(the "Applicant"), I, Julie Vondrak, serving as the Erosion Control Monitor ("ECM") am submitting this monthly monitoring report for weekly construction activities conducted within the Buffer Zone to wetland resource areas on the above referenced site. The following activities were documented during my weekly site inspections and after several rain events on September 2, 9, 13, 17, 23 &30,2021. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings,a clubhouse,parking areas,a pool,retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 2S-foot No-Disturbance Zone and SO-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACU) issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of August 29,2021 (September 2,2021 inspection) An inspection was conducted by the ECM on Thursday,September 2, 2021. The inspection was conducted during a rain event following the previous evening's heavy rain event. The access road has not yet been paved from the installation of the water line. The catch basin silt sacks were filled with water during my inspection and silt build-up was observed around the catch basin grates. Plumbing utility work continues on Building B. Grading activities on the slope behind B have begun. Half of Building A footprint has been excavated.The majority of all utilities,except gas have been installed on site. Minor surface runoff behind building B was observed flowing into the field through the erosion controls. Several areas of the northwestern erosion controls need reinforcement and in the near future several areas may need replacement. Ponded water was observed at the base of the large retaining wall in the western section of the site. The water was being contained by the erosion control barrier. Water flow existed only within the existing drainage channel and no water flow over grassed areas outside of the channel was observed at the time of my inspection. The most southwestern corner of the site appeared to receive some stormwater flow from the recent rain event(see photo 4 attached),as apparent from accumulated debris against the erosion control limit. Some minor silt build-up was observed within the grassed area south of the drainage channel, indicative of water flowing over the erosion control barriers during the night rain event. Several areas of the southern erosion control barrier have accumulated silt to the top of the silt socks and should be cleaned and/or replaced in the upcoming weeks. The proposed new drainage line to accommodate the Prescott Street overflows is being presented before the Conservation Commission on September 8,2021. Approval of this new drainage line will accommodate the stormwater runoff from Prescott Street that has resulted in the excessive stormwater overflow through the site. Summary of Construction Activities: Week of September 6, 2021 (September 10, 2021 inspection) An inspection was conducted by the ECM on Friday, September 10, 2021. The catch basins within High Street were inspected and contained some standing water within the silt sacks. There continues to be sediment around the catch basin grates and the contractor was notified to remove the silt build- up. The site was muddy due to the continuous rain events and ponded water continues to be pumped and collected within the Building A footprint area. Water is settling within this area and then being pumped into the manholes. There was no flow observed within the drainage swale in the western section of the property. Stone is being applied within the Building B footprint in preparation of the pouring of the slab (see photo S attached). The area north of Building B and C has been graded/cut and stone has been applied in preparation of construction of the vertical retaining wall (see photo 6 attached). The recently proposed new 36-inch PVC pipe drainage design was approved by the Conservation Commission this week. This pipe will run through the central section of the site and will convey the Prescott Street stormwater overflow through the western retaining wall. Work is anticipated to commence next week. The perimeter of the erosion control barrier was walked in its entirety.As stated in last week's report (details above), several sections of the erosion controls need to be cleaned/repaired or replaced and the contractor has been notified as to which areas need immediate attention. There is only 1 small area of silt breakthrough in the northern section of the site and the Site Contractor has been notified to remove the silt by hand and reinforce erosion controls in this area. Inspection following rain event:Monday, September 13, 2021 An inspection was conducted by the ECM on Monday, September 13, 2021 following a Sunday evening rain event. The perimeter of the site was walked in its entirety. No flowing water was observed throughout the site but large areas of standing water existed, especially in the dewatering containment area within the vicinity of the Building A footprint (see photo 9). A low flow of water was observed within the drainage channel just west of the western retaining wall. Pictures of erosion control areas that need reinforcement and replacement were sent directly to the Site Contractor,who ensured the areas would be addressed this week. Summary of Construction Activities: `Meek of September 13, 2021(inspection September 17, 2021) An inspection was conducted by the ECM on Friday, September 17, 2021. Construction activities this week included installation of the new 36-inch pvc drainage pipe through the center of the property. Approximately one-third of the length of pipe has been installed and the new pipe has been installed through the opening of the western wall (see photo 10 attached). Additional