OBSERVATION: No written procedure or kit on site for responding to
vomiting and diarrheal events. Provided PIC with written procedure for
how to respond to bodily spills.

OBSERVATION: No handwashing soap available at handwashing sink in kitchen
area at time of inspection. PIC to ensure all handwashing sinks are
supplied with handwashing soap at all times.

OBSERVATION: Observed packaged "power balls, " and overnight cats both in
customer accessible single-door refrigerator near point of sale to be
packaged in a way that does not protect food from potential contamination
and/or tampering. PIC to ensure foods are packaged in a way that the
integrity of food is protected from potential contamination and tampering.

OBSERVATION: Observed juice shots in single-door refrigerator missing
label "WARNING: This product has not been pasteurized and, therefore, may
contain harmful bacteria that can cause serious illness in children, the
elderly, and persons with weakened immune systems. "

OBSERVATION: Observed juices, overnight oats, and "power balls" missing
labeling components. Discussed labeling requirements with PIC.
Label information shall include: (1) The common name of the FOOD, or
absent a common name, an adequately descriptive identity statement; (2)
If made from two or more ingredients, a list of ingredients in descending
order of predominance by weight, including a declaration of artificial
color or flavor and chemical preservatives, if contained in the FOOD; (3)
An accurate declaration of the quantity of contents; (4) The name and
place of business of the manufacturer, packer, or distributor.

DESCRIPTION OF VIOLATION

OBSERVATION: Observed interior of chest freezer containing frozen fruits
with slight accumulation of food spills and residues. PIC to ensure
interior of unit is cleaned at a frequency necessary to decrease such
accumulations.
4-602 .13 REGULATION: Item 49 Non - food contact surfaces clean -- Nonfood Contact

Discussion Limited amount of TCS foods on site at time of inspection.
In Compliance Observed CFPM and allergen certificates posted in public
view. Observed cream of coconut in two door low-boy refrigerator at 41°F.
Observed foods in freezer units to be frozen solid. Observed foods
delivered to establishment frozen solid. Observed quaternary sanitizer in
three bay sink in compliance. Observed dry storage in compliance.
Observed toilet room in compliance. No pest activity observed.
Closing Correct Priority Item and Priority Foundation Item violations immediately;
Core Item violations within 10 days. Correct all violations in entirety
and maintain. Train and supervise staff. Failure to correct all
violations and maintain corrections may result in administrative action
