(the "Applicant"), 1, Julie Vondrak, serving as the Erosion Control Monitor ("ECM") am submitting this monthly monitoring report for weekly construction activities conducted within the Buffer Zone to wetland resource areas on the above referenced site. The following activities were documented during my weekly site inspections on May 5, 12, 19 & 27, 2022. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings,a clubhouse,parking areas,a pool,retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and SO-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACU) issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist ("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of May 2, 2022 (May 5,2022 inspection) An inspection was conducted by the ECM on Thursday,May S, 2022.Weather conditions were sunny in the 60's.Interior work continues on Building B &C.Exterior siding and brick laying work continues on Building B (see attached photo 1). Third floor framing continues on Building A(see attached photo 2 &3). The entire perimeter of the erosion control line was reviewed and no breaching was observed.A section of erosion controls still needs reinforcement in the northwestern corner of the property and the Site Managers have notified their contractors to complete the work. The steep,unvegetated slopes along the southern boundary show no signs of erosion and erosion controls remain in place (see attached photo 2).The erosion control measures at the base of the western retaining wall remain in-tact and no breaching was identified. The wet meadow continues to hold standing water,however,the disturbed path for the phragmite barrier is starting to dry out(see attached photo 4). The landscapers tried to the seed the disturbed areas but it was too wet. The area will be seeded when the there is no longer standing water. Limited flow from the Prescott Street drainage outlet and site drainage outlets was observed in the field at the time my inspection. The outstanding issues addressed in previous inspections have not yet been addressed,however,new silt sacks have been ordered and the Applicant is awaiting the arrival of the new sacks to replace the identified damaged ones. The majority of the construction debris and trash around the site continues to be cleaned. There is still a large piece of plywood beyond the western retaining wall that needs to be removed. Soil,stone and construction material piles throughout the site remain stable and no erosion from the piles was identified. No dust was observed during my inspection. Summary of Construction Activities:Week of May 9, 2022 (May 12,2022 inspection) An inspection was conducted by the ECM on Thursday, May 12, 2022.Weather conditions were mild with an overcast sky. Interior work continues on Building B &C.Exterior siding and brick laying work continues on Building B &C (see attached photo B). Framing continues on Building A and third floor construction is underway (see attached photo 6). The pool footprint has been excavated in front of C (see attached photo 7). The entire perimeter of the erosion control line was reviewed and no breaching was observed. Several areas of the erosion controls have deteriorated,however,no breaching has occurred (see attached photo 8). Several sections should be repaired if slopes are stabilized with vegetation soon. The steep,unvegetated slopes along the southern boundary continue to show no signs of erosion and erosion controls remain in place.The erosion control measures at the base of the western retaining wall remain in tact and no breaching was identified. The wet meadow continues to hold some standing water especially upgradient the phragmite barrier installation. The meadow has not yet been seeded and will continue to be monitored to determine an appropriate seeding time. Given the lack of recent rains,not flowing water was observed within the drainage outlets along the western wall. Catch basin silt sack replacement has still not yet occurred. The contractors responsible for this work have not yet been to the site to replace the silt sacks and sweep the road.No dust was identified during my site inspection. The Site Contractors/Managers continue to be notified that this should be addressed as soon as possible. The majority of the construction debris and trash around the site continues to be cleaned.The large piece of plywood located beyond the fence line along the western property boundary still needs to be removed. Summary of Construction Activities:Week of May 16,2022 (May 19,2022 inspection) An inspection was conducted by the ECM on Thursday, May 19,2022.Weather conditions were mild and cloudy. Exterior work continues on Building B &C and interior sheet rock work is underway(see photo 9). Framing continues on Building A and the fourth floor is under construction. The entire perimeter of the erosion control line was reviewed and no breaching was observed. Several areas of the erosion controls are deteriorated however,they continue to offer sufficient protection at this time. The Contractor has been made aware of areas that may need reinforcement soon. The steep,unvegetated slopes along the southern boundary show no signs of erosion and erosion controls remain in place.The erosion control measures at the base of the western retaining wall remain in-tact and no breaching was identified (see attached photo 10). The wet meadow continues to hold pockets of water but appears to be drying out and should be seeded soon(see attached photo 11). Standing water remains upgradient the phragmite barrier in the eastern section of the meadow. No flow was observed from the Prescott Street drainage outlet and site drainage outlets at the time my inspection. Catch basin silt sacks have not yet been replaced and the Site Managers are waiting for Capitol to complete the work including silt sack replacement and site sweeping (see attached photo 12). The majority of the construction debris and trash around the site continues to be cleaned. The Site Managers were made aware of several areas that need attention. Soil,stone and construction material piles throughout the site remain stable and no erosion from the piles was identified. Summary of Construction Activities:Week of May 23,2022 (inspection May 27,2022) An inspection was conducted by the ECM on Friday,May 27, 2022.Weather conditions were mild and cloudy. Exterior work continues on Building B &C and interior sheet rock work is underway on Building B and C (see photo 13). Framing continues on Building A and the fourth floor is under construction (see photo 14). The entire perimeter of the erosion control line was reviewed and no breaching was observed. Several areas of the erosion controls are deteriorated however,they continue to offer sufficient protection at this time. The Contractor has been made aware of areas that may need reinforcement soon,especially in the northwest corner(see attached photo 15). The steep,unvegetated slopes along the southern boundary show no signs of erosion and erosion controls remain in place.The erosion control measures at the base of the western retaining wall remain in-tact and no breaching was identified.No flow was observed through the drainage outlets at the western wall. Rebar still remains within the pool footprint and no additional work has been conducted. Pockets of standing water have diminished in the wet meadow and the Site Managers were instructed to scarify and seed the disturbed areas. Catch basin silt sacks have not yet been replaced and the Site Managers are still waiting for Capitol to complete the work including silt sack replacement and site sweeping.I again notified them in writing that this must be addressed. The majority of the construction debris and trash around the site continues to be cleaned and site was primarily free of garbage during my inspection.The large piece of plywood adjacent to the western retaining wall has been removed. Soil,stone and construction material piles throughout the site remain stable and no erosion from the piles was identified. There is a large pile of soil stockpiled