A. Meeting Minutes from November 16,2023 presented for signature. Motion made by Dr.Patrick
Scanlon for approval,seconded by Jennifer Abou-Ezzi,all in favor,minutes approved.(5-0-0)
IV. PUBLIC HEARING
A. Bral Spright,motions to open the public hearing,seconded by Dr.Patrick Scanlon. Laura Mesa Garcia
has come before the Board of Health to request a Body Art Regulation Variance. The North Andover
Board of Health requires two years of experience for a body art practioner,Ms.Mesa Garcia has three
years however she only has one documented licensed year. Ms.Mesa Garcia was trained as an
apprentice in Lexington,MA for two years starting in 2020. She opened her own business in 2022 in
Stoneham. At this time Stoneham did not have a director and receiving documentation has proved too
be difficult. She then transferred back to Lexington. Brian LaGrasse reached out to the Lexington
Board of Health for confirmation. She has all her training,coursework,certifications,for two years as
an apprentice but only a full license for one year. Dr.Patrick Scanlon would like to look at the policy
and request letters of recommendations when documentation cannot be provided. Dr.Patrick Scanlon,
motions to close the public hearing,seconded by Jennifer Abou-Ezzi.(5-0-0)

regulation to be modified for further improvement.(5-0-0)
V. OLD BUSINESS

The top three topics identified in the focus groups:

The topics are then broken down into objectives. The chip starts a year-long process for the objectives
to be reviewed and implemented. Brian has asked the board to review the plan and if anyone has
recommendations or questions to reach out. It will be posted on the website. The board will be
updated quarterly on the progress.
Vl. COMMUNICATIONS,ANNOUNCEMENTS,AND DISCUSSION
A. Virginie Kasiame Kamuene,MA,MS,ABA JECC Behavioral Center,LLC—See Appendix B.
JECC Behavioral Center presents a power point presentation to the Board of Health. JECC Behavioral
Center is a mental health and ABA(autism therapy)Services Center. They are located at 10 Main Street,
floor 2 and 3 North Andover,MA 01845. Ms.Kasiame Kamuene is the founder of the behavioral center,a
clinician for eleven years in North Andover and across the Merrimack Valley. She has two masters
degrees,and is Massachusetts State license eligible. She has been providing in home therapy,traditional
therapy,and school-based therapies. She works with the Department of Children and Families(DCF)to
refine and unite families. Ms.Kasiame Kamuene started the center because she has an autistic child. She
experienced a lot of health disparities,and had difficulty accessing resources. Chelsea Kamuene is the vice
president of the behavioral center,graduate of North Andover High School and has a masters degree in
social work from Boston College. Ms.Kamuene is committed to implementing systems within the
community level to provide better positive outcomes for all. JECC serves over 500 clients. JECC is a
versatile organization committed to offering services to a diverse population across the lifespan,including
children,youth and families throughout the Merrimack Valley and the Northeast Region. In 2024,JECC
will continue to help clients identify and address personal challenges. They will also strive to create
opportunities and expand access to mental health services. JECC will continue to support their clients
through every single step of the journey. The assessment is really meaningful to JECC because they are
located in town and provide these services. They hope to work together with the board of health and to
continue to impact within the community and the families served.Quote-"Let's work together to be
proactive and not reactive to our community needs."
B. Annette Garcia—Shared Services Coordinator
Annette has been with the department for six months. The Shared Services Commitee is halfway through
the fiscal year and is making some significant progress on the work plan. One big accomplishment is the
hire of part-time public health nurse,Lea Anne Pero. Currently,Lea Anne is partnering with the senior
center to develop a diabetes education pilot. Le Anne will develop an hour-long presentation with general
information on diabetes education which will target seniors. She will be able to perform blood sugar tests.
Once it is piloted it will be expanded to the other six communities. Lea Anne is also working on a Friends
and Family CPR class in Haverhill. It will be piloted then it will become available to all six communities.
Annette is working on a regionalized plan for permitting food trucks. Currently,there is a subcommittee
developing policies and procedures where the regional inspector would be the one to do a plan review,the
initial inspection and then the truck will be cleared and ready to get a permit from any of the six
communities in our collaborative. Annette is also working on developing a model body art regulation that
can be adopted by various boards for review and integrate any changes into their existing regulations. The
Commonwealth has amended the grant which has raised funds to$532,000 per year. These monies will
allow the communities to receive the same level of Public Health Services and fill any gaps that have been
identified.
C. FDA Grant Award and Approval Request—See Appendix C.
Annette Garcia-Shared Services Coordinator
One of the objectives for the public health excellence grant was to work towards the FDA retail food
standards. This is a set of nine standards that the FDA has developed to help local authorities create
stringent and effective food safety programs. One goal as a region is to start meeting these specific
standards. In advancement of this,Annette applied for the NIHA FDA Retail Flexible Funding Model
Grant. It was awarded for the 2024 calendar year. This award was for$226,500 that can help us work
towards meeting those retail standards. If the funds are accepted by the select board,the funds will support
three main goals. One,to complete a self-assessment and comprehensive strategic improvement plan. This
will see how well the Town of North Andover is currently meeting the standards. Most applicants meet very

