Re: Onsite Wastewater System Plan for 9 Hawkins Lane (Map 106C,Lot 18)
Mr. Scott,
The proposed onsite wastewater system design plan for the above site dated July 20, 2022 with a
final revision date of October 7,2022 has been approved for issuance of a Disposal System
Construction Permit(DSCP). Once issued, a DSCP will authorize the construction of the onsite
wastewater system per the plan referenced above and as allowed under Title 5 of the
Massachusetts Environmental Code (310 CMR 15.000). You will need involvement from the
designer of the onsite wastewater system throughout the installation process so you should be
sure to coordinate with them.
The design has been approved for use in the construction of a new on-site wastewater system for
a 4-bedroom(maximum 9-room)home utilizing the proposed septic tank and a standard stone-
and-pipe soil absorption system indicated on the design plan.
This design plan approval is valid for 2 years from the above date. During this time, a licensed
septic system installer should obtain a permit and complete this work, and a Certificate of
Compliance be endorsed by the installer, designer and the Town of North Andover. If these
activities are not completed within this time period then it is possible a new design plan will need
to be completed and submitted for review.
This approval is subject to the following conditions. Please be sure to review these with the
designer and installer.

Prior to the issuance of a Disposal System Construction Permit:

a. The area of the proposed Soil Absorption System(SAS) is to be staked and
flagged to identify the location(s) and to prevent activities which might damage
the soil absorption system. Stockpiling of materials and equipment within the
SAS area is prohibited
b. The applicant must submit a foundation as-built at the same scale as the approved
plan with the proposed onsite wastewater system components also depicted along
with ties to the foundation
c. Prior to the issuance of the Disposal System Construction Permit,the applicant
must submit the floor plans of the proposed dwelling showing no greater than 4
bedrooms or a total of 9 rooms
d. Prior to issuance of the Disposal System Construction Permit a signed statement
from the owner of 39 Hawkins Lane is to be provided providing their consent to
remove the wall around the swimming pool and patio as indicated on the design
plan
e. Prior to issuance of the Disposal System Construction Permit a draft easement and
easement plan are to be provided for review. If acceptable to the Health
Department, a final version containing such language is to be recorded on the
deed for 39 Hawkins Lane
Throughout the project:
a. If site conditions are found in the field to be different from those indicated on the
design plan, installation of the onsite wastewater system shall stop and the
designer and this office notified within 24 hours
b. If groundwater is present at the bottom portion of the excavation for the soil
absorption system(SAS)the excavation is to be dewatered prior to the placement
of any fill. No fill is to be placed for the SAS during rain or snow storms. Prior to
placement.of any needed fill in the SAS,the bottom is to be scarified and
relatively dry and the fill material is to be stockpiled at the edge of the excavation
and filled in gradually
c. Inspections are to be performed by the designer of the onsite wastewater system
as needed to properly oversee and document construction. At a minimum these
are the initial excavation,prior to backfilling, and during backfilling as well as
any other component of the construction which the designer wishes to examine
d. Inspections are to be completed by the Town of North Andover at a minimum as
follows:
i. Bed bottom
ii. Tank bottom
iii. Final installation of components, confirmation of proper component
selection, examination of construction methodology, and system operation
iv. Final grade
v. Sand fill will be collected from at least one location and analyzed for
compliance with specifications in Title 5
e. No components are to be covered until a final component inspection has been
completed

f. No excavation for or work associated with construction of the soil absorption
system is to occur from November 30 to April 1
After construction:
g. Within 30 days of completion of construction the installer and designer are to
confirm if the installation work was completed in compliance with the design plan
referenced in the DSCP and with Title 5, and if so shall certify this on a
Certificate of Compliance form
h. If changes have been made to the approved plans then preparation of an as-built
plan is required to be completed by the designer. If no changes have been made
to the approved plans then the approved plan shall be modified by the designer to
show the distances from a known structure to the system components.
i. No sewage shall be discharged to the onsite wastewater system until a Certificate
of Compliance has been issued by this office
j. No application for a Certificate of Occupancy may be requested from the Building
Inspector until a Certificate of Compliance has been issued by this office
In addition, you should be aware of the long-term operation guidelines of an onsite wastewater
system(these are not part of the DSCP but are mentioned below as information for the property
owner):
1. An onsite wastewater system can function as an integral part of the infrastructure of
the property if it is properly operated and maintained. This is the responsibility of the
owner of the property and is generally not specified by regulation. You are
encouraged to contact a wastewater system pumper/service provider to partner with
you to implement steps needed to assure the longevity of the onsite wastewater
system being installed at this location
The issuance of a DSCP does not construe and/or imply compliance with any permits, approvals
or licenses which might be needed from other agencies such as the Conservation Commission,
Zoning Board, Planning Board, Building Inspector, Plumbing Inspector and/or Electrical
Inspector.