rip-rap has been installed within the grassed area at the outlet of the new pipe. Building B slab has been poured and is close to completion. The retaining wall north of Building C has been installed (see photo 11 attached). The erosion controls have been reinforced and silt build-up has been removed from a majority of the deteriorated areas. A majority of the silt breach beyond the chain link fence north of Building B has been removed. I spoke with the Site Contractor regarding an additional area where the silt should be further removed. No dust issues were observed during my inspection.All catch basins were inspected and the silt sacks were properly functioning. The accumulated silt build-up around the catch basin grates on High Street has been removed. The entire perimeter of the erosion control barrier was inspected and remains in-tact. As stated above, many sections of the silt sock barrier have been replaced, cleaned and reinforced. They will continue to be monitored and the Site Contractor will be notified when sections must be replaced. The roadways continue to be cleaned at the end of each day. Summary of Construction Activities: Week of September 20, 2021 (inspection September 23, 2021) An inspection was conducted by the ECM on Thursday, September 23, 2021. There were no rain events this week and the site was dry. Installation of the 36-inch drainage pipe throughout the site is close to completion and will be on-line as soon as installation is complete. The western wall has been closed up where the new pipe was installed (see photo 14). Minor underground utility work throughout the site continues.The shafts within Building B are being constructed.Underground utilities within Building C continue to be installed.The site is anticipated to be paved over the next week. Erosion controls along the perimeter of the site remain in-tact and will continue to be monitored. Summary of Construction Activities: Week of September 27, 2021 (inspection September 30, 2021) An inspection was conducted by the ECM on Thursday, September 30, 2021. Installation of the 36- inch drainage pipe throughout the site is completed and the system is currently on line. Construction of the elevator shaft and stairs within Building B is underway. Utility installation including the sewer lines continues within Building C. National Grid has removed the primary overhead wires running throughout the site. All catch basins were inspected and silt sacks remain in place and are properly functioning. Erosion controls along the perimeter of the site were walked in their entirety and remain in-tact and will continue to be monitored. A soil stockpile north of Building B is slightly impacting a small section of the silt socks and materials should be removed from this section of the erosion controls (see photo 17 attached). Replacement of a section of the southern erosion control barrier may need replacement before winter conditions arrive. Catch basins The silt sacks within the S existing catch basins,three (3) on High Street and two (2) within the access road into the site,were inspected during each weekly inspection. During my September 2, 10, 13, 17, 23 & 30 inspections, the catch basin silt sacks were observed in- tact and properly functioning. Standing water was observed within them during several of the inspections, as they were conducted during or following a rain event. As stated above, during several of my inspections,there was silt accumulation around and within several of the basins on High Street which has been addressed by the Site Contractor throughout the month. Erosion Controls The perimeter of the erosion control boundaries was walked/inspected in its entirety during each of my inspections. Several areas of the erosion controls have deteriorated due the excessive rains this season and the Site Contractor was notified as to which areas needed immediate attention and replacement. The barriers continue to prevent sedimentation breakthrough. The one low area in the north continues to be reinforced and the small area of silt breakthrough continues to be cleaned. The erosion control boundaries will continue to be inspected and sections will be replaced with the green mulch socks when necessary or as instructed by the ECM. DEP Signs The DEP sign is posted at the entrance to the construction site on the chain link fence and remains in- tact. Soil/Stone stockpile Areas Gravel and soil stockpiles were observed stockpiled throughout the site during my September inspections,however,all piles appeared stabilized with no signs of runoff observed. Pavement Sweeping/Dust Control The entrance roadway continues to be swept on a daily basis and was documented during my September 2, 10, 13, 17, 23 & 30 inspections. The interior of the site and entrance roadway is expected to be paved in early October. No dust within the site was observed during any of my inspections, particularly due to the ongoing excessive rain events and muddy conditions on site over the last month. The water truck is kept on site daily and continues to actively water vehicle construction access routes when necessary. Ongoing Recommendations/Comments ® Inspect and maintain the erosion controls around the perimeter of the site. Deteriorated sections will continue to be identified and replaced prior to the winter freeze. ® Inspect and the clean the catch basin silt sacks when necessary and remove sediment build-up around the southwestern catch basin on High Street weekly. ® Continue to implement dust control measures daily on within the site and the access roadways to prevent dust. The following activities were documented during my weekly site inspections conducted on August 5, 12, 19 and 26,2021. