Andover, MA-40202015 PAGESIENCL: BACKGROUND/PURPOSE: GSD Associates has been asked by the owner's representative to conduct periodic observations of the construction to verify that the work is in conformance to the design and specifications. Scott Construction has started construction. Foundations and a large majority of the masonry and site work has already been started or completed prior to our initial visit. Concern has been expressed that the masonry was not properly protected from freezing. GSI Testing has been hired to test masonry mortar and grout in situ to determine if it is in compliance with the specifications. PRESENT AT SITE: Mark Griffin, Assistant Superintendent for Scott Construction: Christina Minicucci, Project Manager for SUDS: Bryan Levesque, Geotechnical Services Inc: Gregory Smith, AIA, GSD Associates. Masonry Sub- Contractors. WORK IN PROGRESS: (Refer to attached sketch) Masonry work was ongoing. Portions of the CMU along the West, North and East walls have been completed full height. Interior walls have been partially constructed as shown on the plans. Other interior walls have not been started. The North and South short walls of the long automatic car wash bay have been tented and heated. GSI representative was at the site conducting physical tests of the mortar with a Windsor Pin System testing aperatus. I was provided with a set of plans including the Cover sheet, A-1 through A-5, F-1 and F-2. and one section of the specifications Section 04100 Mortar and masonry Grout. OBSERVATIONS: 1. Not all walls have been protected and not all wails are being protected from freezing. I am not aware of what time the walls have been constructed in the past and cannot comment on the adequacy of protection without knowing when the walls were constructed. 2. CMU Walls being erected are not braced and are not being vertically reinforced as they are constructed. Adequacy and the requirements of the bracing should be determined by the project's structural engineer and or the specifications. 3. CMU walls appear to be reinforced horizontally with diagonal ladder truss reinforcing. Vertical reinforcing may not be possible to install full height due to height of wall, diagonal reinforcing and cell wall obstructions. 4. Interior CMU walls are being protected by insulated tarps and may not be adequate to resist freezing in cold weather. 5. A mortar additive was stated as being used to minimize freezing of the mortar, no information was provided to us at our visit. INFORMATION OR ACTION REQUIRED: 1. A full set of drawings and specifications will be required. 2. Review by the project engineer for bracing requirements of the unreinforced CMU walls. 3. Tenting and heating of the masonry needs to be completed for all new walls being constructed. 4. Specifications and information as to the type of mortar additive needs to be submitted. 5. Foundation Permit Number 307,dated December 03, 2001 is the only permit issued for this project. Building construction is not allowed under this permit.. The Building Permit application required other drawings and supporting documentation for review prior to the issuance of the Building Permit itself. The drawings and documentation was not received until the afternoon of January 31, 2002, THE BUILDING PERMIT FOR THIS PROJECT HAS NOT BEEN ISSUED . However,the masonry walls of the structure are approximately 75% complete with out benefit of the building permit or notification that work was on going to the building department. Mr. Michaud, you made the statement that you were using"anti-freeze"in the mortar mix when asked why the cold weather precautions were not being taken. I am of the opinion that you are in violation of the Massachusetts State Building Code 780 CMR 2112.3 --Cold-Weather Construction. It was evident,yesterday,that you did not agree with this determination and I respect your opinion in this shatter, However,then,please refer to Division 04 MASONARY of the Technical Specifications as prepared by Koch Architects for the project. Pursuant to Division 04,Paragraph METHODS, Item 05 "Do not use antifreeze agents,masonry cement or calcium chloride" --- Scott Construction Company, MT. Michaud as agent,have not followed the specification of the Architect for masonry construction for the project. As such,Mr. Michaud, the purpose of this correspondence is to reaffirm the verbal STOP WORK ORDER that was issued on February 60'. Scott Construction Company, Inc. is to submit a report, acceptable to the building department, that the masonry structure as constructed will meet the Arcl tedural Specifications and the Massachusetts State Building Code. It was noted that there are two(2)signs erected on the property. One sign is a construction sign and the other is a temporary"coming soon" sign. Robert Nicetta Building Inspector Town of North Andover 27 Charles Street North Andover, MA 01845 RE: 740 Turnpike Street Carwash Dear Mr. Nicetta: Thank you for taking the time to speak to me this morning regarding the carwash and allowing me the opportunity to address some of the concerns that you have. In light of the insurgence of a very quick cold front,we would like to suggest the following to address your concerns about winter conditions. I. We would immediately seek your authorization to do the staging and tenting of the interior walls. 2. Upon completion of the interior walls,we would construct the exterior staging along the front of the building to tent and enclose this entire area. 3. This would enable us to heat the area as required, If this proposal meets your requirements,we would begin work this Wednesday February 13. In addition, today we have contacted GSI technical engineers. They are currently in the process of locating a probing device that will analyze the strength of the mortar joints. It appears that if a machine can be located, they will begin the testing tomorrow. Flashing arrow boards will be used at all times when operations occupy the roadway and shall be available for use at all times. All warning devices shall be subject to removal, replacement and repositioning by the Grantee as often as deemed necessary by the Engineer. There shall be a Reimbursable Number for State Engineering Personnel while inspecting this j ob. Free flow of traffic shall be maintained at all times. Two way traffic shall be maintained at wll times. When in the opinion of the Engineer, this operation constitutes a hazard to traffic in any area, the Grantee may be required to suspend operations during certain hours and to remove U,s equipment from the. roadway. The Grantee will be responsible for any damage caused by his operation to curbing, structures, roadway, etc. The Grantee shall be responsible for any settlement :w,hich may occur as a result of the work done under this permit.
