All fire hydrants shall be installed at the required 500 foot intervals throughout the proposed subdivision roadway and be placed on the plans .prior to endorsement . . 2. A Notice of Intent shall be filed with the North Andover Conservation Commission in accordance with the Wetlands Protection Act, G .' L. Ch. 131, Section 40 prior to con- struction. Any changes necessitated by an Order of Conditions may be cause for a modification of submission under Chapter 41 . 3. Improvements to Appleton Street between Station 0 + 00 and Red Oak Farm shall be submitted to the Highway Surveyor and the Planning Board for review and approval. and become part of the definitive plan prior to endorsement of the plans . 4. Drainage shall be designed on Appleton Street from Station 0 + 00 to 4 + 50 to be shown on the final definitive plan. 5 . 8" precast concrete basins shall be used throughout the subdivision. Frames , grates , and R.C. P. piping are to be in accordance with the Highway Department specifications; with brick used for drainage purposes to be clay material. . 6. 18" overlap of gravel shall be required, and three feet ( V ) of overlap where berm is required to be , determined in the field. 7. Sloped granite curbing shall be installed where grades exceed 6 percent ( 6%) . 8. At the intersection of Road D and Bear Hill Road, two ( 2) catch basins shall be installed. 9. The drainage ditch that runs parallel to the New England bower Co. easement shall be cleaned to accomodate any additional off-site drainage from the subdivision . 10. 28 feet of binder coat shall be applied, with a finish coat of 26 feet in width. 11 . Any drainage from cellars or lot development shall be incorporated into the planned drainage system. Plans to be submitted to the Highway Surveyor for review and approval . 3 Any appeal shall be filed within (20)- days after the date of filing of this Notice in the Office of the Town Clerk. 12. Street drainage not shown on the ;rdefjLpitive plan will not be added during construction unless required by the Highway Surveyor and will be detailed on the "As Built" plan. 13 . No town services shall be provided to the subdivision until the binder coat of pavement and , temporary turnaround is provided. 14. The subdivision shall be completed in phases; separate completion dates shall be imposed for each phase as they are initiated. Paved. temporary turnarounds shall be provided at the end of each .phase until the-,roads arq connected to enable vehicular access .for - the safety, health and well being of the town . A finalized sedimentation - and erosion control program shall become part of said subdivision and: a ) will be documented in written form in accordance with the construction schedule; b) said plan shall be reviewed and approved by the Planning Board; c) a performance bond shall be retained to maintain com- pliance; and d ) prior to any construction in each phase, and prior to lot release, all detention pond areas shall be constructed and functioning properly. Note: Items a and b shall be submitted prior to endorsement of plans . 16 . All street trees shall conform to the Tree Warden ' s specifi- cations . 17 . The applicant/owner/developer shall be aware of the fact that approval of this subdivision under the Planning Board' s Rules and Regulations will not relieve said parties of the responsibility to other property owners upon whose land is adversely affected by indirect or direct development storm water discharge. Notation is to be placed on the definitive plans to be recorded . 18. The lot runoff water from Lot 16 shall be collected and, via a cross culvert in Appleton Street , be deposited directly above ground onto Lot 40 which shall ultimately flow into the detention pond on Lot 41 . 19. The lot runoff water from Lots 1 and 2 and from Lots 18 and 19 AnY appeal shall be file 1 within (20) {days after the date of filing of this Notice In the Office of the Town Clerk. ..�-- shall be collected and via cross culverts to be deposited directly above ground onto Lot 36 alld' ltio4tely flow to the proposed detention pond between Lots 35- and 36 . 20. To control rainfall runoff and to transport successfully the water to preselected locations , it is necessary to install "open channels" along the sideline of the road at several locations . This open channel shall be designed to such a crossection as to conduct the greatest runoff expected at any one location. A typical crossection shall be prepared and approved . This crossection shall be similarly constructed in the following locations; a ) along the southerly sideline of Lots 40 and 41 near the streetline for the purpose of conducting lot runoff water to the detention pond on Lot 41 . b) along the southerly sideline of Lot 25 near the street- line for the purpose of conducting lot runoff water to the detention pond on Lots 22 and 23. c) along the southerly side of Lot 16 near the streetline to conduct lot runoff water to the crossculvert (see item 18) . d) along the westerly sideline of Lots 7, 8, 9, and 10 near the streetline for the purpose of conducting lot runoff water to the wetlands and small - brook on Lot 10. e) along the easterly sideline of Lot 32 near the streetline for the purpose of conducting lot runoff water to the detention basin on Lots 30 and 3l . 21 . A statement from the project engineer that no subsurface drain pipe has been designed to transport water at a velocity greater than 10 feet per second shall be required . 22. Driveways shall be installed from the new road to gain access to all lots . In some cases , these driveways shall be installed over drainage "open channels" . The project engineer shall provide a detail on the subdivision plans which shows how a culvert would be installed in the open channel so that the driveway construction would not obstruct the natural flow of water . 23. Plans shall be revised to show "Storm Retention Drainage Easements" in metes and bounds , which shall contain all detention ponds . 24. The Simon lot as shown shall be changed to the "Simon Parcel" . A note shall be added to the recorded plan which states that "building shall be prohibited on this parcel until such time as said parcel conforms to the Zoning By Law. A separate plan shall be prepared which shows what land is to be conveyed from the Bear Hill Develapmpnt Tr-ust to the Pines Subdivision to create Lots 2, 3, 4 5; - 6 7 8, 9, and 22. This plan is to be simultaneously signed when the Pines Subdivision plans are signed . Said subdivision line to also be placed on the Pines Subdivision . 