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings,a clubhouse,parking areas,a pool,retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and SO-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACU) issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of August 2,2021 (August 5, 2021 inspection) An inspection was conducted by the ECM on Thursday,August S, 2021. The inspection was conducted during a rain event.Building B &C foundation work is completed. Foundation B is being backfilled. Internal water and plumbing utilities within Building B will start soon.I walked the entire perimeter of the erosion control barrier.There continues to be accumulated silt build-up against several sections of the silt sock barrier,however,the barrier continues to function properly and the erosion control barrier is preventing silt breakthrough. The breached area in the northern section continues to hold a small amount of standing water and will be evaluated when dry to determine if additional silt needs to be removed. All catch basins were inspected.The southwest catch basin on High Street continues to accumulate silt around the opening,which should be removed. Summary of Construction Activities:Week of August 9, 2021 (August 12,2021 inspection) An inspection was conducted by the ECM on Thursday, August 12, 2021. The main site construction activities underway this week included ongoing drainage installation infrastructure (see photo S attached) and installation of the water pipe utilities within the Building B foundation footprint (see attached photo 4). The water line within the access roadway to the site is being installed (see attached photo 6 attached). All catch basin silt sacks were inspected and were free of debris. Sand material continues to accumulate around the southwestern catch basin on High Street and the Site Contractor was notified to continue to clean around this basin. The roadway entrance was in need of sweeping at the time of my inspection and the contractor stated it would be fully cleaned at the end of the day.No dust within the site was observed during my inspection. The erosion control barrier was walked in its entirety and no sedimentation beyond the erosion control line was observed during my inspection. The breached area in the northern section continues to hold a small amount of standing water and will be evaluated when dry to determine if additional silt needs to be removed. A dewatering pipe was observed in the northern section of site adjacent to the erosion control barrier (same pipe as observed in prior site inspections). Dewatering activities were not being conducted at the time of my inspection,however,the Site Contractor was immediately notified to remove the pipe entirely and was reminded no dewatering activities are allowed within 100-feet of the wetland resource areas. Summary of Construction Activities:Week of August 16,2021(inspection August 19, 2021) An inspection was conducted by the ECM on Thursday, August 19, 202 following a rain event. Underground plumbing utility installation continues within Building B (see attached photo 7). The Project Engineer has revised the drainage plan to include installation of a 36-inch PVC pipe within the site to handle the drainage flows into the site from Prescott Street.The design has been submitted to the Town for review and approval. Ponded water was observed within the catch basins on High Street due to the recent rainfall. Some sediment around the catch basins openings was observed and should be removed weekly. The entire perimeter of the erosion control barrier was inspected and remains in-tact. Some sections of erosion control are exhibiting deterioration from the ongoing rain events. The southern barrier is starting to accumulate silt build-up and should be replaced in the early fall months (see attached photo 9). Entrance roadway continues to be swept by hand and the water truck continues to actively water the construction site when necessary. Given the recent rain, no dust was observed within the site during my August 19, 2021 inspection. Summary of Construction Activities:Week of August 23,2021(inspection August 26,2021) An inspection was conducted by the ECM on Thursday August 26, 2021. Construction activities this week included continued plumbing utility installation on Building B. Excavation activities for Building A have started. No dust issues were observed during my inspection.All catch basins were inspected and the silt sacks were properly functioning. There continues to be silt accumulation around the catch basins on High Street and the Site Contractor was notified to continue weekly silt removal. The entire perimeter of the erosion control barrier was inspected and remains in-tact. As stated in last week's inspection summary,several areas of the erosion controls are exhibiting deterioration and should be replaced in the upcoming weeks. They will continue to be monitored and the Site Contractor will be notified when sections must be replaced. The roadways continues to be cleaned at the end of each day. Catch Basins The silt sacks within the S existing catch basins,three (3) on High Street and two (2) within the access road into the site,were inspected during each weekly inspection. During my August S, 12, 19 & 26 inspections, the catch basin silt sacks were observed in-tact and properly functioning. As stated above, during several of my inspections, there was silt accumulation around and within several of the basins on High Street and the Site Contractor was informed to remove any accumulated sediment. Erosion Controls The perimeter of the erosion control boundaries was walked/inspected in its