few if any of the standards when they first start but it helps to establish the baseline. Then,a comprehensive
plan will be made to improve and meet the standards in the future. The program is aimed to be in a five-year
cycle. It will allow for new policies,new programs and procedures. The second part is to opt into the
mentorship program. North Andover has been matched with a health department in North Carolina who has
already completed and met all nine of the standards. This grant will allow the opportunity to do a site visit
in North Carolina and for them to come to North Andover to observe our current food safety programs. Our
goal this year is to fully meet program standard two which relates around trained regulatory staff. We will
develop a standard training plan and a verification plan to train food safety staff. The mentorship program
will help establish those guidelines. The third part is to send two staff members,(Brian LaGrasse and
Shanene Pierce)to a self-assessment and verification audit workshop which is hosted by the FDA. The
workshop teaches how to be part of the program and continue it on past the mentorship. It is a great
opportunity to learn. The grant allows the department to offer the highest level of food safety protection for
the residents of North Andover.
D. Next meeting the annual agents of the Board of Health will be appointed.
E. AED program is moving forward. AEDs will be placed at the playing fields and playgrounds across
North Andover. The fire department will be in charge of inspections. A video will be completed on
how to use the devices.
F. Carolyn Lam—In December the influenza,Covid-19,and RSV numbers spiked. There were roughly
186 cases of Covid-19 reported. This week were only five. Flu shots,pediatric Covid-19 vaccine and
test kits are available through the Health Department. Covid-19 test kits are also available through
USPS.

Hello Brian,
I'm looking to get a variance on the two years of experience the town requires for permanent make up. I was only licensed
in the town of Lexington for just one year. I do have over two years of experience but before being in Lexington, I was in
Stoneham and the director was no longer with the department. My application was lost then found and was not given the
same information I originally was given from the previous director.That is why I moved my business to Lexington where I
let their rules ®ulations. I have submitted the paperwork through the portal and feel like I meet all of the requirements.

Letter from North Andover Health Department
Dear North Andover Community,
We are pleased to present the 2023 Andover Community Health Improvement Plan (CHIP). The
CHIP is the culmination of a rigorous ten-month process that the Towns of Andover and North
Andover entered together. In the fall of 2022, an Advisory Committee of community members
and municipal staff began meeting to guide a Community Health Needs Assessment (CHNA).
The CHNA process included reviewing existing data and collecting new data through a survey on
community health assets and needs, interviews with community partners, and focus groups
with identified populations. The CHNA uncovered several key themes about the health of our
community.
Together we identified three priority areas to focus our collective efforts: Mental Health Across
the Lifespan; Affordable Housing; and Diversity, Equity, and Sense of Community. Each priority
area has a series of objectives, accompanied by metrics and actionable strategies, which
provide achievable health improvement for North Andover. We urge residents, town staff,
businesses, and nonprofit organizations to review the goals, objectives, strategies, and action
steps outlined in this plan to determine how we can work together to support these efforts and
improve the health and wellbeing of all those who work and live in North Andover.
We are grateful to the Advisory Committee and planning participants listed in the appendix
who contributed many hours creating this plan. We express our sincerest appreciation to
everyone who contributed their thoughtful input.
We look forward to developing partnerships to address this collective work over the next three
years.

