Re: Subsurface Sewage Disposal System Plan for 95 Rea Street (Map 98B,Lot 99)
Dear Mr. Sullivan:
The proposed wastewater system design plan for the above site dated July 25, 2022 has been
reviewed. Unfortunately, the plan cannot be approved until the following items are corrected.
The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by
this design follows each item where applicable.
l. Depict or notate the tank tees to extend 6" above the flow line (15.227)
2. A manhole cover to finish grade is required above the outlet with an effluent filter. This
is noted in one location but a conflicting note is provided in another part of the plan
(15.227)
3. There should be a minimum of 9 inches of cover material above the top of the
distribution box. Since the distribution box will have a greater than 9 inches of cover
material provide a riser(s)to within 6" of finished grade (15.232)
4. Use leach trenches, not a leach field (15.240 and 249)
5. Provide specification for any needed sand fill (15.255)
6. Depict the location of the desired inspection port(s) (15.240)
7. Specify the required annual maintenance of the effluent filter(15.227)
