DESCRIPTION OF VIOLATION

OBSERVATION: Observed egg patties and sausage patties both in "True"
branded single-door refrigerator at 54°F and 52°F respectively. PIC
states both foods were placed in containers approximately an hour and a
half ago. PIC states intention to call for service on unit. PIC
voluntarily relocated affected food items during time of inspection. PIC
to send status update or repair slip to health office within 24 hours.
Discussed cold holding requirements with PIC.
3-501 .16 (A) (2) (B) -P REGULATION: Item 22 Proper cold holding temperature -- Holding TCS Food,
Cold
OBSERVATION: Observed ice machine with slight accumulation of what appears
to be mold around chute area. PIC to ensure machine is emptied, cleaned,
and sanitized.
4-602 .11 REGULATION: Item 16 Food contact surface; cleaned and sanitized --
Equipment Cleaning FCS Frequency
OBSERVATION: Observed one food employee constructing breakfast sandwich
without wearing approved hair restraint. PIC to ensure approved hair
restraints such as a hat or hair net is worn while open-food handling is
conducted.

OBSERVATION: Observed interior of four drawer cabinet refrigerator under
top section of in-line refrigerator and exterior of warming ovens with
accumulation of food and dust particles respectively. PIC to ensure both
surfaces are cleaned at a frequency necessary to decrease such
accumulations.

In Compliance Observed CFPM certificate posted in public view. Observed
coconut milk in "Delfield" branded low-boy refrigerator across from
warming ovens at 37'F. Observed egg patties and bacon strips both in top
section of in-line refrigerator at 41°F. Observed egg patties and sausage
patties both in walk-in refrigerator at 41°F. Observed egg patties and
sausage patties both in sliding drawer warmer at 160°F and 147°F
respectively. Observed quaternary sanitizer in labeled sanitizer bucket
at 200ppm. Testing strips available.
Closing Correct Priority Item and Priority Foundation Item violations immediately;
Core Item violations within 10 days. Correct all violations in entirety
and maintain. Train and supervise staff. Failure to correct all
violations and maintain corrections may result in administrative action