entirety, and remained in-tact during each of my weekly inspections. There is accumulated sediment upgradient the erosion control barriers in several locations around the site, however, the barriers remain properly functioning and no silt breakthrough was observed during any of my inspections. Several of the deteriorated sections of silt socks should be replaced in the upcoming weeks. The erosion control boundaries will continue to be inspected and sections will be replaced with the green mulch socks when necessary or as instructed by the ECM. DEP Signs The DEP sign is posted at the entrance to the construction site on the chain link fence and remains in- tact. Soil/Stone stockpile Areas Gravel and soil stockpiles were observed stockpiled throughout the site during my August inspections, however, all piles appeared stabilized with no signs of runoff observed. The gravel stockpiles continue to be watered to prevent dust plumes. Pavement Sweeping/Dust Control The entrance roadway continues to be swept on a daily basis and was documented during my August 5, 12, 19 & 26 inspections. No dust within the site was not observed during any of my inspections. The water truck is kept on site daily and continues to actively water vehicle construction access routes and gravel stone piles. Ongoing Recommendations/Comments ® Inspect and maintain the erosion controls around the perimeter of the site. Sections to be replaced in the upcoming weeks. ® Inspect and the clean the catch basin silt sacks when necessary and remove sediment build-up around the southwestern catch basin on High Street weekly. ® Continue to implement dust control measures daily on within the site and the access roadways to prevent dust. An inspection was conducted by the ECM on Monday,July 12, 2021 during a rain event. The inspection was conducted following the ongoing rain events from the weekend. No work was being conducted on site at the time of my inspection. Large pockets of standing water remained on the site but no flowing water was observed within the site. I walked the entire perimeter of the erosion control barrier.There was accumulated silt build-up against several sections of the silt sock barrier,however,the barrier was functioning properly and the majority of the erosion control barrier was preventing silt breakthrough (see attached photo 2). There was standing water beyond the chain link fence in the low area in the northwestern corner north of Building B (see attached photo 3).This is the area where water was breaching through last week from the dewatering activities which were instructed to cease. This area will be re-evaluated later this week to determine if any sediment build-up needs to be removed. The pockets of standing water observed in the old baseball field appeared clear and free of silt. Pockets of standing water were observed at the base of the retaining wall under construction in the western section of the property(see attached photo 4). There was water flow observed within the drainage swale. No sediment or silt buildup was observed within any of the grass areas bordering on the swale. A dewatering pipe was directing water from the footprint of Building B towards a manhole in the eastern section of the site,and the Site Contractor was instructed to cease dewatering without proper pretreatment. July 15, 2021 Inspection An inspection was conducted by the ECM on Thursday, June 15, 2021. The main site construction activities underway this week included ongoing construction on Building B foundation footings, construction of the western retaining wall and construction of the northwestern underground infiltration system. All catch basin silt sacks were inspected and were free of debris. Sand material continues to accumulate around the southwestern catch basin on High Street and the Site Contractor was notified to continue to clean around this basin (see photo 5 attached). The roadway entrance continues to be swept. The site continues to be watered when necessary and no dust was observed during my inspection. The area of water breakthrough in the northern section of the site, north of the Building B foundation was re-inspected to determine if any sediment build-up needed to be removed. A small area of accumulated sediment (see photo 6) was observed beyond the erosion control barrier and the Site Contractor was notified to remove the sediment,which was completed by the end of the day. Summary of Construction Activities:Week of June 19,2021(inspection June 22) An inspection was conducted by the ECM on Thursday, June 22, 2021. The main site construction activities underway this week included continued installation of the southwestern underground infiltration system (see photo 12 attached). Foundation works continues on Building B and the footings were being poured for Building C today (see attached photo 10). The western retaining wall construction is almost complete and site utilities are being installed. All catch basins were inspected and are free of silt and debris. The southwestern catch basin on High Street continues to collect sediment around the grate. The Site Contractor stated it is being inspected and cleaned weekly. The area of breach silt in the northwestern corner with the ballfield has been removed by hand. The entire perimeter of the erosion control barrier was inspected and remains in-tact. The Project Engineer is working with Avalon Bay to address the Prescott/High Street stormwater runoff flowing into the manhole through the site. Modifications to the main drainage pipe running through the site is being explored to handle the excessive flow during significant rain events. Entrance roadway continues to be swept by hand and the