The proposed onsite wastewater system design plan for the above site dated January 5 2024 has
peen approved for issuance ofa Disposal System Construction Permit (1.) C',P). Once issued, at
I)`(,',P will authorize the, construction of4thc onsite wastewater system Per the plan referenced
above and as allowed under Title 5 offthe Massachusetts L' nvironmental (:;code (310 C°VIR
15.000). You will need involvcnncnt frorn the designer ol`the onsite wastewater system
throughout the installation process scr YOU should be sure to coordinate with them.
The desigrn has been approved Ifar use in the construction of:a new can-site wastewater systern fear
a 4-bedrown (maxitnrum c)-roonn) home utilizing the proposed septic tank and a standard stone-
and-pipe sail absorption syst:ern as indicated on the design Plain.
This design plan approval is valid fear 2 years frorn the above date. I:)aaring this time, a licensed
septic system installer should obtain a hermit and compicte this work, and a Certificate, of
Compliance be endorsed by the installer, designer and the Town of North Andover. If'these
activities a.re, not conipleted within this tirnre period tinen it is possible a new design plan will need
to be completed and submitted for review'.
This approval is subJect to the following conditions. Please be sure to review these with the
designer and installer.

Prior to the issuance ot'a Disposal Systern Construction permit:
a, Three additional paper copies of the design plan are to be provided to the office
b. The area of the proposed Soil Absorption System (SAS) is to be staked and
flagged to identify the location(s) and to prevent activities which might darnage
the soil absorption system. Stockpiling off terials and equipment within the
SAS area is prohibited

d. Prior to the issuance ofthc Disposal System ''(.)nstrUCtiOn Pen-nit, the applicant
must submit the floor plans of dwelling proposed showing no greater than 4
bedrooms or a total of 9 roorns.

b, If groundwater is present, at the bottom portion of the excavation for the soil
absorption system (SAS) the excavation is to be dewatered prior to the placement

placement of any needed fill in the SAS., the bottom is to be scarified and
relatively dry and the fill material is to be stockpiled at the, edgc of the excavation
and filled in gradually
c. Inspections are to be performed by the designer of the onsite wastewater systern
as needed to properly oversee and document COTAStRICti011. At. a 111ininium these
are the initial excavation, prior to backfilling, and dUring, backt-illing as well as
any other component of the construction which the designer wishes, to examine
d. Inspections arc to be completed by the.Town of North Andover at a minimurn as
follows:
i. Bed bottom
ii. Tank, bottom
iiL Final installation of components, confirmation of proper component
selection, examination ofconstruction methodology, and systern operation
iv. Final grade
v. Sand fill will be collected From at least, one location and analyzed for
compliance with specifications in Title 5
e. 'No components are to be covered until a final component inspection has been
completed
f No excavation f6r or work associated with construction of the soil absorption
systern is to occur from November 30 to April I

la. If changes have been made to the approved plans then preparation of an as-built
plan is required to be completed by the designer. If no changes have been made
to the approved plans then the approved plan shall be modified by the designer to
show the distances froni a known Structure to the system components.
i. No sewage shall be discharged to the onsite wastewater system until a Certificate
of Corripliance has been issued by this office
No application for a Certificate of0ccupancy may be requested from the Building
Inspector Until a Certificate of'Conipliance has been issued by this office
In addition, YOU Should be aware of the long-term operation guidelines of an onsite wastewater
systern (these are not part ofthe DSCP but are mentioned below as information for the property
owner):
I. An orisite wastewater system can function as an integral part of the infrastructure of'
the property if it is properly operated and maintained. ]'his is the responsibility ofthe
owner of the property and is generally not specified by regulation. You are
encouraged to contact a wastewater system pumper/service provider to partner with
YOU to implement steps needed to assure the longevity of the onsite wastewater
system being installed at this location
1'he issuance of DSCP does not construe arid/or imply compliance- with any permits, approvals
or licenses which might be needed from other agencies such as the Conservation Commission,
Zoning Board, Planning Board, Building Inspector, Plumbing Inspector and/or Ellectrical
Inspector.
