approved threshold levels per manufacturer's instructions. PIC to use
testing strips to ensure sanitizer is maintained at proper concentration
levels.

APPROVED THRESHOLD LEVEL. REVIEWED WITH PIC TO FOLLOW MANUFACTURER'S

holding table at 56°F. Observed ice below unit to be low which may be
attributing to elevated temperatures. PIC placed TCS foods in walk-in
refrigerator during time of inspection. Discussed cold holding
requirements with PIC.
**RE-INSPECTION** OBSERVED CHOPPED TOMATOES IN COLD HOLDING TABLE AT
59°F. OBSERVED ICE BELOW UNIT TO BE LOW AND NOT MAKING DIRECT CONTACT
WITH PAN. PIC VOLUNTARILY DISCARDED TOMATOES AND ADDED MORE ICE TO UNIT.
***2ND RE-INSPECTION*** OBSERVED CHOPPED TOMATOES AND SHREDDED LETTUCE
BOTH IN COLD HOLDING TABLE BETWEEN 58-60°F. OBSERVED CONTAINER OF CHOPPED
TOMATOES DOUBLE PANNED WHICH MAY BE CONTRIBUTING TO ELEVATED
TEMPERATURES. CONTAINER OF SHREDDED LETTUCE NOT MAKING CONTACT WITH ICE.
PIC PUT CHOPPED TOMATOES IN SINGLE PAN DURING TIME OF 2ND RE-INSPECTION
AND ADJUSTED CONTAINER OF SHREDDED LETTUCE SO THAT IT MAKES DIRECT
CONTACT WITH ICE. PER DISCUSSION WITH HEALTH OFFICE, PIC TO USE AN
ALTERNATIVE METHOD TO COLD HOLD TCS ITEMS . WILL VERIFY COMPLIANCE AT 3RD
RE-INSPECTION. REVIEWED COLD HOLDING REQUIREMENTS WITH PIC.

kitchen, and on dry storage pallets to left of stairs in basement.
Employees starting cleaning and sanitizing affected surfaces mentioned
above during time of inspection.
**RE-INSPECTION** OBSERVED A FEW DROPPINGS CONSISTENT WITH MICE ON LID OF
DRY STORAGE CONTAINER UNDER PREP TABLE. PIC CLEANED AFFECTED AREA DURING

Verified 05/25/23 OBSERVATION: Observed droppings consistent with mice in plastic container
holding bags of breader & batter mix on bottom shelf of prep table across
from fryers. Observed additional droppings in box of instant rice that
PIC states is their own personal food. Both food items were voluntarily
discarded during time of inspection.

Discussion No pest control reports available on site at time of
inspection. PIC states intention to contact pest control company for
service and for reports. PIC to contact Brian LaGrasse at health office
prior to opening establishment on Monday (5/15) to give status update on
pest activity and scheduled service.
**RE-INSPECTION** OBSERVED PEST CONTROL REPORT FROM ANDOVER PEST CONTROL
STATING GLUEBOARDS AND BAIT BOXES HAVE BEEN INSTALLED IN ESTABLISHMENT.
Verified 05/25/23 OBSERVATION: Observed ample accumulation of grease and dust behind fryers
and on floor below dry storage containers . PIC to ensure area is cleaned
at a frequency necessary to decrease such accumulations.
**RE-INSPECTION** OBSERVED LOCATION MENTIONED ABOVE TO BE IN IMPROVED
CONDITION. PIC STATES INTENTION TO MOVE FRYERS FORWARD AND CLEAN AREA
BEHIND THEM THIS WEEKEND.

AT TIME OF RE-INSPECTION. PIC STATES INTENTION TO CONTINUE REMOVING

PIC states hood cleaning company is scheduled to service hood soon. PIC
to have date scheduled by re-inspection date.
**RE-INSPECTION** PIC STATES PROFESSIONAL HOOD CLEANING COMPANY IS
SCHEDULED TO SERVICE HOOD ON 6/12/2023.
In Compliance Observed CFPM and allergen certificates posted in public
view. Observed seafood salad and ham both in top section of in-line
refrigerator at 40°F and 41°F respectively. Observed fresh mozzarella in
cabinet section of unit at 41°F. Observed cooked peppers and pickles both
in "True" branded single-door refrigerator at 40OF and 36°F respectively.
Observed packaged raw beef in walk-in refrigerator at 41°F. Observed
sausages in table-top hot holding unit at 170°F. Observed shredded
chicken in metal pan on flat-top grill at 178°F. Observed foods in
freezer units to be frozen solid. Observed clean knives in knife holder.
Observed toilet room in compliance.
