OBSERVATION: No disposable paper towels available at handwashing sink
adjacent to lemonade dispensers behind front counter. PIC to ensure
handwashing sinks are supplied with paper towels at all times.

OBSERVATION: Observed slicer in prep area closest to three bay sink with
ample accumulation of dried on food particles on front and back of
blades. PIC to ensure slicer is thoroughly cleaned and sanitized after
each use.

OBSERVATION: Observed ice machine with slight accumulation of what appears
to be mold on front and back of plastic chute guard. PIC to ensure
machine is emptied, cleaned, and sanitized.

OBSERVATION: No consumer advisory on menu at time of inspection. PIC to
ensure foods that may be served or ordered raw/undercooked or contain raw
or undercooked ingredients (such as roast beef) are identified on menus
by asterisking them to a footnote that states, "these items are served
raw or undercooked, or contain (or may contain) raw or undercooked
ingredients" in addition to a reminder that states, "Consuming raw or
undercooked meats, poultry, seafood, shellfish, or eggs may increase your
risk of foodborne illness. "

OBSERVATION: Observed multiple non-food contact surfaces in establishment
including but not limited to on floor of walk-in refrigerator, on floor
behind single-door refrigerator in kitchen, and on shelf containing
containers of mayonnaise and barbecue sauce in kitchen with accumulation
of droppings consistent with mice. Observed latest pest control report
available on site dated in 2022. Observed online confirmation that
Ehrlich Pest Control is schueduled to service establishment tomrrow
(8/9) . PIC to ensure reports are filed on site for review during
inspections.

OBSERVATION: Observed what appears to be dead mice on glueboard on floor
in back right corner of walk-in refrigerator and on glueboard on floor in
dry storage area. PIC to remove both glueboards.

OBSERVATION: Observed soiled, unused slicers as well as other
miscellaneous articles on floor below staircase in basement area.
Discussed with PIC to remove any articles from establishment that are no
longer used.

Discussion Discussed with PIC to post allergen certificate and food
permit in public view adjacent to CFPM certificate.
In Compliance Observed CFPM certificate posted in public view. Observed
butter, sliced tomatoes, and roast beef sandwiches all in "Pepsi" branded
single-door refrigerator. Observed pasta salad and coleslaw both in
additional "Pepsi" branded single-door refrigerator at 41°F. Observed
cooked chicken patties and potato salad both in "True" branded two door
refrigerator. Observed boxes of french fries in walk-in refrigerator at
41°F. Observed french fries in fry basket hanging above fryers at 141'F.
Observed bleach sanitizer available.
Discussion Observed hood sticker in compliance, next service due in
September 2023.
Closing Correct Priority Item and Priority Foundation Item violations immediately;
Core Item violations within 10 days. Correct all violations in entirety
and maintain. Train and supervise staff. Failure to correct all
violations and maintain corrections may result in administrative action
