Present at the meeting were as follows: Jean Enright —Town of North Andover(Planning Dept) Amy Maxner—Town of North Andover (Conservation Administrator) Nick Meninno—Meninno Construction (Site Contractor) Troy Spaulding—Meninno Construction (Site Contractor) Brian Dundon--RJ O'Corunell & Associates (Civil Engineer) The following topics were discussed and observations made: I. RJOC reviewed with the Town the Site Plan Special Permit Condition Nos 22 tl-uru 31 which pertain to certain requirements that need to be completed prior to the start of construction. RJOC and the Town are in agreement that the pre-construction conditions have been satisfactorily addressed with the exception of the following two conditions: • Condition No 25 —Town advised RJOC the performance bond in that amount of$10,000 has not be submitted. RJOC advised the Town that it will follow up with the Owner to have bond submitted ASAP, • Condition No 28 —Town advised RJOC that they are not in receipt of an executed copy of the Independent Monitor's Agreement. I'„ :, c,..' i« 1 ;u .l.,..N 2. Construction activities associated with the installation of new storefront concrete sidewalks has commenced. RJOC and the Site Contractor reviewed with the Town the means and methods of new sidewalk construction. Meninno Construction advised the Town they plan to install the new storefront sidewalks first followed by curb installation and depending on weather site grading, paving, landscaping and traffic control striping activities will be performed. 3. Meninno Construction confirmed with the Town that fill piles generated by the site work will be covered with a plastic tarp and a straw wattle barrier will be installed around its perimeter. Additional wattle barriers are on-site to mitigate any unforeseen condition. 4. The Town advised RJOC and Meninno Construction that in the event sitework construction is suspended due to weather the sitework construction schedule will need to be updated and submitted to the Town. RJOC and Meninno Construction indicated it would comply with said request. 5. At the conclusion of the pre-construction meeting, RJOC and Meninno Construction walk with the Town the areas in which sidewalk construction was being performed. No comments and/or concerns were received from the Town. RJOC has prepared this site inspection report as required under Special Condition No 31 of the Site Plan Review Decision. The writer has prepared this report based on site observations performed on Tuesday, June 23, 2020. 1) Activities performed as of October 8, 2019 • Concrete has been poured for new storefront sidewalk located in front of Bollywood Grill and the former Payless Shoe tenant space. • Existing concrete sidewalk has been sawcut and removed along the storefront of AT&T • Concrete has been poured for new storefront sidewalk located in front of the former Design Lighting & Electric tenant space and the southern corner of Kohls. • Approximately fifty (50+1-) if of existing concrete sidewalk within Kohl's store front area has been removed. • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile which was located in a remote section of the parking lot. Wattle barrier was in good condition and no additional erosion control measures are recommended at this time. 2) Activitiesp_erformed as of October 21, 2019 • Exposed aggregate ADA sidewalk ramp has been installed in front of Bollywood Grill tenant space. • New sidewalk installation has been completed in front of Gentile Dental and AT&T tenant spaces. Exposed aggregate ADA sidewalk ramp has been installed in front of AT&T tenant space. • New sidewalk installation and exposed aggregate ADA sidewalk ramp (approx. 25+/- If) has been installed in front of TJ Maxx entry area. • Approximately fifty (50+/-) If of new concrete sidewalk and exposed aggregate ADA sidewalk ramp has been installed within Kohl's store front area (southerly side). • Approximately fifty (50+/-) if of existing concrete sidewalk on the northerly side of Kohl's entry area has been removed. Area has been fine graded for concrete placement. • Approximately ninety (90+/-) if of existing concrete sidewalk on the southerly entry area to Market Basket has been removed. RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile and the stockpiles are covered with a plastic cover which is located within a remote section of the parking lot. Wattle barrier and plastic cover are in good condition and no additional erosion control measures are recommended at this time. 3) Activities performed as of October 28, 2019 • Approximately fifty (50+/-) if of new concrete sidewalk on the northerly side of Kohl's entry area has been installed. ' • Approximately ninety(90+/-) if of new concrete sidewalk and exposed aggregate ADA sidewalk ramp on the southerly entry area to Market Basket has been installed. • Existing concrete sidewalk from Olympia Sports to the Merrimack Valley Credit Union tenant space (approx. 100 +/- If) has been removed. Temporary ramps have been installed at the entry points to each tenant within the limit of work area. Work area has been barricaded with traffic cones and yellow caution tape. • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile and the stockpiles are covered with a plastic cover which is located within a remote section of the parking lot. Wattle barrier and plastic cover are in good condition and no additional erosion control measures are recommended at this time. 4) Activities performed as of November 05, 2019 • New concrete sidewalk from Olympia Sports to the Merrimack Valley Credit Union tenant space (approx. 