Permit (l)SC II). Once issued, a DSC P will authorize the construction of the onsite wastewater system per the flan referenced above and as allowed under Title 5 of the Massachusetts I rrvironnaental C"ode. (310 C'MR 1.5,000). You will need involvement from the designer ofthe onsite wastewater system throughout the installation process so you should be sure to coordinate with them. The design has been approved for use in the construction ofan upgrade; on-site wastewater syst:ern for a 4-bedroom (maximum 9-room) home utilizing the proposed septic tank and a. standard stone-and-pipe soil absorption system as indicated on the design plan. The goal for system upgrades is to achieve full compliance with state and local orrsite. wastewater regulations. In this instance full compliance was not achieved because the design includes a. request for Local Upgrade Approvals seeking to vary from the standards of full compliance. This letter serves to document the endorsement ofthese I-ocal Upgrade Approvals by the North Andover Ilealth. Iepartment. This design plan approval is valid for 2 years from the above date. During this time, a licensed septic system installer should obtain a permit and complete this work, and a Certificate of Compliance be endorsed by the installer, designer, and the 'Town ofNorth Andover. In the event an imminent health problem such as sewage backup into the dwelling is occurring the North Andover Board of Health may reduce the, time period for which this plan is valid. This approval is subject to the following conditions. Please be stire to review these with the designer and installer. Prior to the issuance of a Disposal System Construction Permit: a. Three additional paper copies of the design plan are to be provided to the office b. The area of'the proposed Soil Absorption System (SAS) is to be staked and I'lagged to identify the location(s) and to prevent activities which might, damage the soil absorption systern. Stockpiling of'rilaterials and equipment within the SAS area is prohibited Throughout the project: a. If site conditions are fou (I in the field to he diflerent, from those indicated can the design plan, installation ot'dic orisite wastewater system shall stop and the designer and this office notified within 24 hours b. If groundwater is present at the bottom portion of the excavation for the soil absorption system (SAS) the excavation is to be dewatered prior to the placement of any fill. No fill is to be placed for file SAS during rain or snow storins. Prior to placement ofany needed rill ire file SAS, the bottorn is to be scarified and relatively dry and the fill material is to be stockpiled at the edge of the excavation and filled in gradually. c. Inspections are to be performed by the designer ol7the onsite wastewater system as needed to properly oversee and document construction. At a rilinirmlin these are the initial excavation, prior to backfilling, and daring backfilling as well as any other component of the C011StRICtion,which the designer wishes to examille d. Inspections are to be completed by the Town of*North Andover at a mininiurn as follows: i. Bed bottom ii. Tank bottom iii. I inal installation of'cornponents, confirmation ot'proper component selection, examination ofconstrUCtioll methodology, and system operation iv. Final grade v. Sand fill will be collected from at least one location and analyzed for compliance with specifications in Title 5 c.
