As $1,515.63 currently remains in the Project Review Fee Account from the preliminary review of the subdivision and a total of$3,000 is deemed adequate to review the site, the Board agreed to reduce the Site Plan Review Project Fee to $1,500 and to roll over the existing $1, 515.63 into the Definitive Plan Project Review Fee. As you are aware the Planning Board has the power,under Section 4.4(C) of the Fee Regulations, to require a supplemental fee if the amount in these accounts should fall below 25% of the initial fee. There have been no material changes in the plan, although appropriate additional detail and. corrections have been added. Roadway construction and drainage are not changed from prior approvals issued for a substantially larger development plan approved as plan#11761 on 3/19/90, The state DPW permit #5-20405 is attached for reference regarding construction of the existing facilities at Orchard Hill Road. The industrial zone line has been shown on the plan Lb. The subdivision roadway drainage is only that which was previously approved, or less, and all lots developed will have on-site controls. 1.c. The water supply has been shown on the plans 2,a. The jog in the ROW has been eliminated by providing a strip of land to the abutter to align the road 2.b. The radius at Osgood street is not provided due to the shape of the parcel, per previous approval 2.c. The grade'at the intersection with Osgood Street is +-5%, which exceeds required grades but is consistent with the previously approved plan specifying 4.74% 2.d. Adequate aggregate shoulder width is provided, but if further development is proposed on this site the roadway will be re-aligned slightly to fully comply. This is consistent with the intention of trying to minimize site and infrastructure disturbance for such a small project. 2.e. Constuction of a sidewalk for the extension of this roadway 190' does not seem consistent with safety concerns since it might encourage pedestrian traffic toward Osgood Street which has no sidewalks. As with other projects, we are receptive to a donation to the sidewalk fund for the equivalent 190' of roadway at the DPW bonding rate of$21.00 per foot ($3,990.00)if required. 3.a. Waiver for pavement width accepted 3.b. Waiver of construction of cul-de-sac, extension of paving to end of Parcel I-2A, and construction of circular driveway on Parcel I-2A accepted. To address the checklist, below is are itemized response. Please acknowledge that we consider this to be a Modification to an existing plan. Most issues were favorably addressed through the Preliminary Planning process a few months ago. 1. We (Tom Z., Kathleen Colwell, Bill Scott) discussed to satisfaction the status of the historical permits, and that the drainage sufficiency using the previously approved plan details would be confirmed or revised according to the Mass. State DPW, We also confirmed that the work at the intersection was done within the time constraints provided by the permits, and subsequently revised/reworked to satisfy State DPW concerns. 2. All erosion control plan was submitted both in plan and narrative form. A reserve stock of(20)'hay bales and 100' of silt fence will be maintained on site for emergency needs. 3. A Professional Engineer will sign and stamp the final plans. 4. The legend has been revised and enhanced. 5. The pavement radius at Osgood Street is an existing condition previously acknowledged and accepted in the Preliminary Review process. We have indicated an approximation of its existing radius Oil the plan. 6. The original submission indicated in narrative form that the property was entirely wooded, and therefore there was no need or method of indicating that on the plans. 7. The existing developed lot is not material to this subdivision. It has extensive frontage on Route 125, and has been developed and operating for many years. We have no right to acquire or obligation to present the topographic information of that property. 8. The road profile has been fully detailed. Again, this is an existing condition which, regardless of technical details, was acknowledged as acceptable during the Preliminary review process. A benchmark has been added to the plan. I do not agree that it is difficult to distinguish existing and proposed structures, they are all clearly marked in the submitted plan and profile details. 9. The street cross-section now complies with town standards, The cross-section originally provided reflected the agreed cross-section which was approved at the Preliminary stage. . 10. MVFCU site is not material to this project or application. (see also Item 7) 11. All abutters are noted as required. Barker property is contiguous and extensive, and cannot be completely shown without multiple additional plan pages, which would be unnecessary. 12, It is our understanding that projects of this size and nature typically do not trigger a request for a Statement of Environmental Impact. 