water truck continues to actively water the construction site areas to prevent dust. No dust was observed within the site during my July 22, 2021 inspection. Summary of Construction Activities:Week of July 26, 2021(inspection July 29) An inspection was conducted by the ECM on Thursday July 29, 2021. Construction activities this week included continued foundation work on both Building B &C (see photos 13 &14 attached). The western infiltration system has been completed and is being backfilled. The western retaining wall is close to completion. The small retaining wall in the central southern section of the site is under construction (see photo 17). Underground utility installation continues. No dust issues were observed during my inspection.All catch basins were inspected and the silt sacks were properly functioning. There was some silt build-up in the northwestern catch basin on High Street and the Site Contractor was notified to have that cleaned. Catch Basins The silt sacks within the 5 existing catch basins,three (3) on High Street and two (2)within the access road into the site,were inspected during each weekly inspection. During my July 12, 15, 22 & 29 inspections, the catch basin silt sacks were observed in-tact and properly functioning. As stated above, during several of my inspections, there was some minor silt accumulation within several of the basins on High Street and the Site Contractor was informed to remove any accumulated sediment. Silt/sand from High Street surface flow continues to accumulate around the southern catch basins on High Street and the Site Contractor is inspecting and removing any sediment on a weekly basis. Erosion Controls The perimeter of the erosion control boundaries was walked/inspected in its entirety, and remained in-tact during each of my weekly inspections. There is some accumulated sediment upgradient the erosion control barrier in the northern section of the site from the July rains, however, the erosion controls remain in-tact and no breakthrough was identified, except for the one area noted in this report,which has been addressed. The erosion control boundaries will continue to be inspected and sections will be replaced with the green mulch socks when necessary or as instructed by the ECM. (the "Applicant"), I, Julie Vondrak, serving as the Erosion Control Monitor ("ECM") am submitting this weekly monitoring report for weekly construction activities conducted within the Buffer Zone to wetland resource areas on the above referenced site. The following activities were documented during several inspections during the week of July S, 2021 following several significant rain events. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings, a clubhouse,parking areas,a pool,retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and 50-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACC") issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES/RAIN MONITORING EVENTS Summary of Construction Activities: Week of July 5 2021 (inspections conducted over the previous weekend and on July 7, 9&10,2021) An inspection of the site was conducted during the weekend (July 4, 2021) following the weekend rains.The perimeter of the site was inspected,the roadway appeared free of sediment and the catch basins appeared to be properly functioning. The interior of the site was holding pockets of standing water but no significant flow was identified during my inspection. July 7,2021 Inspection Following the weekend rains,I again inspected the site on July 7, 2021 and carefully walked the entire perimeter of the erosion controls. Pockets of standing water remained on site and work being conducted included foundation work associated with the Building B and construction of the large block retaining wall along the western property boundary. Gravel stone has been placed at the base of the wall and the erosion controls were back in place as they were temporarily removed for wall footing construction. There was a small area of sediment buildup along the erosion controls in the northwestern section of the site (see photo 4 attached). This was the area where the erosion controls had been recently replaced with the green silt sock. Sediment had accumulated against the silt sock and a small breach of sediment/silty water penetrated through the erosion control barrier. The contractor was immediately notified and instructed to reinforce this section and remove any siltation accumulation. During this inspection,catch basins appeared to be free of sediment and properly functioning.Silt continues to build-up around the southwestern basin on High Street and the contractor was notified to remove the accumulated sediment. July 9, 2021 Inspection An inspection was conducted by the ECM on Friday,June 9, 2021 around noon during an extremely heavy period of rainfall associated with the storm. Heavy water flow down High Street was running over the catch basins which may have reached storage capacity.There was a significant flow of water throughout the site resulting from a water surge out of the manhole identified as DMH 3-B (see attached photo 5) located in the eastern section of the site. This manhole takes on water from the surrounding streets and the contractor stated he would further investigate which contributing drainage lines off-site fed into this particular basin.Overflow of this manhole was the reason for the surge of water flowing throughout the site. The water was originally following a flow path along the southern erosion control line and flow extended to the western section of the site behind the new retaining wall under construction. On-site contractors worked throughout the morning to construct