HISTORY AND BACKGROUND On September 19, 2003 , Eaglewood Properties , LLC filed an Application fox a Site Plan Review Special Permit with tie Planning Board pursuant to section 8 . 3 10 . 3 , 10 . 3 1 and 13 of the Town of North Andover Zoning Bylaws . On February 5 , 2004 the Planning Board unanimously voted to grant the Original Appl icant a Site Plan Review Special Permit for the Premises affected and P�•oceeded to issue a Special Permit Decision approving the Project . On July 19 , 2012 , Ross Hamlin, Englewood Propet�ties Manager, requested a Waiver of Site Pl an Review with respect to a change in use for the space Previously occupied by Appleseeds (women ' s clothing) to professional office, to be occupied by Doctors Express „ The applicant has submitted tie following un ormation ". • There w111 be no change in size o e space . • No changeisanic ae for parking . • The entrances and egress will remain the same . The Englewood facility does provide for retail and restaurant uses, each with dt" fferent peak parking requirements , The proposed changes are therefore deemed insubstantial ar�d not materiallydifferent than the approved plan, referenced in the February 5 , 2004 Special Permit Decision.