in front of Building C which is the. excavated materials from the pool footprint and will be removed off-site (see attached photo 16). Catch Basins All catch basins within High Street, the access roadway and within the construction site have been inspected on a weekly basis. As stated above in the weekly reports, several silt sacks need to be cleaned/replaced and the Client is waiting for the Site Contractors to complete the work. The contractor continues to be notified if catch basin silt sacks need attention. Erosion Controls The perimeter of the erosion control boundary was walked/inspected in its entirety during each of my inspections. Currently, all erosion controls remain in place and sufficiently prevented any potential erosion issues over the last month. Several areas may need replacement and have been identified in this report. Erosion controls will continue to be inspected and replaced as necessary or as instructed by the ECM. DEP Sign The DEP sign is posted on the chain link fence at the entrance of the Project Site and continues to remain in place. Stone/Soil/Construction Material stockpile Areas Currently soils, stone and construction material stockpiles are located throughout the site and will continue to be monitored for erosion. Pavement Sweeping/Dust Control No dust issues were observed during my weekly inspections.The site is currently in need of sweeping and the Site Managers are waiting for Capitol to complete the work. The site will continue to be monitored and the Site Managers will be notified when sweeping is necessary. Ongoing Recommendations/Comments ® Continue to inspect and maintain erosion controls along the perimeter of the site. ® Replace deteriorated erosion controls in the northwest section of the site. ® Seed the disturbed wet meadow areas with the specified seed mix ASAP. ® Site currently needs sweeping. Continue to sweep the paved areas when silt build-up occurs. ® Inspect and the clean the noted catch basin silt sacks. Replace deteriorated silt sacks as instructed by the Environmental Monitor. ® Any accumulated construction debris/trash should continue to be removed from the perimeter of the site. The entire perimeter of the erosion control line was reviewed and no breaching was observed. Several areas of the erosion controls,particularly behind Building B have been reinforced and additional areas will be addressed in the future,if necessary. The steep,unvegetated slopes along the southern boundary show no signs of erosion and erosion controls remain in place (see attached photo 4). The phragmite barrier installation route was inspected and no runoff was identified (see attached photo 3). The disturbed areas will be seeded in May. The Conservation Department has inquired whether the disturbed areas should be scarified before seed application and recommended salt hay mulch be applied over the newly seeded areas. Mrs.Vondrak will discuss these recommendations with the Applicant/Site Manager. Overland flow from the Prescott Street drainage outlet and site drainage outlets was observed in the field at the time my inspection. The majority of the paved areas were free of silt accumulation.Silt sacks are in place and properly functioning with the exception of the silt sack in the catch basin behind Building C,which needs to be addressed (see attached photo S). The majority of the construction debris and trash around the site has been cleaned. Soil,stone and construction material piles throughout the site remain stable and no erosion from the piles was identified. Summary of Construction Activities:Week of April 11, 2022 (April 14,2022 inspection) An inspection was conducted by the ECM on Thursday,April 14,2022.Weather conditions were mild with an overcast sky. Interior work continues on Building B &C.Exterior siding and brick laying work has begun on Building B (see attached photo 6). Framing continues on Building A(see attached photo 7). The entire perimeter of the erosion control line was reviewed and no breaching was observed. Several areas of the erosion controls,particularly behind Building B have been reinforced and additional areas will be addressed in the future,if necessary. The steep,unvegetated slopes along the southern boundary show no signs of erosion and erosion controls remain in place.The erosion control measures at the base of the western retaining wall remain in tact and no breaching was identified (see attached photo 8). Interior work continues on Building B &C. Exterior siding and brick laying work continues on Building B (see attached photo 10). Framing continues on Building A(see attached photo 11). The entire perimeter of the erosion control line was reviewed and no breaching was observed. Several areas of the erosion controls,will need reinforcement in the next several weeks especially in the northwestern corner of the property.The steep,unvegetated slopes along the southern boundary show no signs of erosion and erosion controls remain in place.The erosion control measures at the base of the western retaining wall remain in-tact and no breaching was identified (see attached photo 13). The wet meadow was walked and is saturated to the surface and new vegetation including grasses and sedges are growing. Standing water remains upgradient the phragmite barrier in the eastern section of the meadow(see attached photo 12). The Site Managers were sent the seed information for ordering. The meadow should be seeded mid to late May. Light flow from the Prescott Street drainage outlet and site drainage outlets was observed in the field at the time my inspection. Several sections of the paved areas were in need of sweeping and the Site Manager was notified.The Site Manager informed me that"Capitol",the site contractors responsible for catch basin repairs and site sweeping,will be on site the week of April 25th to address the outstanding issues addressed in my earlier inspections. The majority of the silt sacks are in place and properly functioning.The silt sacks with the basin/basins behind Building C and within High Street are still in need of replacement. The majority of the construction debris and trash around the site continues to be cleaned. Soil,stone and construction material piles throughout the site remain stable and no erosion from the piles was identified. Summary of Construction Activities:Week of April 25, 2022 (inspection April 28,2022) An inspection was conducted by the ECM on Thursday,April 25, 2022.Weather conditions were mild and it was sunny. Interior work continues on Building B&C.Exterior siding and brick laying work continues on Building B (see attached photo 14). Second and third floor framing continues on Building A(see attached photo 15). The entire perimeter of the erosion control line was reviewed and no breaching was observed.A section of erosion controls needs reinforcement in the northwestern corner of the property(see attached photo 16).The steep,unvegetated slopes along the southern boundary show no signs of erosion and erosion controls remain in place.The erosion control measures at the base of the western retaining wall remain in-tact and no breaching was identified. The wet meadow continues to hold standing water and the standing water remains upgradient the phragmite barrier in the eastern section of the meadow.The Site Managers were sent the seed information for ordering.The meadow should be seeded mid to late May. Pavement Sweeping/Dust Control No dust issues were observed during my weekly inspections.The site is currently in need of sweeping and the Site Manager has been notified and work is expected to be completed during the week of May 2, 2022.The site will continue to be monitored and the Site Managers will be notified when sweeping is necessary. Ongoing Recommendations/Comments ® Continue to inspect and maintain erosion controls along the perimeter of the site. ® Replace deteriorated erosion controls in the northwest section of the site. ® Seed the disturbed wet meadow areas with the specified seed mix mid to late May. ® Site currently needs sweeping. Continue to sweep the paved areas when silt build-up occurs. ® Inspect and the clean the noted catch basin silt sacks. Replace deteriorated silt sacks as instructed by the Environmental Monitor. ® Any accumulated construction debris/trash should continue to be removed from the perimeter of the site. Project,North Andover, MA,Buffer Zone Monitoring Report#14,Weeks of March 1, 7, 14, 21 and 28,2022 Dear Commissioners and Planning Board Members, On behalf of AvalonBay Communities, Inc. (the "Applicant"), 1, Julie Vondrak, serving as the Erosion Control Monitor ("ECM") am submitting this monthly monitoring report for weekly construction activities conducted within the Buffer Zone to wetland resource areas on the above referenced site. The following activities were documented during my weekly site inspections on March 1, 4, 9, 17, 24 &31, 2022. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings,a clubhouse,parking areas,a pool,retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and SO-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACC") issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist ("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events, SUMMARY OF CONSTRUCTION ACTIVITIES Phragmites Barrier Installation,Tuesday,March 1, 2022 Two onsite inspections were conducted by Mrs.Vondrak throughout the day during installation of the Phragmites barrier installed in accordance with the approved plan. Snow cover existed on the ground and several inches of frost made it a suitable time to install the barrier, as there would be less disturbance to the ground surface. Prior to barrier installation, the site contractor marked the location of the barrier in the field and an excavator removed the top layer of snow along the barrier route (See attached photo 1). An approximate 3-foot trench was dug in 20-foot increments and the barrier was installed and backfilled before the next 20-foot section was completed (See attached photo 2). Excavated soils were immediately cast to the side of the trench and backfilled immediately upon installation of the plastic phragmites barrier. Approximately 1-2 inches of the plastic barrier is exposed above grade (see attached photo 4). The barrier installation was completed within one day. An approximate 3 to 4-foot wide area of unvegetated surface remains post barrier installation. No woody vegetation was disturbed during the installation activities, and the disturbed surface was further raked to establish the original grades. Given the work areas were located in fairly level grades, it does not appear additional erosion controls are necessary at the time along the limit of disturbance. The disturbed areas should be immediately seeded in the Spring in order to stabilize the disturbed wet meadow areas. I recommend the Applicant purchase the New England Wet Mix from New England Wetland Plants,Inc. (see attached species list) to revegetate the temporarily disturbed wet meadow. Summary of Construction Activities:Week of March 1,2022 (March 4,2022 inspection) An inspection was conducted by the ECM on Friday,March 4, 2022.It was cold during the time of inspection and the snow-covered ground was frozen. Interior work within Building B continues along with roof work(see attached photo 6). Areas within Building A are being filled with stone in preparation of the cement slab installation. Frame work continues on Building C. The entire perimeter of the erosion control line was reviewed and is mostly frozen and under snow cover. No obvious breaching was observed and all erosion controls appeared to remain in-tact.No sedimentation runoff was identified. The phragmites barrier installation route was inspected and no erosion or runoff was observed from the disturbed section. The eastern section of the route is along a slight slope and a section of the above ground barrier,along the upland edge, contained frozen standing water(see attached photo 7). Light overland stormwater overflow from the Prescott Street drainage outlet through the western wall was observed. Several paved areas throughout the site were muddy and sweeping activities were recommended. The silt sacks within all the catch basins were inspected and several of the silt sacks still need to be replaced and cleaned (see photo 9). Stockpiled snow,stone and construction materials remain on site but appeared stabilized at the time of inspection Summary of Construction Activities:Week of March 7, 2022 (March 9,2022 inspection) An inspection was conducted by the ECM on Wednesday,March 9, 2022.The temperature was milder at the time of the inspection and no snow cover existed on the ground. Interior work within Building B continues along with frame work on Building C. The shaft is under construction on Building A(see attached photo 9). Only a small section of the slab remains to be poured on Building A. The entire perimeter of the erosion control line was reviewed and several sections need new replacement and/or reinforcement. The Contractor was notified that several areas will need replacement in the upcoming weeks. No breaching was observed at the time of inspection. The phragmite barrier installation route was inspected and no runoff was identified. The eastern section continues to hold some frozen standing water upgradient the barrier (see attached photo 10). The fence line behind Building B needs to be re-established to prevent access into the wet meadow area(see attached photo 11). Overland flow from the Prescott Street drainage outlet and site drainage outlets was observed in the field at the time of inspection. The site was in need of a sweeping and several catch basin silt sacks within the site need to be replaced and the Site Contractor was notified. Additionally, construction debris and trash around the site should be cleaned. Summary of Construction Activities:Week of March 14, 2022 (March 17, 2022 inspection) An inspection was conducted by the ECM on Thursday, March 17, 2022. Weather conditions were mild during my inspection and no snow cover existed on the site. Interior work continues on Building B. Exterior framing on Building C appears close to completion and window installation has begun (see attached photo 13). Shaft work continues on Building A and framing has begun (see attached photo 14). The entire perimeter of the erosion control line was reviewed and no breaching was observed.As stated in earlier inspections this month,several areas of erosion controls need to replaced over the next several weeks (see attached photo 15). The phragmite barrier installation route was inspected and no runoff was identified (see attached photo 16). A specified seed mix should be applied at the start of the growing season in mid-April. The fence line behind Building B needs to be re-established to prevent access into the wet meadow area. Overland flow from the Prescott Street drainage outlet and site drainage outlets was observed in the field at the time of inspection. The site was in need of a sweeping and several catch basin silt sacks within the site still need to be replaced and the Site Contractor was notified (see attached photo 17). Summary of Construction Activities:Week of March 21,2022 (inspection March 24,2022) An inspection was conducted by the ECM on Thursday, March 24, 2022. I was on site during a rain event and no snow cover existed on site. Interior work continues on Building B. Exterior framing on Building C appears close to completion. Shaft work and framing continues on Building A(see attached photo 18). The entire perimeter of the erosion control line was reviewed and no breaching was observed.As stated in earlier inspections this month,several areas of erosion controls need to replaced over the next several weeks. The fence line behind Building B has been restored (see attached photo 19). The phragmite barrier installation route was inspected and no runoff was identified. The disturbed areas will be seeded in early May. Overland flow from the Prescott Street drainage outlet and site drainage outlets was observed in the field at the time my inspection during the rain event. The site