Information Required for Limited Site Plan Review A. Limited Site Plan Review form,along with any fees as may be set; B. Drawings prepared at a scale of one inch equals 40 feet or larger, or at a scale as approved in advance by the Town Planner. Revised plans shall contain a notation listing and describing all revisions, additions, and deletions made to the originally submitted plans and the date of each. C. All site plans shall be prepared by a certified architect, landscape architect,and/or a civil engineer registered in the Commonwealth of Massachusetts. All landscape plans shall be prepared by a certified landscape architect registered in the Commonwealth of Massachusetts. All building elevations shall be prepared by a certified architect registered in the Commonwealth of Massachusetts.All stormwater management plans and drainage calculations must be submitted with the stamp and signature of a professional engineer(PB)licensed to conduct such work in the Commonwealth of Massachusetts. Inspected the proposed site for code required safety requirements for a daycare facility. The 4,230 squafe foot area of an existing building is proposed to be changed to I4 Daycare facility. Per the owner of the Dayeare. The facility is planning for 9 toddlers (2-3 years old), 20 preschooler(4+years old) and 7 infants (2+months old), It will be considered a large group where it could be over 10 children.All the children will be constantly monitored by trained personal. The denlising walls between the rest of the building and this area is from floor to roof and fire rated. The existing building has a NFPA 13 fire suppression systern. The sprinkler head coverage in this area is installed per NFPA 13 standard and each head is properly located to support the area and use. The area is open concept area.with some new low partition walls that will not binder the existing water spray from existing heads. There are two rooftop HVAC units providing heating (gas), cooling and ventilation. The thermostats will be changed to programmable and run the fan continuously during occupied periods for ventilation. One unit has a duct smoke detector in the return (over 2000 cfm). The existing NFPA 72 fire detection system has pull stations at each exit from the space. The exits lead directly to the exterior. There are two exits from this area. There are the proper amount of notification devices and locations in [lie area to notify everyone it smoke is detected or someone pulls the manual pull station(s).All children will be supervised with trained personal for both fire and security notifications. Drop-off is spread out from 7:30 o,m, to 9:00 a,m, every morning. After families drop off their children and check in using the mobile phone platform, staff will be responsible for greeting the child and guiding them to their assigned classroom.
