The proposed Log and Hay Erosion Check Dam shall be construct pd.l as shown on the above referenced plans . Before this is done all catch basins and pipes draining. to. this' area shall be cleaned. This Check Dam shall be completed before any work begins on the headwall, and shall be cleaned as needed, or as directed by the Conservation Commission and/or the Highway Surveyor. The Check Dam shall remain in place until all erodable surfaces of the Lakeside Develo went have been permanently stabilized (by July 15, 1981 . : , Upon 1 its removRl , all accumulated sediments shall be removed from this channel , al the direction of the NACC. 14. Hand-place rip-rap, approved by the Highway Surveyor, shall be placed along the western bank of the existing nhannel , t o Pv.'_vOnt e osi on of this bank, caused by the def l eaLlon of ,Ire ilna�;e waters . 15 . The applicant shall dredge the heavily sedimented areas at the end of the defined and existing charnel , under the direction of the Conservation Commission. This dredging shall remove the sediment created through previous dredging and the stoc_kpil ! "8 of the dredged material, and from sedimentation via drainage waters. This dredging shall return this area to a state reasonably resembling its natural state. The Conservation Commission, and the Highway Surveyor, shall be notificad 48 hours in advance of the commencement of work, to approve procedures to the ksed in the dredging operations . 16 . All hay bales (and loose hay) shall be cleaned from the brook and adjacent Wetlands on the southern side jof Pleasant Street , upon the completion of all work. 17. After all work is completed in the headwall area, those areas not vegetated shall be grubbed and seeded as specified in the above refreenced plan (condition 11) . Upon requesting a Cirtificate of Compliance, they applicant nhnq submit a letter from the professional who pcepared the plans , stating that all work has been completed in accord-,nee with this Order of Conditions . The applicant shall .also submit an as-built topographical plan of .this site, detailing, but not limited to, the following: a, Headwall structure location with invert elevations and pipe slopes . b. Grading of all associated areas ; c. Location of shoreline. . d. Relationship of headwall to existing headwall, catch basins, edge of pavement, and other pertinent features, 19. All work shall be completed by Augustml, 1981. . However, the Check Darn shall not be removed until after all slopes and exodeable surfaces within the Lakeside Development have been permanently stabilized (see condition 13) . The slopes ' adjacent to this site, or on this site, shall be stabilized Rs soon as possible. 20. At,y changes in Me s"KI ted plans wi 11 require that the applicant c.:ume befoi e the Conservation Commission to determine the significance of said changes . Should said changes be significant to any of the interests of the Wetlands Protection Act, the Public Hearings may be reopended and additional con- ditions may be imposed to adequately protect the interests of the Act, Errors in the plans shall be considered as changes . 21 .