has been swept over the last week and catch basin silt sacks were replaced in those basins identified in my earlier inspections (see attached photo 20). All silt sacks were in-tact and properly functioning with the exception of the catch basin behind Building C,which may have been clogged. The basin was under water during the rain storm and should be cleaned,if necessary,when the ponded water dries up (see attached photo 21). The majority of the construction debris and trash around the site has been cleaned. The majority of the snow stockpiles have melted and the remaining soil and stone piles throughout the site remain stable and no erosion from the piles was identified. Summary of Construction Activities:Week of March 28,2022 (inspection March 31,2022) An inspection was conducted by the ECM on Thursday, March 31, 2022.An inspection was conducted by the ECM on Thursday, March 24, 2022. Weather conditions were mild. Interior work continues on Building B. Exterior work on Building C appears close to completion (see attached photo 22). Shaft work and framing continues on Building A(see attached photo 23). The entire perimeter of the erosion control line was reviewed and no breaching was observed. Several areas of the erosion controls,particularly behind Building B have been reinforced and additional areas will be addressed in the future,if necessary(see attached photo 24). The phragmite barrier installation route was inspected and no runoff was identified. The disturbed areas will be seeded in early May. Overland flow from the Prescott Street drainage outlet and site drainage outlets was observed in the field at the time my inspection during the rain event. The sweeper was on-site during my inspection and paved areas were clean and free of silt(see attached photo 2S). Silt sacks are in place and properly functioning. The ponded water behind Building C has evaporated and the silt sack should be replaced in this basin (see attached photo 26). The majority of the construction debris and trash around the site has been cleaned. The snow stockpiles have melted and the remaining soil,stone and construction material piles throughout the site remain stable and no erosion from the piles was identified. Catch Basins All catch basins within High Street, the access roadway and within the construction site have been inspected on a weekly basis. As stated above in the weekly reports, several silt sacks started to deteriorate over the month, however, all deteriorated silt sacks continue to be replaced and will continue to be monitored. The contractor continues to be notified if catch basin silt sacks need attention. Erosion Controls The perimeter of the erosion control boundary was walked/inspected in its entirety during each of my inspections. Currently, all erosion controls remain in place and sufficiently prevented any potential erosion issues during the winter months. Several areas have been recently reinforced and additional areas will be identified and replaced over the next month. Erosion controls will continue to be inspected and replaced as necessary or as instructed by the ECM. DEP Sign The DEP sign is posted on the chain link fence at the entrance of the Project Site and continues to remain in place. Stone/Salt/Snow stockpile Areas Currently (end of March),there are snow stockpiles left on site.Sand, stone and construction material stockpiles on site will continue to be monitored for erosion. Pavement Sweeping/Dust Control No dust issues were observed during my weekly inspections.The site was in need of sweeping during several of my inspections and was addressed during the week of March 21. The site will continue to be monitored and the Contractor will be notified when sweeping is necessary. Ongoing Recommendations/Comments ® Continue to inspect and maintain erosion controls along the perimeter of the site. ® Replace deteriorated erosion controls over the next several weeks. ® Seed the disturbed wet meadow areas with the attached specified seed mix. ® Continue to sweep the paved areas when silt build-up occurs. ® Inspect and the clean the catch basin silt sacks when necessary and remove sediment build-up weekly.Replace deteriorated silt sacks as instructed by the Environmental Monitor. ® Any accumulated construction debris/trash should continue to be removed from the perimeter of the site. Project,North Andover,MA,Buffer Zone Monitoring Report#13,Weeks of February 1, 7, 14&21, 2022 Dear Commissioners and Planning Board Members, On behalf of AvalonBay Communities, Inc. (the "Applicant"), I, Julie Vondrak, serving as the Erosion Control Monitor ("ECM") am submitting this monthly monitoring report for weekly construction activities conducted within the Buffer Zone to wetland resource areas on the above referenced site. The following activities were documented during my weekly site inspections on February 3, 10, 17 & 24, 2021. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings, a clubhouse,parking areas,a pool,retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and 50-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACU) issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of February 1, 2022 (February 3, 2022 inspection) An inspection was conducted by the ECM on Thursday, February 3, 2022.Window installation on Building B continues and appears close to completion (see attached photo 1).Frame work on Building C continues. Underground utility work within the Building A foundation footprint continues (see attached photo 2). The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact.No sedimentation runoff was identified. Light overland stormwater overflow from the Prescott Street drainage outlet through the western wall was observed (see attached photo 3). The silt sacks within all the catch basins were inspected and several of the silt sacks still need to be replaced.Paved areas on site were free of silt at the time of inspection. There are several snow stockpile areas within the internal sections of the site (see attached photo 4). Summary of Construction Activities:Week of February 7,2022 (February 10,2022 inspection) An inspection was conducted by the ECM on Thursday,February 10, 2022.The temperature was mild at the time of the inspection. Several inches of frozen snow remains on the site. Building B window installation appears complete (see attached photo 5).Underground utility work within the Building A foundation footprint continues (see attached photo 6). The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact(see attached photo 7). There appeared to be a lot of overland flow from the Prescott Street drainage outlet,as observed from the water flow and scour over the rip/grass area (see attached photo 8). The silt sacks within all the catch basins were inspected and it appears the damaged ones were replaced and all sacks were in-tact and properly functioning (see attached photo 9). Summary of Construction Activities: Week of February 14, 2022 (February 17, 2022 inspection) An inspection was conducted by the ECM on Thursday, February 17, 2022. Weather conditions were mild during my inspection. Window installation appears complete on Building B and apartment deck construction is underway (see attached photo 10). Roof work has also commenced on Building B. A majority of the underground utility work within the Building A footprint has been completed and sections are being filled with crushed stone in preparation of the pouring of the foundation slab. The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact.A section in the northwest corner of the site will continue to be monitored closely, as the erosion controls will need reinforcement/replacement in the upcoming Spring months (see attached photo 11). The silt sacks within all the catch basins were inspected and appear to be in-tact and properly functioning. Several of the silt sacks had some minor accumulation of sediment and will continue to be monitored and the Site Contractor will be notified when cleaning is necessary. Given the warmer temps, sections of the paved roadway were muddy and