Re: Subsurface Sewage Disposal System Plan for(Lot 5) 9 Hawkins Lane (Map 106C,Lot
18)
Dear Mr.Nemchenok:
The proposed wastewater system design plan for the above site dated July 20, 2022 has been
reviewed. Unfortunately, the plan cannot be approved until the following items are corrected.
The specific section in Title 5: 310 CMR 15.000,or North Andover regulation that is not met by
this design follows each item where applicable.
1. Provide the street address of the parcel per the Town's records (15.220)
2. Provide the names of all abutters (NA 3.2)
3. Provide the name and address of the owner of record (NA 3.2)
4. Provide the legal boundaries of the property(15.220)
5. Provide the lot line dimensions (NA3.2)
6. Provide for and depict a foundation drain, or indicate one is not to be provided(NA 3.2)
7. Provide a benchmark within 50'-75' of the proposed onsite wastewater system(15.220)
8. The distribution box is to be H-20 rated. This is noted in some areas of the design plan
but a different rating is provided in other locations of the design plan((NA 3.2)
9. The notation regarding site survey having been made on the ground should be signed by
the engineer(NA3.2)
10. It appears that the bottom of the septic tank may be below the ESHWT based on
extrapolation of information in test pits. Provide buoyancy calculations or explain if not
required(15.221).
11. It appears the tank pipe penetrations might be within 12"of the ESHWT. Confirm if this
is the case and adjust accordingly if needed(15.227)

12. Visually depict the proposed tank to have tees located beneath the tank openings (15.227)
13.Notate the distribution box outlets are to be at the same elevation(15.232)
14. Provide the slope of the pipe from the tank to the distribution box,provide a tee or baffle
if needed(15.232)
15. Provide soil evaluation report forms and the location and data on the design plan for TP 1
and TP 2
16. Provide for pea stone which has been washed(15.247)
17. Provide for leach stone which has been double washed(15.247)
18. Grading is proposed to be placed past the lot line. This is not allowed(15.211)
19. The four proposed trenches are not labeled on the Plan
20. The locations of the soil examination test pits and percolation tests are not correctly
depicted. There is also a tree noted in the lawn area which does not exist. Correct this.
