Box 3039 Andover, MA 01810 RE: Rt. 114 Car Wash Dear Brian, Per your request, I have visited the site on October 1, 2002 and met with the Maurice Michaud of Scott Construction regarding our most recent site observation report#4 dated 9-27-2002. Maurice indicated that they were in the process of completing the deficient items included in our report. A. The items that need to be addressed at that time were as follows: 2. The Structural Engineer provide a stamped letter stating that the masonry and the masonry openings have been designed in accordance with the building code specifically the seismic requirements. 3. Installation of a fire rated ceiling assembly at the ceiling above the boiler rooms. 4. Fire rated enclosure of the stairwell ceilings needs to be completed. 5. Fire rated and factory labeled doors and frames are required and are not provided. 6. Penetrations through fire rated assemblies and CMU wails from the mechanical room to be fire rated. 7. Removal of temporary wiring/or installation of Junction boxes at exposed wire connections. 8. Fire rating of the supporting structure of the stairwell framing. B. In response to the above list I have received a letter dated Oct. 1, 2002 from the Architect Mr. Rainer Koch generally stating the following: (See Attached) - The doors are to be changed to a "C° labeled fire rated doors of 45 Min. (See comment below) - The stairwell ceiling should be constructed to enclose the stairwell in a one hour rated enclosure per UL Design L522. - The ceiling of the mechanical/boiler room shall be rated 1 hour per UL Design L522. - Fire Rate around all pipes and conduits per UL Design W-J-1100 Paint steel at support structure of stairwell with fire retardant paint. (See comment below) C. With respect to the Architects letter above, the following items need to be further addressed by the Architect , Doors are required to be rated for 60 minutes in exit enclosures per Table 716.1. - The paint specified is a fire retardant paint and does not provide the fire ratings as required by the code for structures supporting one hour rated assemblies. The paint/coating system needs to be rated for 1 hour, I reviewed these two items with Brian Darcy for follow up with the Architect& Contractor. D. I received a letter from Sabbagh Associates, Inc stating that the building masonry and masonry openings were designed to meet the Massachusetts State Building Code. This item is completed (See Attached letter) E. On Thursday October 3, 2002, 1 again visited the site to observe the condition of the remaining items that have not been resolved. 1. Installation of the fire rated ceiling assembly in the mechanical room. 2. Installation of a fire rated enclosure at the ceiling of the stairwell. 3. Installation of 60 min. labeled fire rated doors. 4. Completion of the rated penetrations through walls and ceilings. 5. Removal of the temporary wiring and enclosure of wiring in junction boxes. 6. Fire rating of the supporting structure of the stairwell, Upon my visit the following was noted. Installation of the fire rated ceiling assembly had not yet started. Installation of the fire rated ceiling in the stairwell had not yet started. Installation of the fire rated doors was underway and they are B labeled doors of 1 hour rating. The purpose of the visit was to discuss the outstanding issues that remain, and to do a final review and Inspection for the Owner. OBSERVATIONS: 1. Met at the site to discuss two remaining issues that I have not had adequate responses to from the Architect/Structural Engineer, and changes to fire rating assemblies and information shown on the drawings. 2. Scott Construction stated that they will have a stamped letter from the engineer specifically addressing the Masonry and the Masonry openings being designed to meet the requirements fo the Building Code and specifically the seismic requirements of the code. 3. FRP panels on plywood have been completed on the ceiling at the Mechanical Room. We had previously brought this to the attention of the contractor that this was a deviation from the drawings shoving a 1 hour fire rated ceiling. The Architect's Drawings indicated that the ceiling was to be 1-hour rated, and the drawings indicated a Gypsum Ceiling System and Underwriters Labratories(UL)-Design number L 502 for the rated Floor/Ceiling Assembly. According to the UL Fire Resistance Directory, L502 is a Floor/Ceiling Assembly that has a number of requirements of which a gypsum ceiling, framing, and plywood deck with 1 inch thick floor toppings of various cementitious design options. (see attached UL L502 Specification. This UL L502 system has not been constructed from my observations in any area including the area of the stairwell. This cementitious floor topping system is also probably impractical, and a more modern assembly should be investigated. The FRP on Plywood is not indicated on this UL design. I am not aware of a UL rated system using this material at this time. In my opinion the ceiling system design installed should be changed or a new UL Design or other labratory testing certification needs to be provided for the system as installed. I would suggest that a possibility of UL design L513 maybe more appropriate as a topping is not required if constructed according to specifications listed. Other fire rated systems that may be possible are Factory Mutual Research Corp. (FM) FC-172, or Gypsum Association (GA) FC5406 or(GA) RC2601 which uses a double layer of Gypsum Wallboard to obtain a fire rating without a topping. For pitched roof truss areas without a floor deck UL P533 might also be possible. These are possible suggestions that should be reviewd and specified by the Architect. The Architect stated that this is not the intent of the drawings that the mechanical room be rated and that only the small area of the ceiling under the stair was to be rated. However, I still disagree with his statement for two reasons. A.)The code requires a fire rated enclosure of Boiler and Furnace rooms per Table 302.1.1. There are clearly two largo boilers/furnaces located on the new mezzanine area with two exhaust flues exiting through the roof. However,the Architect stated that there are not any boilers/furnaces in this room during our meeting. B.)The statement the Architect makes that the fire rated ceiling is only required under the stair landing area is not what the code requires. Section 709.0 Fire separation assemblies specifies the requirements for rated assemblies. In 709.4 Continuity, and in 713.2 Continuity, the code requires that the walls be continuous from the top of the fire rated assemblies belowto the underside of the floor or roof slab or deck above. It also requires that the supporting framing of the fire rated assemblies be fire rated also. The concept here is that a 1 hour fire rated wall cannot be supported from an unrated structure. The condition that the Architect states is the intent of the drawings is not allowable by code since the fire rated assembly with sheetrock is only located in a Tx3'+-section of the ceiling, leaving the rest of the supporting structure unrated. The top ceilings of the stairwells are also simarly not rated or meet the requirements for continuity as specified by the code in 709.4 4. The Boiler/Furnace flues penetrating the ceiling are required to have a T'clearance to combustable materials. The blown- in insulation around this flue is up tight to the flue and should be protected by a collar to keep the loose insulation from touching the flue. The penetration should be installed through the fire rated ceiling per 709.0, and 713.0 of the code. 5. There are exposed wires and wire nut connections not located in a Junction box above the stair area with blown-in insulation hanging from these open connections. This was stated to be temporary wiring that has not yet been removed. The GC.should verify if this is temporary and remove it or provide a junction box in accordance with electrical code, 6. Doors into the fire rated stairwells are not labeled as required by Section 716.0 Fire Door Assemblies and specifically 716.2.1 of the code requires that the labels be applied in factory, and not field applied as was the statement made in the field. INFORMATION OR ACTION REQUIRED: 1. The Structural Engineer provide a stamped letter stating that the masonry and the masonry openings have been designed in accordance with the building code specifically the seismic requirements, 2. Installation of a fire rated ceiling assembly at the ceiling above the boiler rooms. 3. Fire rated enclosure of the stairwell ceilings needs to be completed. 4. Fire rated and factory labeled doors and frames are required at all openings in all rated walls and are not provided. 5. Penetrations through fire rated assemblies and rated CMU walls from the mechanical room to be fire rated. 6. Removal of temporary wiring/or installation of Junction boxes at exposed wire connections. 7.