the site should be swept (see attached photo 12). There are stockpiles of stone,snow and salt throughout the site,but appear stable and are not located near any catch basin inlets. There is some accumulated garbage around the perimeter of the site that should be removed. Summary of Construction Activities: Week of February 21, 2022 (inspection February 24, 2022) An inspection was conducted by the ECM on Thursday, February 24, 2022. A snow storm is arriving tomorrow, Friday, February 25, 2022. Exterior and roof work continues on Building B. Building C framework has progressed over the last week. A portion of the foundation slab within Building A has been poured (see photo 14). The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact.As stated during last week's inspection,several sections of erosion controls will need reinforcement/replacement over the next month after the winter thaw. The silt sacks within all the catch basins were inspected and remain in-tact. After tomorrow's snow storm,the basins should be inspected and cleaned of any debris and frozen snow(see attached photo 16). Several sections of pavement need sweeping and should be addressed after the snow fall. Project,North Andover,MA,Buffer Zone Monitoring Report#12,Weeks of January 3, 10, 17 &24, 2022 Dear Commissioners and Planning Board Members, On behalf of AvalonBay Communities, Inc, (the "Applicant"), 1, Julie Vondrak, serving as the Erosion Control Monitor ("ECM") am submitting this monthly monitoring report for weekly construction activities conducted within the Buffer Zone to wetland resource areas on the above referenced site. The following activities were documented during my weekly site inspections on January 6, 13, 20 & 27, 2021. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings,a clubhouse,parking areas,a pool,retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and 50-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACU) issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist ("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of January 3,2022 (January 6, 2022 inspection) An inspection was conducted by the ECM on Thursday,January 6,2022. Frame work on Building B (see attached photo 1) &C continues. Building A foundation work continues and the foundation is being backfilled (see attached photo). The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact.No sedimentation runoff was identified. The silt sacks within all the catch basins were inspected and appear to be in-tact and properly functioning. The chain link fencing has been re-installed behind building B along the limit of work line (see attached photo 3). The new DEP sign has been installed and is hung on the fence at the entrance to the site (see attached photo 4). The Site Contractor continues to sweep the site and access roadway on an as-needed basis. Summary of Construction Activities:Week of January 10, 2022 (January 13,2022 inspection) An inspection was conducted by the ECM on Thursday,January 13,2022.The ground was frozen and had approximately an inch of snow cover.Windows are being installed in Building B and Building C framework continues (see attached photo S). Foundation work continues on Building A and is close to completion (see attached photo 6). The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact(see attached photo 7). The silt sacks within all the catch basins were inspected and appear to be in-tact and properly functioning(see attached photo 8). Water flow was observed through the western retaining wall outlet pipes. Summary of Construction Activities.Week of January 17,2022 (January 20,2022 inspection) An inspection was conducted by the ECM on Thursday, January 20, 2022. Window installation continues on Building B and framework continues on Building C (see attached photo 9). Foundation work is close to completion on Building A and the foundation slab will be poured in the near future (see attached photo 10). The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact(see attached photos 11&12). The silt sacks within all the catch basins were inspected and appear to be in-tact and properly functioning. One of the central catch basins needs cleaning and the pavement area surrounding the catch basin inlet behind Building C has some accumulated mud debris and should be cleaned (see attached photo 13). There are several small stockpiles of stone and salt throughout the site,but appear stable and are not located near any catch basin inlets. There is some accumulated garbage around the perimeter of the site that should be removed. Summary of Construction Activities: Week of January 24,2022 (inspection January 27, 2022) An inspection was conducted by the ECM on Thursday, January 27, 2022. Taylor Andrews from the North Andover Conservation Department was present during my site inspection.Window installation continues on Building B and framework continues on Building C (see attached photo 14). Underground utility work within the Building A foundation is underway (see attached photo 16). The paved areas within the site were just recently swept(see attached photo 1S). The entire perimeter of the erosion control line was reviewed in the field with Mr.Andrews,and no breaching was observed and all erosion controls remain in-tact(see attached photo 17). The silt sacks within all the catch basins were inspected and several needed to be cleaned and/or replaced within the site (see attached photo 18). The Site Contractor was notified and he stated new silt sacks have been ordered. There are several small stockpiles of stone and salt throughout the site,but appear stable and are not located near any catch basin inlets. The contractor will be notified to stockpile snow piles in the appropriate locations after the upcoming storm. Catch Basins All catch basins within High Street, the access roadway and within the construction site have been inspected on a weekly basis. As stated above in the weekly reports, all basins have silt sacks which continue to be maintained. During the last inspection of the month,the Site Contractor was notified to clean and replace several of the deteriorated silt sacks. Erosion Controls The perimeter of the erosion control boundary was walked/inspected in its entirety during each of my inspections. Currently,all erosion controls remain in place and appear to sufficiently prevent any potential erosion issues over the winter months. Erosion controls will continue to be inspected and replaced as necessary or as instructed by the ECM. DEP Sign The replaced DEP sign has been posted on the chain link fence at the entrance of the Project Site and continues to remain in place. Stone/Salt/Snow stockpile Areas Currently,there are limited small stockpiles on site as the majority of soil materials on site have been used for grading activities or removed from the site entirely. Several stone piles remaining are covered in plastic and show no signs of erosion or runoff.At this time there are no snow stockpiles on site. Pavement Sweeping/Dust Control As the majority of the site is paved, pavement sweeping and dust control has dramatically reduced, however, the site will continue to be inspected to ensure paved areas are kept clean. No dust issues were observed during my monthly inspections, and the site was thoroughly swept prior to my last inspection in January. Ongoing Recommendations/Comments ® Continue to inspect and maintain erosion controls along the perimeter of the site. ® Continue to sweep the paved areas when silt build-up occurs. ® Inspect and the clean the catch basin silt sacks when necessary and remove sediment build-up weekly.Replace deteriorated silt sacks as instructed by the Environmental Monitor. ® Snow stockpiles should be located in appropriate locations as approved in the Stormwater Operation and Maintenance plan. Lots of old fill stone material was excavated from the foundation footprint and will be removed from the site and disposed of properly. The catch basin and surrounding inlet areas north of Building C has been cleaned. One of the catch basin silt sacks within