Introduction
Background
A community health improvement plan, or CHIP, is an action-oriented strategic plan that outlines the
priority health issues for a defined community and how these issues will be addressed, including
strategies and measures, to ultimately improve the health of the community.The North Andover Health
Department, or North Andover, led its first comprehensive community health improvement process to
measurably enhance the health of the North Andover community.
In the fall of 2022, North Andover partnered with the Andover Health Department, or Andover, to
contract with Health Resources in Action, Inc. (HRiA)to develop a shared Community Health Needs
Assessment(CHNA) and facilitate two subsequent CHIP processes.This partnership between the two
towns provided an opportunity to pool resources and collaborate on the towns'first community health
improvement processes.
The CHNA process included both qualitative and quantitative data collection as well as an analysis of
secondary data.The data collection methods for the assessment included a survey on community health
assets and needs, key informant interviews with key community partners, and focus groups with
identified populations.
Following the development of the CHNA, HRiA worked with town leadership in the spring of 2023 to
guide and facilitate the CHIP planning process. The CHIP utilized a participatory, collaborative approach
guided by the Mobilization for Action through Planning and Partnerships (MAPP) process.' MAPP, a
comprehensive, community-driven planning process for improving health, is a strategic framework that
coalitions and local public health departments across the country have employed to help direct their
strategic planning efforts. MAPP comprises distinct assessments that are the foundation of the planning
process and includes the identification of strategic issues and goal/strategy formulation as prerequisites
for action. Since health needs are constantly changing as a community and its context evolve, the
cyclical nature of the MAPP planning/implementation/evaluation/correction process allows for the
periodic identification of new priorities and the realignment of activities and resources to address them.
Purpose of a Community Health Improvement Plan
CHIPS are created through a community-wide, collaborative planning process that engages partners and
organizations to develop, support, and implement the plan. A CHIP is intended to serve as a vision for
the health of the community and a framework for organizations to use in leveraging resources, engaging
partners, and identifying their own priorities and strategies for community health improvement.
The CHIP provides an essential framework for guiding future services, programs, and policies for
community agencies and organizations in the area.
This CHIP is designed to:
• Identify priority issues for action to improve community health
'Advanced by the National Association of County and City Health Officials(NACCHO), MAPP's vision is for
communities to achieve improved health and quality of life by mobilizing partnerships and taking strategic action.
Facilitated by public health leaders,this framework helps communities apply strategic thinking to prioritize public

