Hi Amy,
On hold at the moment.When the homeowner purchased the property there was money held in escrow to permit
and complete the septic repair, so they are not in an immense rush as he has told us the system has not been a
problem for them—when we did the soil testing I did not observe any breakout either.The other reason for the delay
is the homeowner also has a few large pines which border his and Harold Parker(I believe they are on Harold Parker
land) land along his driveway which are in the 200' RA and 100' buffer zone that he would like to remove and include
as part of the NOI application.They are currently leaning towards his home.They appeared to be quite unhealthy
when I visited this past fall, so we recommended that the homeowner contact Harold Parker HQ to discuss the issue
with them before bringing it to the Commission.

All went well at the construction inspection at this site. No issues were noted. Inspection report is attached.
Before issuing a Certificate of Compliance please note that the non-traditional system used at this location has some
requirements:
• A deed notice indicating the presence of the Enviro-Septic system needs to be recorded on the deed and a copy of
this provided
• The designer and installer are to certify that the system was built in compliance with the terms of the DEP approval
for the non-traditional system used
I have let the designer know about these items and suggest you hold off on issuance of a Certificate of Compliance until
they are tended to.

We have completed our review of the revised plan for 1761 Salem Street and believe all is in order. Please note there are
two items in the approval which would need to be complied with before issuance of a DSCP to a licensed installer. These
are needed because of the type of system selected for use at this site.