the site needs to be properly fitted and the Contractor was notified to address this catch basin. The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact. The DEP sign was missing from the fence at the entrance of the site. The site Contractor was notified and will replace the sign that was taken down by the fence company when they added the extra screening. The Site Contractor continues to sweep the site and access roadway on an as-needed basis. Summary of Construction Activities: Week of December 6, 2021 (December 10, 2021 inspection) An inspection was conducted by the ECM on Friday, December 10,2021. Frame work on Building C and B continues and floor 3 is currently being framed on Building B (see attached photo 5). Sections of Building A's foundation has been poured (see attached photo 7).The excavated stone from the foundation footprint has been moved off-site. Current soil stockpiles on site are covered and will be used for backfilling activities within the Building A footprint. Currently 2 silt sacks within the site need to be repaired (one mentioned in last week's report).Upon speaking with the Site Contractor,the silt sacks have torn and 2 new ones are being ordered for replacement. The entire perimeter of the erosion control line was reviewed and no breaching was observed and all erosion controls remain in-tact. The DEP sign was still missing from the fence at the entrance of the site and the Site Contractor will be ordering a new one,if the fence company does not return the one taken down during fencing re- installation. The paved areas were observed muddy during my inspection The Site Contractor continues to sweep the site and access roadway on an as-needed basis. Summary of Construction Activities: Week of December 13, 2021(December 16, 2021 inspection) An inspection was conducted by the ECM on Thursday, December 16, 2021. The site was muddy and weather conditions were warm and no frost existed on the ground. Frame work continues on Buildings B &C (see attached photo 9). Foundation work continues on Building A(see attached photo 11). A breach through the chain link fence north of Building was observed in the field and ground disturbance to the meadow was documented (see attached photo 10). One of the Site Contractors was immediately notified and he was aware of the situation and stated it was going to be addressed shortly. The disturbance was caused by a lull machine slipping on ice and sliding into the buffer zone wiping out the fence. I recommended in an email to the Site Contractors that the area be hand raked back to original grade, the fence be re-installed and two layers of new silt socks be installed. Disturbed areas of the meadow should be seeded with an appropriate seed mix in the Spring. Two of the catch basins within the site still have damaged silt sacks that are currently on order. Muddy water was observed surrounding one of the basins (see attached photo 12) and the contractor was notified to place stone around the basin inlet if the sack can't be replaced quickly. All other catch basins were inspected and the silt sacks were properly functioning on the access roadways and within the site,with the exception of the two basins discussed above. The entire perimeter of the erosion control line was reviewed and no breaching beyond that identified behind Building B,was observed and all erosion controls remain in-tact. Summary of Construction Activities: Week of December 20, 2021 (inspection December 23, 2021) An inspection was conducted by the ECM on Thursday, December 23, 2021. It was a cold day and the unvegetated ground conditions were frozen. Frame work continues on Buildings B & C (see attached photo 13). Foundation work continues on Building A(see attached photo 14). The breached meadow area behind Building B has been restored and erosion controls have been re- installed but the fence has not yet been installed (see attached photo 15). The disturbed areas should be seeded in the Spring with an appropriate seed mix. The entire perimeter of the erosion control line was reviewed and no breaching beyond that identified behind Building B was observed and all erosion controls remain in-tact. All catch basins were inspected and the silt sacks were properly functioning on the access roadways and within the site.The two damaged silt sacks mentioned in last weeks'report have been repaired. Stockpiled soil materials were protected with a plastic covering. The pavement areas within the site were in need of a sweeping. Summary of Construction Activities: Week of December 27, 2021 (inspection December 29, 2021) An inspection was conducted by the ECM on Wednesday, December 29, 2021. It was a warmer day and the ground conditions were not frozen at the time of inspection. Frame work continues on Buildings B (4th floor construction underway) & C. Foundation work continues on Building A (see attached photo 17). The fence has not been re-installed behind Building B in the area that was breached several weeks ago (see attached photo 19). The Site Contractor has notified the fence company to re-install the fence as soon as possible. The entire perimeter of the erosion control line was reviewed and no breaching beyond that identified behind Building B was observed and all erosion controls remain in-tact. Water appears to run clear through the western retaining wall drainage pipes (see attached photo 19). All catch basins were inspected and the silt sacks were properly functioning on the access roadways and within the site. Ponded silty water was observed around one of the catch basins within the internal section of the site, however,the silt sack remains in-tact. The Site Contractor was notified to put some stone around the basin and/or sweep the surrounding paved areas to limit the amount of silty water flowing through the silt sack. Stockpiled soil materials continue to be protected with a plastic covering. Catch Basins All catch basins within High Street, the access roadway and within the construction site have been inspected on a weekly basis. As stated above in the weekly reports, all basins have silt sacks which continue to be maintained. Two of the silt sacks within the site were replaced and continue to function properly. Erosion Controls The perimeter of the erosion control boundary was walked/inspected in its entirety during each of my inspections. The breached erosion controls in the northern section of the site behind building B have been replaced. Currently,all erosion controls remain in place and appear to sufficiently prevent any potential erosion issues over the winter months. Erosion controls will continue to be inspected and replaced as necessary or as instructed by the ECM. DEP Sign The DEP sign posted on the chain link fence at the entrance of the Project Site has been missing for several weeks and a new one is on order. If the new sign is not available soon, I recommend a temporary one be posted. Soil/Stone stockpile Areas Currently,there are limited stockpiles on site as the majority of soil materials on site have been used for grading activities or removed from the site entirely. The several piles remaining are covered in plastic and show no signs of erosion or runoff. Pavement Sweeping/Dust Control As the majority of the site is paved, pavement sweeping and dust control has dramatically reduced, however, the site will continue to be inspected to ensure paved areas are kept clean. No dust issues were observed during my monthly inspections, however, I recommend the site be swept as paved areas were muddy during several of my latest inspections. Ongoing Recommendations/Comments ® Continue to inspect and maintain erosion controls along the perimeter of the site. ® Install the fencing behind Building B that was damaged by a piece of machinery. ® Continue to sweep the paved areas when silt build-up occurs. ® Install new DEP sign as soon as possible. ® Inspect and the clean the catch basin silt sacks when necessary and remove sediment build-up weekly. (the "Applicant"), 1, Julie Vondrak, serving as the Erosion Control Monitor ("ECM") am submitting this monthly monitoring report for weekly construction activities conducted within the Buffer Zone to wetland resource areas on the above referenced site.The following activities were documented during my weekly site inspections on November 4, 10, 18 &24, 2021. