Street Acceptance Documentation Checklist Lots 1-7, Rea Street,North Andover, MA 1. Certificate of Completion request(Form K) attached and completed as much as possible thus far prior to review of Planning and DPW and/or the inspecting agent. 2. Request(s) to the DPW requesting a final punch list of outstanding items to be completed within the Subdivision attached. 3. Copies (2) of the as-built plans, including stone bound certification, attached. 4. Copies (2) of the layout plans attached. S. Certificate of Compliance from Con. Comm. requested on 6. NIA- Instruments suitable for recording running to the "Inhabitants of the Town of North Andover" for all easements which are not a part of the street or streets (drainage, sewer, water). 7. Draft deeds for the street:itself from the current owners of Lots 1 through 7, inclusive, and the developer, running to the "Inhabitants of the Town of North Andover" attached. 8. NIA—Draft deed(s) of gift in trust for"open space"parcels. 9. Draft certificate to be signed by the applicant's attorney certifying that all necessary parties have signed all easement instruments and street deeds will be submitted for review upon approval of the submitted documents by the Town. 10. List of all current mortgagees on Lots 1 through 7 attached. There are no mortgagees holding liens solely on the street, easements, or open space. Abutter's list forthcoming, if necessary. Request for a Public Hearing by the Board of Selectmen for Street Acceptance pursuant to Massachusetts General Laws, Chapter 82, Section 22 attached. 11. Certificate of Notice will be provided upon further direction by the Board and scheduling of the hearing to be referenced in the notice. Sample notice and attorney's certificate attached for review. All legal documents conveying a fee interest and all necessary easements to the Town have been received and reviewed.
