(Chambers) If sediment has accumulated flush chambers and remove water with a vacuum truck i 2.0 CERTIFICATION OF COMPLIANCE WITH FEDERAL, STATE AND LOCAL REGULATIONS This Operation and Maintenance Platt reflects the storm water management requirements of the Town of North Andover and the Commonwealth of Massachusetts, as well as the requirements of the US EPA's National Pollutant Discharge Elimination System I (NPDES)permit. There are no other applicable State or Federal requirements for sediment and erosion site plans (or permits) or storin water management site plans (or permits). 3.0 INSPECTION AND MAINTENANCE OF STORMWATER MANAGEMENT CONTROLS: 3.1 Post-Construction: The following maintenance schedule for stormwater controls shall be established in accordance with Table 1 of this plan. 3.1.1 Grass Treatment Swales and forebays -Remove sediment fi-om treatment swales twice per year. -After construction,the treatment swales shall be inspected annually, in the spring, for the failure or erosion of sidcslope embanlunents, and for accumulated sediment.Necessary sediment removal, earth repair, and/or reseeding will be performed immediately upon identification. Disposal of the accumulated sediment must be in accordance with applicable local,state,and federal guidelines and regulations. 2 Wellington Woods Wellington Way,North Andover Mass. Pollution Prevention and Operations and Maintenance Plan -Post Construction Phase Date: June 26, 2015,Rev 8/10115,rev 10/15/18 -The grass on the swale sidestopes should be mowed, and grass clippings,organic matter, and accumulated trash and debris removed,at least twice yearly during the growing season, 3.1.2 Catch basins with SNOUT Maintenance Recornniendations: - Monthly monitoring for the first year of a new installation after the site has been stabilized. - Measurements should be taken after each rain event of.5 inches or more, or monthly, as determined by local weather conditions. Checking sediment depth and noting the surface pollutants in the structure will be helpful in planning maintenance, - The pollutants collected in SNOUT equipped structures will consist of floatable debris and oils on the surface of the captured water,and grit and sediment on the bottom of the structure. It is best to schedule maintenance based on the solids collected in the sump. - Optimally,the structure should be cleaned when the sump is half Rill (e.g. when 2 feet of material collects in a 4 foot sump,clean it out). - Structures should also be cleaned if a spill or other incident causes a larger than normal accumulation of pollutants in a structure. Maintenance is best done with a vacuum truck. All collected wastes must be handled and disposed of according to local environmental requirements. To maintain the SNOUT hoods themselves, an annual inspection of the anti-siphon vent and access hatch are recommended. A simple flushing of the vent,or a gentle nodding with a flexible wire are all that's typically needed to maintain the anti-siphon properties. Opening and closing the access hatch once a year ensures a lifetime of trouble-free service. 3,1.3 Infiltration/Detention Pond (Chambers) Infiltration Chambers should be inspected annually.The inspection will entail viewing the bottom of the chambers through the monitoring ports to determine if the units are draining properly. Water should not be standing in the chambers 72 hours after a storm. 3.1.4 Detention/Infiltration Area -Remove sediment twice per year. -After construction,the Infiltration/Detention Area shall be inspected annually, in the spring, for the failure or erosion of 3 Wellington Woods Wellington Way,Nortli Andover Mass. All snow from impervious surfaces shall be plowed to adjacent vegetated pervious surfaces to melt and recharge into the ground or run off into the stormwater system for treatment. If snow piles on site become too large to accommodate additional plowed snow they should be removed from the site; snow shall never be plowed onto adjacent properties or into adjacent resource areas. j 3.1.5 De-Icing and Sanding { De-icing compounds shall be used sparingly and only in areas required for the safety of the public. Sanding of the driveways and walks should also be kept to a minimum; if sanding is required the best management practices listed in i this plan should be inspected immediately after the winter and, if needed, accumulated sediment from sanding shall be removed. 