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4-story buildings, a clubhouse, parking areas, a pool, retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an.athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland("BVW') and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and 50-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACC") issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of November 1,2021 (November 4,2021 inspection) An inspection was conducted by the ECM on Thursday,November 4, 2021. Framework on Building B continues (see attached photo 1). The slab for Building C has been poured. Over excavation work is occurring within the footprint of Building A (see attached photo 4). Work on Building A will start after removal of the overhead power lines. Silt sacks have been installed within the catch basins located within the site (see attached photo 2). The one basin within the access roadway is missing a silt sack and it should be re-installed. Last month's report noted that one of the access roadway catch basins was buried from pavement activities,however,it must be clarified that the basin was moved further north within the parking spot areas (see attached photo 3).The southwestern catch basin within High Street had a lot of accumulated silt that should be removed. The entire perimeter of the erosion control line was reviewed and the Site Contractor was notified as to which areas needed immediate attention. No flowing water was observed from the new outlet pipe conveying stormwater runoff from Prescott and High Street. The Site Contractor continues to sweep the site and access roadway on an as-needed basis. Summary of Construction Activities: Week of November 8, 2021 (November 10, 2021 inspection) An inspection was conducted by the ECM on Wednesday, November 10, 2021. Taylor Andrews from the North Andover Conservation Department was present during the site inspection to review all existing conditions. Framing work continues on Building B (see attached photo S). No further construction has been conducted on Building C. Grading activities associated with the foundation for Building A are complete, however no foundation work can occur on Building A until National Grid removes the overhead poles. The entire perimeter of the erosion control barrier was reviewed during the site walk. The low area behind Building B continues to accumulate silt (see attached photo S). This area needs further reinforcement with additional erosion controls to prevent sediment breaching. Built-up sediment along the southern erosion control barriers has been removed and the erosion controls are in-tact and properly functioning. All catch basins within the site and access roadway have functioning silt sacks. The internal and site entrance roadways continue to be swept and were clear of sediment during the site inspection. Summary of Construction Activities: Week of November 15, 2021 (inspection November 18, 2021) An inspection was conducted by the ECM on Thursday,November 18, 2021.A rain event did occur this week and there were small pockets of standing water within unpaved areas on site. Framing work continues on Building B. No additional work has occurred on Building C or within the Building A footprint. The overhead poles have not yet been removed from the site. Erosion controls have been reinforced in the low area behind Building B (see attached photo 10) and no sediment breakthrough was observed on site. There was a low flow of water observed within the western wall drainage channel (see attached photo 9). One of the catch basins north of Building C (see attached photo 11) was covered in mud and the Site Contactor was notified to the clean the surrounding inlet area and the silt sack. All other catch basins were inspected and the silt sacks were properly functioning on the access roadways and within the site,with the exception of the one basin discussed above that needs cleaning. The entrance roadway and internal site roadways were free of sediment and appear to be swept on as as-needed basis. Summary of Construction Activities. Week of November 22, 2021 (inspection November 24, 2021) An inspection was conducted by the ECM on Wednesday, November 24, 2021 prior to the long holiday Thanksgiving weekend. Active work on site during my inspection was limited to framing work on Building B. No new construction activities have occurred on Building C or within the Building A footprint. The erosion control perimeter was walked in its entirety and all erosion controls were in-tact and properly functioning. The ground was frozen and no signs of erosion were identified. All catch basins were inspected and silt sacks appeared to be properly functioning. The catch basin north of Building C still had accumulated silt around it and the Site Contractor was notified immediately. It was recommended that either stone be placed around the inlet grate or that erosion controls be installed as the base of the upgradient unvegetated slope. Catch Basins All catch basins within High Street, the access roadway and within the construction site have been inspected on a weekly basis. As stated above in the weekly reports, all basins have silt sacks which continue to be maintained. Only 1 basin north of Building C needs immediate attention. Erosion Controls The perimeter of the erosion control boundaries was walked/inspected in its entirety during each of my inspections. All deteriorated erosion controls mentioned in earlier reports and weekly inspections have been addressed and the erosion controls should be sufficient to address any potential erosion issues over the winter months.Built-up sediment has been removed from the southern border barrier and the low area north of Building B has been reinforced and will continue to be monitored. ' The erosion control boundaries will continue to be inspected and sections will be replaced when necessary or as instructed by the ECM. DEP Sign The DEP sign is posted on the chain link fence at the entrance of the Project Site and remains in-tact. Soil/Stone stockpile Areas Currently,there are limited stockpiles on site as the majority of soil materials on site have been used for grading activities or removed from the site entirely. Pavement Sweeping/Dust Control As the majority of the site is paved, pavement sweeping and dust control has dramatically reduced, however, the site will continue to be inspected to ensure paved areas are kept clean. No dust issues were observed during my monthly inspections. Ongoing Recommendations/Comments ® Continue to inspect and maintain erosion controls along the perimeter of the site. ® Immediately address the sediment around the catch basin inlet north of Building C. ® Inspect and the clean the catch basin silt sacks when necessary and remove sediment build-up weekly. Permitted Project Summary The proposed Project includes redevelopment of an historic mill site including demolition of an existing parking lot and construction of a 170-unit residential apartment complex consisting of two 4- story buildings, a clubhouse,parking areas,a pool,retaining wall construction and associated utilities, stormwater management structures, buffer zone restoration plantings and landscaping activities. Work will be located within previously disturbed areas containing an athletic field and parking lots. Activities will be located within the 100-foot Buffer Zone to Bordering Vegetated Wetland ("BVW") and there are no proposed impacts to resource areas or the local 25-foot No-Disturbance Zone and SO-foot No-Build Zone enforced under the North Andover Wetland Protection Bylaw. The North Andover Conservation Commission ("NACU) issued an Order of Conditions ("OOC") approving the Project with conditions on June 3, 2020. In accordance with the Conditions of the OOC and