earthern berms and stone berms to better divert the overflow water from the surging manhole into the central section of the site in the area of Building A.Heavy water flow originating from the surging basin was observed running through the erosion control barriers along the southwestern border. Groundwater was also observed flowing from under the retaining wall under construction into the existing drainage swale beyond the limit of work. The flowing water was murky but did not appear to have a high silt content and no accumulated sediment build-up was identified over the grassed areas beyond the erosion control barrier at the time of my inspection. During my inspection I observed a dewatering pipe in the northern section of the site that was pumping water from the Building B foundation footprint(see attached photos 10&11). The water was discharging along the erosion control barrier but was running downslope to the lowest point that was previously breached this week. The murky water was flowing through the erosion control barrier into the field and the Site Contractor was notified to either shut off this pump or divert the pumping water into the interior of the site.It is my understanding the pumping activities were addressed and diverted after I left the site. July 10,2021 The following day after the storm,the site was re-inspected. No work was being conducted on site. Large pockets of standing water existed on the site but all flowing water appeared to have stopped. The entrance roadway to the site was clean. The site will again be re-evaluated on Monday,July 12, 2021. Ongoing Recommendations/Comments ® Inspect and maintain the erosion controls around the perimeter of the site and reinforce any degraded silt socks. ® Remove any accumulated silt build-up adjacent or beyond the erosion controls following the heavy storm event. ® Inspect and the clean the catch basin silt sacks following the storm events. The building pad footprints for Building B and the Clubhouse have been excavated below the final grade and the earth will be compacted to support the weight of the new building pad foundations.Only 1 gravel construction entrance was being utilized during the week of my inspection. No obvious dust was observed during my inspection and the water truck was actively moving through the site watering the soil substrate. The access roadway from High Street is being swept daily by hand and sweeping activities were observed during my inspection.The Site Contractor informed me that some minor dewatering occurred in the vicinity of the abandoned drainage swale and that the water was filtered overland the grass areas. No signs of erosion or scour was identified at the time of my inspection. Summary of Construction Activities:Week of June 7, 2021 (inspection June 11) An inspection was conducted by the ECM on Friday, June 11, 2021. The main site construction activities underway this week included installation of the remaining electrical/conduit manhole installation in the westerly section of the site. Drainage infrastructure continues to be installed. The southern section of the site continues to be filled to bring up to grade. The soil compaction machine to be utilized for the building pad footprints broke during early compaction activities and compaction activities will be delayed. Entrance roadway continues to be swept by hand and the water truck continues to actively water the construction site areas to prevent dust. No obvious dust was observed during my inspection. No signs of erosion or scour was identified at the time of my inspection. Summary of Construction Activities:Week of June 14,2021(inspection June 17) An inspection was conducted by the ECM on Thursday, June 17, 2021. The main site construction activities underway this week included continued installation of drainage infrastructure including installation of the underground infiltrator in the eastern section of the site. Site excavation continues in the westerly section of the site. The western infiltration basin footprint has to be over-excavated as the soils in this area were not original parent materials and the Applicants were required to remove the soils and bring in new material. The excavated soils will be used under paved parking areas. Installation of the electrical conduit is almost complete. The soil compaction machine was being fixed on site at the time of my inspection and building footprint compaction activities will resume soon. Entrance roadway continues to be swept by hand and the water truck continues to actively water the construction site areas to prevent dust. No dust was observed within the site, however, minor dust was observed from access roadway(see comment below). There was a quick heavy rain event at the beginning of the week, however, the site did not display signs of runoff or erosion. No signs of erosion or scour was identified at the time of my inspection. Summary of Construction Activities:Week of June 21,2 02 1(inspection June 24) An inspection was conducted by the ECM on Thursday June 24, 2021. Construction activities this week included compaction activities within the building footprints, continued construction of the underground infiltration systems and ongoing installation of drainage infrastructure. The eastern infiltration system has been completed and backfilled and the western one is under construction. While on-site I observed the soil compaction activities within the Building B footprint (see attached photos). The access road entrance was swept and no dust issues were observed during my inspection. All catch basins were inspected and the silt sacks were properly functioning. Summary of Construction Activities:Week of June 