How to Use the CHIP
A CHIP is designed to be a broad, strategic framework for community health, and should be modified
and adjusted as conditions, resources, and external environmental factors change. It is developed and
written in a way that engages multiple perspectives so that all community groups and sectors—private
and nonprofit organizations, government agencies, academic institutions, community and faith-based
organizations, and citizens— can unite to improve the health and quality of life for all people who live,
learn, work, and play in North Andover. People and organizations should review the CHIP's priorities and
goals, reflect on the suggested strategies, and consider how to participate in this effort, in whole or in
part.
Relationship Between the CHIP and Other Guiding Documents and Initiatives
The CHIP was designed to complement and build upon other guiding documents, plans, initiatives, and
coalitions already in place to improve the health of North Andover. Rather than conflicting with or
duplicating the recommendations and actions of existing frameworks and coalitions, the participants of
the CHIP planning process identified potential partners and resources wherever possible for inclusion in
this CHIP.
Community Engagement
To develop a shared commitment and plan for improved community health, and help sustain
implementation efforts,the planning process engaged community members and local public health
partners through different avenues:
The Advisory Committee was responsible for contributing to the development of the
community health improvement plan.
The CHIP Workgroups, representing broad and diverse sectors of the community and organized
around each priority area, were responsible for developing the goals, objectives and strategies
for the CHIP.
Additional stakeholders were invited to share input and feedback on the draft plan components as they
were being developed.
Development of Data-Informed, Community-Identified Health Priorities
Priorities for the community health improvement plan (CHIP) were identified by examining data and
themes from the CHNA findings using a systematic, interactive approach.The following themes emerged
most frequently and were considered in the selection of the CHIP health priorities:

• Older Adults and Aging Health Concerns
• Mental Health, including Youth Mental Health

At a Prioritization Session held via Zoom in February 2023, 10 members of the Advisory Committee
reviewed the key findings of the community health needs assessment and engaged in a facilitated
discussion of the findings.
Participants were then guided through the use of an online multi-voting tool to identify their three top
choices for CHIP priorities,taking the following selection criteria into consideration:

Following the meeting, the Prioritization Session Presentation and prioritization voting tool were shared
with the Advisory Committee, and an additional three members provided input.The voting results were

3 In discussion of the priorities, it was noted that both towns had recently passed Complete Streets ordinances that
would facilitate the improvement of motor vehicle and pedestrian safety and improve access to active
transportation.

As a result, Mental Health Across the Lifespan, Affordable Housing, and Diversity, Equity, and Sense of
Community were selected as the priority areas for the 2023 CHIP.The following table contains a list of
potential topics for objectives under each priority based on the assessment findings.

Community Making North Andover a More Welcoming Community
for People of All Identities
These three priority areas were the focus of virtual planning sessions conducted to identify goals,
measurable objectives, outcome indicators, and strategies to address these issues. North Andover
leadership had a strong desire to focus the efforts of this CHIP on the root causes, or Social
Determinants of Health, and on the policies, systems and environmental factors that contribute to the
health issues that emerged from the assessment.This focus is reflected across the elements of this plan.
Development of the CHIP Strategic Components
Planning Model
Planning for the CHIP took place virtually via Zoom. North Andover leadership invited Advisory
Committee members as well as additional community members and partners who work in areas
relevant to the plan's three priority areas.
Planning participants from both towns were invited to two 2-hour joint Capacity Building Sessions
facilitated via Zoom.The purpose of these sessions was to build relationships between participants,
develop a shared language for equity and planning, and ground the planning work in a systems change
framework.
Following the Capacity Building Sessions, North Andover planning participants were invited to a 1.5-hour
Pre-Planning session facilitated via Zoom. In this meeting, participants were prepared for the planning
sessions. HRiA shared the CHIP planning framework and planning process and discussed the role of and
expectations for planning participants.
After both the Capacity Building and Pre-Planning Sessions, four virtual planning sessions were held
between March and April 2023.The sessions ranged in length from 1.5 to 2.5 hours and were structured
in both small and large group formats to develop plan components (goals, objectives, potential outcome
indicators, strategies, and potential partners).Sessions were facilitated by HRiA and included

opportunities for cross-priority feedback and refinement of each of the core elements of the CHIP. Eight
people participated in the Planning Sessions. See Appendix A for a list of CHIP planning participants.
Following the planning sessions, HRiA consultants reviewed the draft output from the workgroups and
edited the plan components for clarity and consistency. North Andover CHIP leadership shared the CHIP
with the Advisory Committee, subject matter experts, external partners, and other key stakeholders for
input and feedback.The input and feedback were reviewed and has been incorporated into the final
version of the CHIP where appropriate. See Appendix B for a list of Advisory Committee members.