3.2 Estimated Operations and Maintenance Budge 3 Any opinions of probable project cost or probable construction cost provided by Christiansen&Sergi, Inc, are made on the basis of information available to Christiansen&Sergi,Inc and on the basis of Christiansen&Sergi,Inc's experience and qualifications,and represents its judgment as an experienced and qualified professional engineer. Egan, Town Counsel Date: April 16, 2019 Re: Wellington Way,Street Layout and Acceptance With regard to the layout and street acceptance request for Wellington Way, all legal documents including the deeds for the street, open space, and instruments conveying easements have been reviewed and satisfy the requirements of the North Andover Subdivision Regulations and Massachusetts General Laws. However, the Assistant Director of the Department of Public Works/Town Engineer, in his April 16, 2019 memorandum, concludes that all necessary work on the berms in the right of way have not been satisfactorily constructed, Additionally, the Planning Department has concluded that the bank of the right of way has not been stabilized with vegetation, Section 255-5,11 of the Subdivision Rules and Regulations provide that the applicant is responsible for the maintenance of the streets from the edge to edge of the right of way until the street is accepted as a public way. During the site visit on April 9th, Planning was able to observe the functionality of the stormwater infiltration within the cul-de-sac following a .45"rain event which took place on April 8, 2019. The erosion along the roadway and within the drainage swales located along the edge of the roadway has since been corrected with loom and seed,however, these areas are not stabilized. Due to the following outstanding issues, Planning recommends the following corrections prior to street acceptance: • Planning staff should visit the site after at least one (1) additional rain events of.5"-1" or greater in order to view the drainage within the Wellington Way center island, Planning Staff visited the site on April 15'h after a.78" rain event and observed that the cul-de-sac infiltration system had functioned pr operly. 120 Main Street,North Andover,Massachusetts 81845 Phone: 978.688,9535 Fax: 978.688.9542 www.nortliandoverma.gov • The Wellington Way cul-de-sac,45 Wellington Way(Iot 5), and areas abutting the roadway has been repaired loaned and seeded, however,these areas are not stabilized. The Staff Planner should visit the site to confirm stabilization; Planning Staff visited the site on April 15"' and April 16th and observed that grass had started to grow with in the cul-de-sac and areas abutting the roadway, however this area is not stabilized. The rnodifred pond(Pond 8P) located at 45 Wellington sway is starting to grow grass as well, but this area is not stabilized. The Staff Planner should visit the site to confirin stabilization. • Loom and seed has been added at the site entrance and along the roadway; however, these areas are not stabilized. Please see pictures below. These aeas treed to be stabilized. • The asphalt berms located on lots 4, 5 and 6 should be repaired as they are breaking apart. Planning Staff visited the site on April 16"' and observed that the berms had been. repaired. However, we defer to the Town Engineer as to whether the repair was done statisfactorily. • The asphalt is cracked in front of the 37 Wellington Way driveway and should be repaired. Planning Staff visited the site on April 16"' and observed that the crack had been. repaired. However, we defer to the Town Engineer as to whether the repair was done statisfactorily. • The Operations and Maintenance Plan for the Stormwater System(the O&M Plan) was revised in 2018,however, the Special Permit Decision and Declaration of Trust reference the O&M Plan dated June 26,2015,Revised to August 10, 2015. Planning recommends that the applicant resolve this in a manner that ensures that both current and future homeowners are made aware of their maintenance obligations. Planning Staff approved an Insubstantial Change which reflects the revised Post Construction secion of the Operations and Maintenance Platt which is to be recorded at the Registry of deeds. • Planning requires a final revised stamped as-built plan,roadway profile and street Layout Plan to be submitted. These plans must be submitted both in digital and paper formats. Based on our site visit the majority of the punch list items have been addressed and the street construction including utilities have been substantially completed, with the following minor seasonal items that remain to be finished: 1. The top course of asphalt is cracking in front of the driveway of house #37. In addition, the asphalt berm installed at the edge of the driveway looks to have not been properly rolled after placement and is now falling apart. These items need to be addressed; 2. Similarly the asphalt berm at the edge of the driveway in front of house#45 also appears to have not been rolled after placement and is breaking apart. This item needs to be addressed; and 3. The center island infiltration system has been installed however the DPW has not seen how the system functions after multiple (at least 2) storm events. In addition the area is not permanently stabilized. It is our recommendation that the Planning Board hold the subdivision bond for the remaining items and the continued maintenance until such time as Town Meeting has accepted (or rejected in favor of private ownership) the roadway.
