In accordance with the provisions of Chapter 214 of the acts of 1974, also known as chapter 148, Section 268 G.L.
application is hereby made for installation of Fire Alarm System as provided for therein at the following described
premises.

It is agreed that the installation shall comply with the North Andover Fire Department Guidelines and provisions of
the most recent editions of National Fire Code Standard No. 101 relative to life safety and of National Fire Code Stan-
dard No. 74 relative to household Fire Warning Equipment adopted by the National Fire Protection Association, also as
specified by the most recent provisions of the Life Safety Code, National Fire Protection Association No. 101, and
Household Fire Warning Equipment, National Fire Protection Association No. 74,which requirements are incorporated
in such act and made part thereof and that upon Completion of such installation a statement of Completion shall be filed
with the North Andover Fire Department.
