OBSERVATION: Unable to verify a CFPM certificate at time of inspection.
PIC to ensure a valid CFPM certificate is posted in public view adjacent
to copy of food permit.

OBSERVATION: No written procedure or kit available for responding to
vomiting and diarrheal events. Provided written procedure for how to
respond to bodily spills at time of inspection.

OBSERVATION: Observed grilled chicken, sausage patties, and lox all in
cold holding table closest to baked goods display at 47°F, 52°F, and 56°F
respectively. Foods observed in plastic bags inside of containers which
may be attributing to elevated temperatures. PIC to remove foods from
plastic bags and monitor temperatures in unit.

OBSERVATION: Observed ice machine with slight accumulation of what appears
to be mold around chute area. PIC to ensure machine is emptied, cleaned,
and sanitized.

OBSERVATION: No food thermometer available at time of inspection. PIC to
ensure a food thermometer that is capable of taking temperatures of hot
and cold foods is readily accessible for use at all times.

Measuring Devices
OBSERVATION: Observed water dripping on packaged foods and pitchers of
beverages in walk-in refrigerator. It appears water is coming from
refrigerator's A/C unit. PIC to move foods to a location where they are
protected from dripping water. PIC to have A/C unit inside of
refrigerator repaired by licensed repair personnel. PIC to send status
update or repair slip to health office within 24 hours.

preparaticn, storage and display -- Storage, Food-Preventing Contamination

OBSERVATION: Observed in-use utensils being stored in room temperature
water at time of inspection. Discussed with PIC that if utensils are
going to be stored in water, water must be maintained at 135°F.

OBSERVATION: Observed fan and area surrounding fan in walk-in refrigerator
with accumulation of dust. PIC to ensure fan and surrounding area is
cleaned at a frequency necessary to decrease such accumulation.

OBSERVATION: Unable to verify an allergen awareness certificate at time of
inspection. PIC to ensure a valid allergen certificate is posted in
public view adjacent to copy of food permit.

Discussion No food permit available at time of inspection. PIC to
contact health office to obtain copy. PIC to post permit in public view
once obtained.
In Compliance Observed gallon of milk in low-boy refrigerator below
espresso machine at 40°F. Observed chicken salad, tuna salad, and sausage
patties all in cold holding table to right of microwaves at 40°F.
Observed lox and sliced ham both in cabinet section of unit at 41°F.
Observed sliced tomatoes and butter both in walk-in refrigerator at 41°F.
Observed bleach sanitizer and testing strips available.
Closing Correct Priority Item and Priority Foundation Item violations immediately;
Core Item violations within 10 days. Correct all violations in entirety
and maintain. Train and supervise staff. Failure to correct all
violations and maintain corrections may result in administrative action