28,2O21(inspection July 1) An inspection was conducted by the ECM on Thursday July, 2021. The main construction activities this week included installation of Building B's footings (compaction activities complete) (see attached photo). Soil compaction activities were still being conducted within Building A's footprint. The western infiltration basin has been excavated to grade and is under construction.The western section of the site is being prepared for construction of the approximate 8 to 9-foot retaining wall. A section of the western erosion controls have been temporarily removed to allow for the base wall construction and will be immediately replaced when the wall is built to existing grade. The entrance roadway was observed swept and cleaned. All catch basins were inspected, however, there was some observed silt build-up around one of the basins on High Street(see comments below). Catch Basins The silt sacks within the 5 existing catch basins,three (3) on High Street and two (2)within the access road into the site,were inspected during each weekly inspection. During my June 4, 2021 inspection, the south-western catch basin on High Street had some accumulated debris and the Site Contractor was notified to inspect and clean it. Catch basins appeared clean and free of debris during my June 11, 2021 inspection and silt sacks appear to continue to be properly maintained. During my June 17, 2021 inspection, one of the catch basin silt sacks had several inches of standing water and will continue to be monitored to ensure it is free of silt build-up. During my June 24, 2021 inspection, the catch basin silt sacks were observed in-tact and properly functioning. During my July 1, 2021 inspection, all catch basins were inspected and silt sacks were in-tact and properly functioning. One of the catch basins had some accumulated sand around the basin perimeter and the site contractor was notified to remove the material. Erosion Controls The perimeter of the erosion control boundaries was walked/inspected in its entirety, and remained in-tact during my weekly inspections. Site grading had been raised in the western section of the site during the June 4 inspection, and the grading sloped down to the erosion control limits, however, no signs of erosion were identified at the time of inspection. The erosion control barrier was inspected during my June 11, 2021 inspection and remained in- tact with no signs of damage or breakthrough of soil materials. No soil or debris was identified beyond the erosion control barrier perimeter. During my June 17, 2021 inspection, a small amount of soil material had accumulated over a small section of the erosion control barrier in the north western section of the site (see attached picture) and the Site contractor was notified to remove the material. During my June 24, 2021 inspection, the perimeter of the erosion controls was inspected and no issues were identified. The soil material observed over a section of the erosion control the previous week was removed. During my July 1, 2021 inspection, several areas of the erosion control barrier in the western section of the site were being replaced/re-aligned as they had temporarily been removed. The western section of erosion control was temporarily removed to allow for the retaining wall base to be installed. As soon as the footings are installed the erosion control barrier will be re-installed in its previous location. The erosion control boundaries will continue to be inspected and sections will be replaced with the green mulch socks when necessary or as instructed by the ECM. DEP Signs The DEP sign is posted at the entrance to the construction site on the chain link fence and remains in- tact. Soil stockpile Areas Gravel, soil and asphalt stockpiles were observed throughout the site, however, all piles appeared stabilized with no signs of runoff observed. The gravel stockpiles continue to be watered to prevent dust plumes. Pavement Sweeping/Dust Control The entrance roadway continues to be swept on a daily basis and was documented during my June 4, 11, 17, 24 &July 1 inspections. At the time of my June 4 inspection, a contractor was hand sweeping the immediate access point into the site (see attached photo). The water truck is kept on site daily and continues to actively water vehicle construction access routes and gravel stone piles. During my June 17, 2021 inspection, the entrance roadway was a little dusty and equipment access was producing a small dust plume and the Site Contractor was immediately notified to address the issue. Ongoing Recommendations/Comments ® Inspect and maintain the erosion controls around the perimeter of the site. ® Remove any stockpiled debris or site materials from the erosion control barriers. ® Inspect and the clean the catch basin silt sacks mentioned in this report. ® Continue to implement dust control measures daily on within the site and the access roadways to prevent dust. (the "Applicant"), 1, Julie Vondrak, serving as the Erosion Control Monitor ("ECM") am submitting this monitoring report for weekly construction activities conducted within the Buffer Zone to wetland resource areas on the above referenced site. The following activities were documented during my weekly site inspection conducted on May 24, 2021. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings, a clubhouse,parking areas,a pool, retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and SO-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACU") issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of May 17, 2021 An inspection was conducted by the ECM on Thursday, May 27, 2021 and again on May 29, 2021 during the weekend rain storm. The main site construction activities underway this week included ongoing electrical/conduit manhole installation throughout the site (see photo 1).The majority of the duct bank has been installed.The electrical manhole located across High Street has been installed and backfilled. Topsoil materials continue to be stripped and sorted. Loom continues to be screened and unused materials are being trucked off site. The western section of the site is being prepped for the new building pads. The building pad areas will be compacted end of next week by an outside contractor. A new drain manhole was being installed within the access roadway to site at the time of my inspection (see photo 6). Catch Basins The silt sacks within the 5 existing catch basins,three (3) on High Street and two (2)within the access road into the site,were inspected. Two of the silt sacks did contain a little standing water and should be inspected and cleaned of any debris. Erosion Controls The perimeter of the erosion control boundaries was walked/inspected in its entirety, and remains in-tact. No soil or debris was identified beyond the erosion control barrier perimeter. In one section, conduit was stored adjacent/on a section of the silt sock and should be removed to protect the integrity of the silt sock (see photo 4). The erosion control boundaries will continue to be inspected and sections will be replaced with the green mulch socks when necessary or as instructed by the ECM. Additional stone was temporarily placed at the downgradient section of the stormwater swale that will soon be removed (see photo 5). The stormwater flow to this channel has already been diverted and no downstream sedimentation was observed. DEP Signs The DEP sign is posted at the entrance to the construction site on the chain link fence and remains in- tact. Soil stockpile Areas Gravel, soil and asphalt stockpiles were observed throughout the site, however, all piles appeared stabilized with no signs of runoff observed. The large loom pile discussed in last week's report has been removed. Pavement Sweeping/Dust Control At the time of my inspection, the entrance road into the site was clean and not in need of sweeping. Additionally, dust did not appear to be an issue during the time of my inspection. It was my understanding that the morning of my inspection, a complaint was made regarding dust on the site. I spoke with Site Contractor, Joe Cincotta, who informed me the water truck is kept on site daily and that the dust may be coming from the crushed stone piles when disturbed. He informed me that Avalon will set up a sprinkler system to continuously water the stone piles so that dust will be controlled when materials are removed from the piles. Additionally, the pavement on site was pulverized and applied as a base on the site to further control dust. Ongoing Recommendations/Comments ® Inspect and maintain the erosion controls around the perimeter of the site. ® Remove any stockpiled debris or site materials from the erosion control barriers. ® Inspect and the clean the catch basin silt sacks mentioned in this report. ® Continue to implement dust control measures daily to address the abutters concerns. An on-site meeting with the Applicant, their consultants and the Conservation and Planning Department personnel was conducted on Friday, May 7, 2021 to review the erosion controls and to discuss the pre-construction conditions. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of May 10,2021 An inspection was conducted by the ECM on Thursday, May 13, 2021. Site construction activities conducted this week included installation of the five (5) subsurface concrete electrical boxes (see photo 1 & 2). No dewatering was necessary for the installation of the electrical boxes. Two gravel construction entrances have been constructed for access into the site and remain in-tact.The majority of the pavement has not yet been removed and no dust issues from the current construction activities was observed. Catch Basins Currently silt sacks have been installed within 5 existing catch basins, three (3) on High Street and two (2) within the access road into the site. The silt sacks have been inspected and no significant silt build-up was observed (see photo 3). The silt sacks must remain in-tact and should be inspected weekly by the contractors and cleaned when necessary. Erosion Controls Erosion controls consisting of silt socks and filter fabric have been installed in the proper locations according to latest revised record plans and remain in-tact. As discussed during the preconstructi.on meeting,the straw silt socks are allowed to remain in place as installed,however,if the silt socks need to be replaced, they will be replaced with the green, mulch socks, which are stockpiled on site along with additional silt fencing materials. The silt sock connection was adjusted in the field so that the connection points were not stacked on top of each other, but rather overlapped in front of each other (see photo 6). The entire length of the erosion controls was inspected in the field and no siltation beyond the erosion controls was identified during inspection. DEP Signs The DEP sign is posted at the entrance to the construction site on the chain link fence and remains in- tact(see attached photo 4). Soil stockpile Areas At this time there were several small soil stockpile areas within the central portion of the site.No run- off was observed from any the piles. Ongoing Recommendations/Comments ® Inspect and maintain the erosion controls around the perimeter of the site. ® Inspect and the clean the catch basin silt sacks when necessary. ® Implement dust control measures when necessary.
