North Andover, MA 01845 2e: Subsurface Sewage Disposal System Plan for 531 Johnson Street (Map 98A Lot 14 Dear Mr. Sorrin: The proposed onsite wastewater system design plan for the above site dated 2/2/24 with a final revision date of2/27/24 has been approved fur issuance ofa Disposal Syste►n Construction. permit (DS("P). 011ce issued, a DSCT will authorize the construction ofthe onsite wastewater system per the plan referenced above and as allowed under Title 5 ofthe Massachusetts Environmental Code (310 C'MR 15.000). You will need involvement from the designer ofthe onsite wastewater system throughout the installation process so you should be sure to coordinate with them. The design has been approved for use in the construction of an upgrade on-site wastewater system or a 3-bedroom home utilizing the proposed eftluent filter in the septic tank, a pump and pump tank and a standard stone-and-pipe soil absorption systern as indicated on the design plan. This design plan approval is valid for 2 years from the above date. During this time, a licensed septic system installer should obtain a permit and complete this work, and a Certificate of Compliance be endorsed by the installer, designer, and the Town of North Andover. If these activities are not completed within this time period then it is possible a new design plan will need to be completed and submitted for review. In the event an imminent health problem such as sewage backup into the dwelling is occurring the North Andover Board of health may reduce the time period for which this plan is valid. Please be sure to review these with the designer and installer. Prior to the issuance of a Disposal System Construction Permit: a. 'Three additional paper copies of the design plan are to be provided to the office b. The area ofthe proposed Soil Absorption System (SAS) is to be staked and flagged to identify the location(s) and to prevent activities which might damage the soil absorption systern. Stockpiling of materials and equipment within the SAS area is prohibited Throughout the project: a. If site conditions are found in the field to be different 1'roni those indicated on the design plan, installation of the onsite wastewater system shall stop and the designer and this office notified within 24 hours b. The brand and model of effluent filter specified on the design plan is to be utilized. The installer is to dernonstrate these have been made part ofthe onsite wastewater system c. If groundwater is present at the bottom portion of the excavation for the soil absorption system (SAS) the excavation is to be dewatered prior to the placement of any till. No fill is to be placed for the SAS during rain or stiow storms. Prior to placement of'any needed fill in the SAS, the bottom is to be scarified and relatively dry and the fill material is to be stockpiled at the edge ofthe excavation and filled in gradually d. Inspections are to be performed by the designer ofthe onsite wastewater system as needed to properly oversee and document construction. At a minimum these are the initial excavation, prior to backfilling, and during backtilling, as well as any other component of the construction which the designer wishes to examine e. Inspections are to be completed by the Town of North Andover at a rninimurn as follows: L lied bottom i i. Tank bottom iii, Final installation of components, confirmation of proper component selection., examination of construction methodology, and system operation iv. Final grade v. Sand fill will be collected from at least one location, and analyzed for compliance with specifications in 'l."itle 5 I'. No components are to be covered until a final component inspection has been completed g. No excavation for or work associated with construction of the soil absorption system is to occur from November 30 to April I After construction: h. If changes have been made to the approved plans then preparation of an as-built plan is required to be cornpleted by the designer. Ifrio changes have been rnade to the approved plans, then the approved plan shall be modified by the designer to show the distances froni a known structure to the system components. J.