13. This waiver is no longer requested. 14. No ROW radius is provided at Osgood Street because existing parcel layout does not allow.it. This was acceptable in previously approved plans as well as the Preliminary submission. Furthermore, the pavement radius proposed in this modification is not affected by the shape of the ROW here, and there would be no access or utility issues involved with this layout. IS. This waiver is no longer requested, 16. The grade at Osgood Street cues comply with grade and leveling requirements; the detail has been revised to reflect these conditions in more detail. 17. We have provided a design for a standard cirl-de-sac. 18. This waiver is no longer requested, 19, The paving will comply with town standards in this regard (2.5" binder'). 20:No waiver is being requested, The plans have been revised to blend existing edge of pavement on the Forth side to new pavement to accommodate the acceptable shoulder of 8' as the roadway passes in front of the Lot I2-A, 21. Yes. 22. No street trees are proposed. Extensive plantings are required through the Site Plan Review process on each lot, and it is my understanding that NADPW would prefer not to have to maintain additional plantings in the ROW of industrial projects. 23. Cable utilities are'the jurisdiction of the utility companies themselves, and they determine proper layout and design. We have shown proposed facilities, although they are subject to the above. The Planning Board regulations dictate that these elements need only be presented to DPW prior to installation, not provided at this time. 24. The fire hydrant has been moved back to the ROW, therefore no easement is required. 25, Details of the proposed water facilities which comply with Town specifications have been added to the plans. 26. All site development drainage is being dealt with on the individual site through the Site Plan Review. Drainage infrastructure previously approved and partially installed is being completed as part of this Modification and, per discussion with Kathleen Colonel and Bill Scott after review of existing permits, . will be confirmed for continued acceptability with the State DPW, It is worth noting here that the i Y contribution to the state drainage facility proposed by these revisions,reflects dramatic reductions in stortnwater contribution to the state system versus the original plan and permits, and we therefore expect no objection through State DPW. 27, Monuments and benchmarks Have been added to the plan. 28. Inverts at the proposed manhole and catchbasins were already noted in the profile; we have now added them on the plan page but this request is contrary to our practice and all previous work presented to the Town. 29, Pipe material was already noted on the profile;we have now added it to the plan page but this is contrary to our practice and all previous work presented to the Town. 30, The sewer notation has been removed from the roadway cross section detail,as all properties are to be serviced by SSDS. 31. Handicapped ramps and retainers are not part of the subdivision submission; appropriate details will be provided to the Building Inspector at the time of Building Permit Application. 32, The grade line at elevation 90/92 has been corrected. I believe this response completely addresses the checklist from John Chessia. In summary, we have changed the roadway to a standard 40' paved section and complied with all other relevant Town design standards. Additional information has been added to the plans to reflect it] more detail the existing conditions,as well as clarify some items requested by John Chessia regarding proposed improvements. We intend to work with the existing conditions on the roadway, bleeding the new improvements to accommodate a functioning, stabilized, and beautified but substandard existing condition. Our goal is to assist you in reaching street acceptance with a completed project.In going through our files, we jioticed that you have documentation required to be submitted to our department to maintain compliance with the Town of North Andover Subdivision Regulations and the Notice of Decision issued relative to your project. In the absence of this information, you will be unable to get your street accepted. On the following page, please find a detailed list of information we will need in order for you to get your street'accepted. The list may refer to conditions in your Notice of Decision, which we are attaching for you to easily refer to. Additionally, I would suggest that you set up a meeting with myself, so that I may explain to you what is left in your subdivision to be completed. At that time we can also schedule an inspection of your subdivision by the Department of Public Works at a mutually convenient time in order for you to be ready for street acceptance. This list is only a preliminary review, and there may be more items outstanding that need to be completed once an inspection.has been conducted.
