Indicate the map and lot of the parcel (15.220). The map and lot number of the parcel have been added to Sheet S-1 underneath the Lot and acreage information. 2. Indicate the scale of the system profile drawing or provide a scaled profile(NA 3.2). The scale of the profile view of the system is noted on the title block(right hand side) of Sheet S-2. 3. Clarify Note#16 to indicate of the design has been completed to include the use of a garbage disposal or not. Note#16 has been revised to say that the proposed septic design has not been designed to accommodate the use of a garbage grinder. 4. Clarify if the Enviro-Septic or Advanced Enviro-Septic treatment/disposal system is being proposed.Different names are used on the design plan.In addition,provide the MassDEP approval for the system proposed to be used. The Presby Enviro-Septic Leaching System is being proposed.Any reference to the Advanced Enviro-Septic treatment system has been removed from the plan.A copy of the DEP approval will be provided to the local approving authority. 5. Clarify where it indicates that the breakout elevation is to be measured at the bottom of the System Sand as indicated on the design plan. Section G, Standard System Configurations, Page 22 under "Definition"for Raised(Mounds) System Sections refer to the breakout elevation as the bottom of the system sand bed/soil interface (6"below the bottom of the ES pipes). 6. The Building Sewer needs to be specified to have watertight joints with the pipe laid on a compact firm base in a straight line on a continuous grade (15.222 and 225). A note has been added on Sheet S-2 (profile view)with this specification. 7. The tees inside the tank need to be located beneath an opening in the tank lid.The detail on Sheet 3 does not depict this and could be a source of confusing during construction and should be correct(15.227). The proposed septic tank has been revised to a one compartment septic tank to avoid confusion. 8. Depict the location of the test pits and percolation tests (15.102). The location of the test pits and percolation tests was provided on the plan.It appears they may have become too light once printed and scanned, however. We have darkened the coloring on the test pits and percolation tests on the revised plans. 9. Provide an explanatory note regarding the C2 soil horizon not being considered as permeable soil,or else provide for a percolation test in this soil layer. The Form 11 soil suitability assessment forms submitted note that the C2 layer was very unlikely to be able to pass a percolation test.As such Sheet S-1 notes a request for a reduction in the 4'of required naturally occurring pervious material beneath the soil absorption system.An additional note has been added on Sheet S-2 with the "Soil Test Pit Data"information noting that the C2 layer is not permeable. 10. Provide a draft of the deed notice to be recorded. A draft of the deed notice is being provided to the local approving authority alongside the resubmittal of the design plan. 11. Provide proof that you have completed any required training for use of this brand system(Std Condition II (18)). My Presby certification will be provided alongside the resubmittal of the design plan. 12. Provide required designer certification of the plan(Std Condition II(18)). My Presby certification will be provided alongside the resubmittal of the design plan. 13. Provide a system owner acknowledgement of the type of system being proposed and the various owner responsibilities(Std Condition II (18)). A signed owner acknowledgement of the type of system being proposed and various owner responsibility is being provided alongside the resubmittal of the design plan. 14. Clarify the note on the design plan regarding alternative system design standards as it indicates a secondary treatment unit is being utilized however none seems to be present. Section 10,D of the Standard Conditions for Alternative Soil Absorption Systems with General Use Certification and/or Approved for Remedial Use specifically reference when a reduction in the required 4'of naturally occurring pervious material is needed that it may be taken in accordance with the Standard Conditions for Secondary Treatment Units Approved for Remedial Use.Although there is no secondary treatment unit proposed, this is the approval document that is referenced under these circumstances. 15. Raised Connection Detail Note#2 references specifications located in a different document and the specifications for system abandonment and for Inspection Port construction references sections of Title 5.All required construction standards which are to be applied need to be indicated on the design plan,not on a third party- document. Note#2 has been corrected, and the specification for system abandonment and for inspection port construction are now more clearly described on the plan.
