The work involves: installation of Smartjacks and supplemental beams to support existing;floor joist system beneath the Douse. l.nstalling a sure. purnp system that will dischar>e via a 10-foot 3" PVC pipe to connect to a subsurface 10--foot Verforated.,PVC pipe level s Treader 6" below grade in 1,5" crushed stone. The connections of these two pipes will be at.a 90-de ree an le so that the perforated section .will be to the left side of the connection if one is facing the back of the house). **See. attached modified plan as discussed and a1312roved by the NACC at the meetin . At its closest point die discharge pipe will be 26 feet from the bordering vegetated wetland as measured by Conservation staff in the field. IDuring the February 14 2024 public meeting, the North Andover Conservation. Commission ACC found this project to qualif, as a Small Project Permit and voted unanimously to approve this pfojiect as proposed under Cale nr `L"0115eo )v nIs a �>rirr>rrrrl arrr as subject to compliance with the conditions listed below. The project is approved only as it relates to die Conservation. Commission and any and all other approvals should be obtained by the applicant/homeowner. All work shall conform to the following: RECORD DOCUMENTS. Small Project Filing Including: • Application.Checklist with supporting narrative and attachments prepared by Erickson Foundation Solutions o Modified flan as annotated by Conservation Commission dated February 14, 2024 The following conditions are hereby mandated: CONDITIONS: 1. Prior to work commencing, a sufficient length of S" straw wattle, staked down, shall be installed between the area of discharge pipe work and the-,wetland. The wattle shall be not less than 18 feet from the edge of the wetland. 4 Oak Avenue—Small Project Permit Approval Category L Page 1 of 2 2. A pre-activity site inspection shall be scheduled with the Conservation Department and the contractor to verify installed erosion controls, review scope of work and conditions of this approval. 3. Post-construction inspection shall be scheduled with the Conservation Department to verify compliance and approve removal of erosion controls. 4. This permit shall expire one year fron-i the date of issuance. The applicant, homeowner and their contractors are advised that NO other work on the properiy is allowed under this permit. The NACC has,jurisdiction over all of the wetlands and associated I 00-Foot buffer zones and 200 1�iverfront Area to the Shawsheen River on site and-an work that involves ground disturbance or changjng the current conditions in these areas will require permitting with the NACC. This includes,but are not limited to, house additions,pools,patios decks, sheds, fences, garden beds, landscaping and bridge installation, tree or vegetation cutting/pruning.
