Tlie applicant must schedule an on-site pre-construction meeting with the contractor and the Conservation Department to inspect installation and to review the scope of work and conditions of this approval. 12 JARIpond-Small Project Permit:Approval Category I, Page 1 of 2 2. All excess soils resulting from the work must be transported off site or utilized outside of the I 00-foot buffer zone to the resource area. 3. An on-site post-construction inspection shall be scheduled with the Conservation Department to verify compliance and approve removal of the straw wattle. 4.
