We received Planning Board Approval on March 16, 2021 and have been working diligently to prepare for construction this spring. Richards, MA Registration Number: 51848, License Expiration Date: 06/30/2024, am a registered design professional, and I have prepared or supervised the preparation of all design plans, computations and specifications concerning site civil design for the above named project and that to the best of my knowledge, information, and belief such plans, computations and specifications meet the applicable provisions of the Massachusetts State Building Code (780 CMR) and accepted engineering practices for the portions of the project for which Bohler has been contracted to provide Civil Engineering Services, with the following exceptions: • Four(4) light poles have not been mounted on the installed concrete bases on the project site. The installation of these poles is to be completed in the future by the American Construction Corporation under contract with Core States Group for JP Morgan Chase Bank due to supply chain complications with back-ordered poles awaiting delivery to the site. The contractor will notify the Town in writing when the materials have arrived and the installation is completed per the approved design plans; • One of the tenants added brand specific pavement marking to the site that was not a part of the approved design plans; • Some planting materials, quantities, and locations in the landscape beds vary from the approved design plans that will remain installed on-site; • Soveral plantings were observed to be dead or dying during the visits to the site that will need to be replaced, as necessary, per the approved design plans; • Minor landscape finishes such as planting edge definition, flush grading behind the Rte. 114 curbing and burying of irrigation lines are scheduled for Spring completion. Nothing in this document relieves the contractor of its responsibility regarding the provisions of the Massachusetts State Building Code (780 CMR 107)and this is not afield certification of ADA accessible route and parking compliance on the project site. I understand and agree that I (or my designee) performed the necessary professional services and was present on the construction site on a periodic basis to: 1. Review, for conformance to this code and the design concept, shop drawings, samples, As-Built Plan prepared by Control Point Associates, Inc., dated April 27, 2022 and other submittals by the contractor, as requested, in accordance with the requirements of the construction documents for the portions of the project for which Bohler has been contracted to provide Civil Engineering Services. 2. Our office had previously issued a certification letter on May 31, 2022 with some minor exceptions noted based on field observations. Since then, those exceptions have been corrected at the project site and removed from this letter for record. I, Zachary L. Richards, MA Registration Number: 51848, License Expiration Date: 06/30/2024, am a registered design professional, and I have prepared or supervised the preparation of all design plans, computations and specifications concerning site civil design for the above named project and that to the best of my knowledge, information, and belief such plans, computations and specifications meet the applicable provisions of the Massachusetts State Building Code (780 CMR) and accepted engineering practices for the portions of the project for which Bohler has been contracted to provide Civil Engineering Services,with the following exceptions: • One of the tenants added brand specific pavement marking to the site that was not a part of the approved design plans. Nothing in this document relieves the contractor of its responsibility regarding the provisions of the Massachusetts State Building Code (780 CMR 107)and this is not a field certification of ADA accessible route and parking compliance on the project site. I understand and agree that I (or my designee) performed the necessary professional services and was present on the construction site on a periodic basis to: 1. Review, for conformance to this code and the design concept, shop drawings, samples, As-Built Plan prepared by Control Point Associates, Inc., dated April 27, 2022 and other submittals by the contractor, as requested, in accordance with the requirements of the construction documents for the portions of the project for which Bohler has been contracted to provide Civil Engineering Services. 2. Perform the duties for registered design professionals in 780 CMR Chapter 17, as applicable for the portions of the project for which Bohler Engineering has been contracted to provide Civil Engineering Services. 3.