100 +/- If) has been installed. ADA accessible sidewalk ramp needs to be installed. Temporary ramp has been installed at the location permanent ADA accessible ramp is to be installed. • Existing landscape island endcaps located in front of Olympia Sports and in front of Market Basket's northerly and southerly entry areas are currently being removed (see dwg G1A for location). • Existing landscape island endcaps located at the northerly corner of Kohl's and in front of Kohl's entry area are currently being removed (See dwg C-IA for location). • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile and the stockpiles are covered with a plastic cover which is located within a remote section of the parking lot. Wattle barrier and plastic cover are in good condition and no additional erosion control measures are recommended at this time. S) Activities perforined as of November 11 2019 • New concrete sidewalk from Olympia Sports to the Merrimack Valley Credit Union tenant space (approx, 100+/- If) has been installed. ADA accessible sidewalk ramp needs to be installed. Temporary ramp has been installed at the location permanent ADA accessible ramp is to be installed. • Existing pavement has been sawcut and removed for future landscape island cadcaps from Kohl's to Payless Shoes (see dwg C-3B & C-3C from limits of pavement removal). • Existing concrete sidewalk located in front of Market Basket's northerly entrance has been removed. Entrance has been temporarily closed to the public, Sidewalk removal extends to Wine & Beer at the Andovers tenant space, • Existing landscape island endcaps located along Market Basket's store front have been removed (See dwg no C-1 A for location) • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile and the stockpiles are covered with a plastic cover which is located within a remote section of the parking lot. Wattle barrier and plastic cover are in good condition and no additional erosion control measures are recommended at this time. G) Activities performed as of November 18, 2019 • New concrete sidewalk from Market Basket's northerly entrance to Wine & Beer at the Andovers tenant space has been completed. ADA accessible sidewalk ramp has been installed (See dwg C-3A for limits of sidewalk installation completed). It should be noted that the limits of new concrete sidewalk construction as shown on Dwg Nos C-3A—C- 3C has been completed. • Installation of sloped granite curb landscape island endcaps along the storefront tenants has commenced (See Dwg C-3A--C-3C for limits of curb installation). • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile and the stockpiles are covered with a plastic cover which is located within a remote section of the parking lot. Wattle barrier and plastic cover are in good condition and no additional erosion control measures are recommended at this time. 7) Activities perforned as of November 25, 2019 • Installation of sloped granite curb landscape island endcaps continuing along the storefront tenants (See Dwg C-3A--C-3C for limits of curb installation). • Existing pavement along the south side of Payless Shoes tenant space has been removed for future landscape island. • Existing curbed landscape islands installed to date have been patched with bituminous pavement to match existing pavement grade (See Dwg C-3A---C-3C for limits of pavement patch), • A bituminous pavement patch has been installed at the ends of the new sidewalk ADA ramps that have been installed (See Dwg C-3A-C-3C for limits of pavement patch). • Loam has been placed within the curbed landscape islands installed to date • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile and the stockpiles are covered with a plastic cover which is located within a remote section of the parking lot. Wattle barrier and plastic cover are in good condition and no additional erosion control measures are recommended at this time. 8) Activities performed as of December 2, 2019 • Installation of sloped granite curb landscape island endcaps have been completed along the tenant storefronts and the south side of building (Payless Shoe tenant space). See Dwg C-3A & C-3C for limits of curb installation installed the week on November 25'h. • RJOC observed that the erosion control wattle barrier that was installed around the perimeter of the construction laydown area has been removed. Laydown area has been cleaned of sand and sediment Note: Site work construction activities have been suspended due to weather conditions until the Spring of 2020. Final grading &paving, parking lot striping and signage installation will be performed in Spring of 2020. RJOC will notify the Planning Dept a week prior to work resuming at the site. �E E I E 9) Activities Derfornmd as of June 9I 2020 • Limits of pavement reclaiination, fine grading and base course pavement installation has been completed along the main access drive in front of the shopping center store fronts. • Installation of landscape trees along the parking aisle curbed endcaps located in front of j the tenant store fronts has commenced. • Metal bollards for ADA parking signage is currently being installed • RJOC observed that an erosion control wattle barrier was installed around the perimeter of loam pile which is located in a remote section of the parking lot. Wattle barrier appears to be in good condition and no additional erosion control measures are recommended at this time, 10)Activities performed„as of Jane 1.6,2020 • Limits of finish pavement installation has been completed along the main access drive in front of the shopping center store fronts. • Installation of landscape trees and shrubs within the parking aisle curbed endcaps located in front of the tenant store fronts has been completed • Installation of ADA parking stall signage has been completed • Installation of traffic control signage has been completed • Fire Lane, centerline and crosswalk striping has commenced and is approximately 30% completed • Environmental hoods have been installed on the outfall pipes of the existing catch basins located on-site. • Sitework construction has been substantially completed. Erosion control wattle barriers have been