Re: Subsurface Sewage Disposal System Plan for 125 Boston Street (Map 107B,Lot 43)
Dear Mr. Morin:
The proposed wastewater system design plan for the above site dated October 26, 2022 has been
reviewed. Unfortunately,the plan cannot be approved until the following items are corrected.
The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by
this design follows each item where applicable.
1. Provide the elevation statement as described in section 3.2 of the North Andover Board
of Health onsite wastewater regulations, if appropriate
2. Provide the Lake Cochichewick statement as described in section 3.2 of the North
Andover Board of Health onsite wastewater regulations,if appropriate
3. Indicate the orifices in the perforated pipe shall be 3/8"-5/8" (15.251)
4. Correct the soil testing date
5. Remove break line in leach facility cross section if appropriate to do so
6. Provide the system dimensions on the site plan(15.220)
7. Provide for a distribution box that is H-20 rated(NA 3.2)
8. Clarify the location of the barrier on the site plan. It is not clear where it is to be
constructed and where contour 98 is located
9. It seems the existing building sewer and septic tank are proposed to be re-used, but that a
different brand and model effluent filter are to be provided in the outlet tee of the tank. Is
that the only changed proposed for these components? If so,make it clear which
components of the building sewer and septic tank are to remain and which are to be
adjusted
10. Depict the inspection port on the system profile and provide a detail of hoe it is to be
constructed(15.240)

Although not a reason for disapproval, you may wish to consider changing note 12 to reference
Title 5 (versus Title V)to provide for correct citation.