The following pages outline the Goals, Objectives, Outcome Indicators, Strategies, and Potential
Partners for the three priority areas outlined in the CHIP. See Appendix C for definitions of these
planning terms. See Appendix D for a list of Acronyms used in the CHIP.

Next Steps — Implementation Phase
The components included in this report represent the strategic framework for a data-informed,
community health improvement plan. Given the unpredictable nature of changing resources, priorities,
and events, planning participants examined the CHIP's 46 strategies and prioritized a subset of 20
strategies to start implementing in the first year of the CHIP. When thinking about which strategies to
prioritize for the first year of implementation, planning participants considered:
1. Is the strategy foundational? Does it need to happen first, before other strategies?
2. Is there already momentum for the strategy?
3. Is it feasible to start the strategy in the coming year?
Individual responses were tallied, and 20 strategies were prioritized to initiate in the first year of the
CHIP. Planning participants developed a Year 1 Implementation Plan that includes: prioritized strategies
and specific 1-year action steps with timelines, identified lead responsible parties and resources for each
prioritized strategy. The implementation plan includes monitoring and evaluation processes and
procedures to ensure that successes and challenges are captured on an annual basis. Working groups for
each priority area, comprised of partners, stakeholders, and community participants, are responsible for
this annual process for implementing and updating the plan.

Re: FDA Grant Award and Approval Request
The North Andover Health Department has applied for and been awarded the 2024 NEDA-FDA Retail
Flexible Funding Model Grant. The Department has been awarded a total of$26,500 to support our goal
of meeting the Food and Drug Administration(FDA)Voluntary National Retail Food Regulatory
Program Standards. The Town will not incur any direct costs as a result of accepting this grant.We ask
the Select Board to approve the acceptance of these grant funds.
By the end of 2024,the Health Department will utilize these funds to complete the following activities:
• Complete a self-assessment and comprehensive strategic improvement plan to understand how
well the Department is currently meeting the retail program standards and establish a plan to
improve our ability to meet them in the future.
• Work with an assigned Mentor who has already met all nine(9)Retail Program Standards.
Through this partnership we will be able to learn from their best practices and fully meet Retail
Program Standard 2: Trained Regulatory Staff by the end of 2024.
• Send two staff members to a self-assessment and verification audit workshop hosted by the FDA.
During this training, staff members will learn how to complete future assessments of our ability to
meet the Retail Program Standards and develop program plans for future advancement.
The acceptance of these funds will aid the Health Department in protecting residents of North Andover by
supporting a robust and standardized food protection program.

The grantee must complete the full scope of work and all tasks outlined in the approved grant application by

Reports with due dates will be accessible by logging into the Grant Portal,found on the NEHA-FDA RFFM

Interim Progress Reports will be required each year for awards made through this program to assure that each
funded project remains on track for timely completion.For one-year awards,an Interim Progress Report will
be due halfway through the project period.
When all project objectives have been completed,a Final Project Report must be submitted through the
online grant portal no later than 45 days after your Project End Date. As part of the final report,the grantee
must provide a full accounting of all expenditures made with funds from this grant award accompanied by the
required documentation.

Reimbursement Requests
For one-year awards made through this grant program,payment is normally made on a reimbursement basis

Base Grant Requirement
Once awards under the NEHA-FDA RFFM Grant Program have been made,all grantees must have an active

remain eligible for open Optional Add-On Grants.During the performance period of open awards,if a Base
Grant is cancelled for any reason (at the request of the Grantee or due to non-performance),all open
Additional Add-On Grants may also be in jeopardy of cancellation.
For grantees that have been awarded both a Track 2 Development Base Grant(a one-year award) and a
Capacity Building Grant(a three-year award),it is the awarded jurisdiction's responsibility to apply for
Development Base Grants in years two and three of this grant program,to assure that their Capacity Building
Grant remains eligible for continuation.