removed from the site. I 11)Activities performed as of June 23 2020 • All phases of site work construction associated with performing the ADA improvements have been completed. Site is deemed stable and all erosion control measures have been removed. Note: With the site improvements completed this will be the last site inspection report prepared for this scope of work. RJOC will prepare and submit an as-built plan to the Planning Dept, for the purposes of closing out the project and requesting release of the construction bond. RJOC advised the Town that it will follow up with the Owner to have bond submitted ASAP. • Condition No 28 —Town advised RJOC that they are not in receipt of an executed copy of the Independent Monitor's Agreement. Meninno Construction advised the Town they plan to install the new storefront sidewalks first followed by curb installation and depending on weather site grading, paving, landscaping and traffic control striping activities will be performed. 3. Meninno Construction confirmed with the Town that fill piles generated by the site work will be covered with a plastic tarp and a straw wattle barrier will be installed around its perimeter. Additional wattle barriers are on-site to mitigate any unforeseen condition. 4. The Town advised RJOC and Meninno Construction that in the event sitework construction is suspended due to weather the sitework construction schedule will need to be updated and submitted to the Town. RJOC and Meninno Construction indicated it would comply with said request. 5. At the conclusion of the pre-construction meeting, RJOC and Meninno Construction walk with the Town the areas in which sidewalk construction was being performed. The writer has prepared this report based on site observations performed on Monday,November 25, 2019. 1) Activities performed as of October 8 2019 • Concrete has been poured for new storefront sidewalk located in front of Bollywood Grill and the former Payless Shoe tenant space. • Existing concrete sidewalk has been sawcut and removed along the storefront of AT&T • Concrete has been poured for new storefront sidewalk located in front of the former Design Lighting & Electric tenant space and the southern corner of Kohls. • Approximately fifty (50+1-) if of existing concrete sidewalk within Kohl's store front area has been removed. • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile which was located in a remote section of the parking lot. Wattle barrier was in good condition and no additional erosion control measures are recommended at this time. 2) Activities performed as of October 21, 2019 • Exposed aggregate ADA sidewalk ramp has been installed in front of Bollywood Grill tenant space. • New sidewalk installation has been completed in front of Gentile Dental and AT&T tenant spaces. The writer has prepared this report based on site observations performed on Monday, October 28, 2019. 1) Activities performed as or October S 2019 • Concrete has been poured for new storefront sidewalk located in front of Bollywood Grill and the former Payless Shoe tenant space. • Existing concrete sidewalk has been sawcut and removed along the storefront of AT&T • Concrete has been poured for new storefront sidewalk located in front of the former Design Lighting & Electric tenant space and the southern corner of Kohls. • Approximately fifty(50+/-) If of existing concrete sidewalk within Kohl's store front area has been removed. • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile which was located in a remote section of the parking lot. Wattle barrier was in good condition and no additional erosion control measures are recommended at this time. 2) Activities verformed as of October 21, 2019 • Exposed aggregate ADA sidewalk ramp has been installed in front of Bollywood Grill tenant space. • New sidewalk installation has been completed in front of Gentile Dental and AT&T tenant spaces. Exposed aggregate ADA sidewalk ramp has been installed in front of AT&T tenant space. • New sidewalk installation and exposed aggregate ADA sidewalk ramp (approx. 25+/- 10 has been installed in front of TJ Maxx entry area, • Approximately fifty(50+1-) If of new concrete sidewalk and exposed aggregate ADA sidewalk ramp has been installed within Kohl's store front area(southerly side). • Approximately fifty(50+/-) If of existing concrete sidewalk on the northerly side of Kohl's entry area has been reproved. Area has been fine graded for concrete placement. • Approximately ninety(90+1-) If of existing concrete sidewalk on the southerly entry area to Market Basket has been removed. • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile and the stockpiles are covered with a plastic cover which is located within a remote section of the parking lot. Wattle barrier and plastic cover are in good condition and no additional erosion control measures are recommended at this time, 3) Activities Performcd as of Octoher 28,2019 • Approximately fifty (50+1-) If of new concrete sidewalk oil the northerly side of Kohl's entry area has been installed. • Approximately ninety (90+/-) If of new concrete sidewalk and exposed aggregate ADA sidewalk ramp on the southerly entry area to Market Basket has been installed. • Existing concrete sidewalk from Olympia Sports to the Merrimack Valley Credit Union tenant space (approx, 100+/- 10 has been removed. Temporary ramps have been installed at the entry points to each tenant within the limit of work area. Work area has been barricaded with traffic cones and yellow caution tape. • RJOC observed that an erosion control wattle barrier was installed around the perimeter of a fill/debris stockpile and the stockpiles are covered with a plastic cover which is located within a remote section of the parking lot. Wattle barrier and plastic cover are in good condition and no additional erosion control measures are recommended at this time.
