Joyce A. Bradshaw, Trustee of Gorton Family Trust udder Declaration of Trust
dated April 20, 1999, recorded at Book 6407, Page 43 ("Grantor"), for the
consideration of the Planning Board's issuance of a Decision on the Approval of
a Definitive Subdivision Plan, dated September 1, 2015, and the Planning
Board's issuance of a Decision on the Approval of Special Permit for a Planned
Residential Development, dated September 1, 2015, hereby grants, transfers
and deliver unto the Town of North Andover, a municipal corporation in Essex
County, the following:
A. The perpetual rights and easements to construct, inspect, repair, remove,
replace, operate and forever maintain (1) a sanitary sewer or sewers with
any manholes, pipes, conduits, and other appurtenances, (2) pipes,
conduits and their appurtenances for the conveyance of water, and (3) a
covered surface and ground water drain or drains with any manholes,
pipes, conduits and their appurtenances, and to do all other acts Incidental
to the foregoing, including the right to pass along and over the land for the
aforesaid purposes, in, through, and under the whole of Wellington Way
(the "Roadway") and upon any of the drainage easement areas, including
the 10'Wide Drainage Easement area adjacent to the Roadway and the
Stormwater Management Easement area (the "Drainage Easement
Areas") as depicted on the subdivision plan entitled, uDefinitive P.R.D.
Subdivision Plan of Wellington Woods in North Andover, MA.," Record

as Plan No. said Subdivision Plan is referenced and incorporated
herein for a complete and detailed description of said Roadway and of l
said Drainage Easement Areas.

B. The perpetual rights and easements to use, for the conveyance of water,
groundwater and stormwater associated with residential uses, the way
known as Wellington Way and the Drainage Easement Areas situated
adjacent thereto, in said Town of North Andover and as depicted on the
Subdivision Plan.
The Grantor warrants that the aforesaid easements are free and clear of all
liens or encumbrances, that Grantor has good title to transfer the same, and
that the Grantor will defend the same against claims of all persons.
For Grantor's title see deed of William Gorton dated April 20, 1999 and
recorded at Book 5407, Page 47 (being Parcel I as referenced In said deed),
as affected by deeds recorded at: (1) Book 14145, Page 206, (ii) Back 14145,
Page 208, (iii) Book 14223, Page 270, and (iv) Book 14223, Page 273.
Reference is hereby made to that certain Form M—Conveyance of
Easements and Utilities, dated October 14, 2015 and recorded at Book
14430, Page 304 (the "Original Form M"). Whereas the Original Form M was
incomplete and failed to make specific reference to the Subdivision Plan, to
the easement areas or to the Grantor's title deed, and the Town has
requested that such references be corrected, this instrument is granted and
recorded as a confirmatory and corrective Form M easement to supersede
and replace the Original Form M by completing and correcting all of such
references as herein contained.
This is not a homestead property.
To be completed if a mortgage exists:

the above described land, which mortgage is dated , and
recorded in said Deeds, herewith, at Book , Page , for
consideration paid, hereby releases unto the Town forever from the operation
of said mortgage, the rights and easements herein above granted and
assents thereto.