the Planning Board Special Permit issued February 26, 2020, the Applicant is required to obtain the services of an Erosion Control Monitor/Wetland Scientist("ECM") to oversee the placement of the erosion control measures and to conduct weekly inspections and monthly monitoring reports throughout the construction duration of the Project and following significant rain events. SUMMARY OF CONSTRUCTION ACTIVITIES Summary of Construction Activities:Week of October 4, 2021 (October 8, 2021 inspection) An inspection was conducted by the ECM on Friday, October 8, 2021. The site is currently being graded and prepared for paving activities scheduled for the following week.The elevator shaft within Building B has been completed and the stairway is close to completion. The access road has also been prepared for paving activities. The erosion control barrier remains in tact and will continue to be inspected over the next several weeks to determine which areas need replacement prior to winter conditions. Several areas of the southern erosion control barrier have accumulated silt to the top of the silt socks and should be cleaned and/or replaced in the upcoming weeks. After pavement activities are completed,the catch basins will be fully inspected to determine if the silt sacks need to be cleaned. Summary of Construction Activities.Week of October 11,2021 (October 14, 2021 inspection) An inspection was conducted by the ECM on Thursday, October 14, 2021. The site and the entrance roadway has been paved (see attached photo 4) and curbing and sidewalk construction will occur over the next week. The catch basins within High Street and the access roadway were inspected and some silt build-up was observed around the basin inlet grates. The area immediately surrounding one of the catch basins within the entrance roadway has not been fully paved and the silt sack should be cleaned when work has been completed. Stairway shaft work continues within Building B. Building C slab has not yet been poured. Erosion controls need reinforcement in the low area behind Building B (see photo 7 attached) and the Site.Contractor was notified that this needs to be addressed immediately. There is a stockpile of soil materials adjacent to the erosion control barrier north of Building B, which in my understanding will be removed soon. Upon removal, the erosion controls should be replaced/reinforced as they are partially covered with soil (see attached photo S). As stated in last week's report above,the southern border erosion controls should be replaced or reinforced prior to the winter season. Soil stockpile areas on site are limited to within the footprint of Building A (see attached photo 6). The materials will be used on site over the next several weeks and the remainder of the material will be hauled off-site. There was no flow observed from the outlet pipes in the western retaining wall. Summary of Construction Activities: Week of October 18,2021 (inspection October 21, 2021) An inspection was conducted by the ECM on Thursday, October 21, 2021.No rain events occurred this week. The entrance roadway is paved and granite curbing has been installed (see attached photo 8). Interior granite curbing work is underway. Framing has started on Building B. There was no flow through the western wall drainage outlet pipes at the time of my inspection. The erosion controls north of Building B were being reinforced/replaced during the time of my inspection. All catch basins were inspected and the silt sacks were properly functioning on High Street. It appears one of the catch basins within the access roadway has temporarily been covered by the paving activities but will be exposed when paving and roadway activities have been completed. Several of the catch basins within the site do not have silt sacks and the Site Contractor stated the silt sacks are on order and will be installed when they arrive. The entire perimeter of the erosion control barrier was inspected and remains in-tact. As stated above, sections of the erosion control barrier were being replaced and the Site Contractor has been advised of areas that will need reinforcement before the winter. Summary of Construction Activities:Week of October 25,2021 (inspection October 27, 2021) An inspection was conducted by the ECM on Wednesday, October 27, 2021 following Tuesday's rain event.Frame work continues on Building B and Building C foundation will be poured by the end of the week. The entire perimeter of the erosion control line was inspected. There was some new silt runoff from the heavy rain storm,however all silt was contained by the erosion control barrier. Some trash build up against the erosion control barriers was observed and the Site Contractor was notified to clean the areas.Standing water was observed in the baseball field north of the newly reinforced erosion control barrier.Water continues to pond on the sandy areas that occupied the baseball field. A small area of older silt accumulation should be removed by hand. An application of a meadow seed mix may be beneficial in the Spring of 2022 to further stabilize the unvegetated sandy substrate. Water flow through the drainage channel at the western wall was observed during my inspection. The water appeared to be running fairly clear and no siltation build-up was observed. Dewatering from the Building A footprint was occurring during my inspection. The clear, silt free water was sheet flowing over the pavement into a nearby catch basin (see photo 16). The silt sacks within the catch basins on High Street and the access roadway were inspected. There was a lot of new leaf build-up around the catch basins on High Street and should be removed. One of the catch basins on the access roadway is still covered. As stated above, the Site Contractor is awaiting for the additional silt sacks to be delivered so they can be installed within the site. Catch Basins All catch basins within High Street, the access roadway and within the construction site have been inspected on a weekly basis. As stated above in the weekly reports, silt sacks remain in-tact, and several basins need new silt sacks. Accumulated debris continues to be addressed and the basins will continue to be monitored to ensure the silt sacks are properly functioning. Erosion Controls The perimeter of the erosion control boundaries was walked/inspected in its entirety during each of my inspections. Several areas of the erosion controls have deteriorated due the excessive rains this season and the Site Contractor continues to be notified as to which areas need to be addressed. The one low area in the north continues to be reinforced and the small area of silt breakthrough continues to be cleaned. The erosion control boundaries will continue to be inspected and sections will be replaced when necessary or as instructed by the ECM. DEP Sign The DEP sign was posted at the entrance to the construction site on the chain link fence at the beginning of October, but was taken down during paving activities, as that section of fencing was removed. The Site Contractor was notified to replace the DEP sign as soon as possible. Soil/Stone stockpile Areas Soil stockpiles over this month were limited to the area within the footprint of Building A. Piles appeared stabilized with no signs of runoff observed. Pavement Sweeping/Dust Control As the site has been recently paved, pavement sweeping and dust control has dramatically reduced, however, the site will continue to be inspected to ensure paved areas are kept clean. No dust issues were observed during my monthly inspections. Ongoing Recommendations/Comments • Inspect and maintain the erosion controls around the perimeter of the site. Deteriorated sections will continue to be identified and replaced prior to the winter freeze, especially along the southern site border. • Hang the DEP sign in a visible location. • Inspect and the clean the catch basin silt sacks when necessary and remove sediment build- up/leaves weekly. Silt sacks should be installed in all internal catch basins as soon as they arrive on site.