26. Sewer easements shall be provid'sd: f or' between Lots 30 and 31 from the streetl.,ine to ,the rear property line and along the southerly sideline o'f• `dot 16'•''f.rom the, streetline to the rear of the property, ; subject to Board of Public Works recommenda- tions . 27 . All easements to run continuous through' to abutting land in accordance with the Highway Surveyor ' s recommendations`: All easements to be documented in recordable form, running to the Town of North Andover , subject to approval prior to endorsement of plans . All easements to be properly documented by metes and bounds in their entirety. 28. No 'construction shall proceed on Appleton Street before at least the binder is placed on the existing section of Appleton Street from Dale Street to Red Oak Farm. 29. Prior to endorsement of plans , the applicant shall provide approved street names and draft them on the plans . 30. No binder shall be placed without a certification from the applicant ' s engineer listing all utilities have been installed in accordance with the Rules and Regulations . 31 . All sewer lines within the development shall be laid in the middle of the road layout .. Sewer installation in the shoulders of town roads will not be allowed. 32 . The water main on Appelton Street shall be 12 inch class 52 ductile iron cement lined water pipe installed ten ( 10) feet left of the center line of the road and with five ( 5) feet of cover below the finish grade from Station 0 + 0 to Station 16 + 20. 33. Twelve inch Mueller or equal gate valves (open left) shall be provided at Station 0 -- 10, 4 + 60, 4 + 80 and 12 + 70. Eddy brea%flange hydrants to North Andover specifications are to be installed at Station 0 - 50, 4 + 3Q, 9 + 10, 12 + 30 and 16 + 20. 34. The water main on Road D shall be eight inch class 52 ductile iron cement lined water pipe installed ten ( 10) feet right of the centerline of the road and with five ( 5) feet of cover below the finish grade . Eight inch Mueller or equal gate valves (open left) shall be provided .at the existing 8" pipe on Bear Hill Road and at Station 5 + 40. Any appeal shall be filed within (20) days after the date of filing'of this Notice In the Office of. the Town Clerk. 35 . The water main on Road E shal ,be eight incl) class 52 ductile iron cement lined water pipe Ths"tbXldl ten ( 10) feet left of the centerline of the road and with five ( 5) feet of cover below the finish grade . Fight inch Mueller or equal gate valves shall be provided at Station 0 + 20, 10 + 80, 21 + 70, 21 + 90, 30 + 0 and 39 + 30. Eddy breakflange hydrants are to be installed at Station 4 + 90, 10 + 70, 15 + 0, 25 + 0, 29 + 70 and 34 + 50. 36. All hydrant branches shall be provided with six inch Mueller or equal gate valves (open left) . 37 . All gate valves shall comply with AWWA specifications L500 52T and will be supplied with a 100 lb. sliding type gate box marked "WATER" . 38 . All water mains shall pass leakage and pressure tests per- formed in accordance with Section 13 of the AWWA standard C600 and shall be chlorinated in accordance with AWWA C601 "Standard Procedure for Disinfecting Water Mains" . 39 . The proposed. sewer . on Appleton Street shall be 12 inch vitrified clay pipe installed in the centerline of the road and shall be extended to Station 10 + 75 to provide service to Lot 16 and Lot 40. 40. The proposed sewer mains on Road D and Road E shall be 8 inch vitrified clay pipe installed in the centerline of the road . 41 . Sewer manholes shall be relocated or manholes should be added on Road E on curves at Stations 9 + 31 to 11 + 09 and 32 + 07 to 33 + 66 in order to maintain better alignment and eliminate crossing of utilities . 42 . A sewer manhole and an intersection with the stationing of the Bear Hill off--site sewer plan shall be shown on the Appleton Street profile at Station 0 + 00. 43 . All sewer mains shall have seven ( 7) feet of cover below the finish grade wherever feasa,b.ly:,,possible ,, 44. The sewer manho-le', at Statioh�,' ' + :00 of Appleton Street shall be moved in order to avoid conflict with the twelve inch R.C . P. crossing at Station 3 + 017 . 45. The two sewage pump stations on Road E -and their accompanying force mains shall be deleted and replaced with one sewage pump station Located in the southerly corner of Lot 28 adjacent to land of the New England Power Company. and Anna Tymakievicz . The accompanying :force main shall eject into the summit manhole at Station 10 + 75 Appleton Street . The gravity sewers shall be re-designed to flow to the new pump station location. Any appeal shall be filed ' within (20) d9ys after the date of filing of this Notice in the Office of the Town Clerk. 46. Plan and profile shall be ,.providgd for all re--designed sewers and all cross-country sewers . 47. Permanent utility easements of twenty ( 20) foot minimum width to be deeded to the Town of North Andover shall be provided parallel to the centerline of all cross-country sewers . 48. Temporary construction and maintenance easements of forty (40) foot minimum width shall be provided 'where the depth of the sewer would require large excavations . 49. A forty (40) foot wide easement and Fight-of-way shall be provided for access between Appleton Street and the pump station location along the southerly property lines of Lot 26, 27, and 28. A sixteen (16) foot wide gravel roadway shall be provided within the right-of-way for vehicle access . 50. A twenty ( 20) foot wide easement shall be provided along the southwest properly line of Lots' 28 through 33 adjacent to land of the New England Power Company from the property line of Anna Tymvakievi.cz to the property line of Neil and Margaret Patnaude. 51 . A twenty (20) foot wide easement shall be obtained for the Town of North Andover on land of Anna Tymvakievicz along the southwest property Line adjacent to land of the New England Power Company and shall extend from the pump station locatin to the So. Bradford Street right-of-way. 52 . Any sewer designed at a depth of greater than sixteen ( 16) feet shall be constructed 'of ductile iron cement lined pipe . 53 . All sewer at depths' of less ' than sixteen ( 16) feet shall be unglazed extra 'strength •vitrified clay pipe . 54 . Precast manholes shall, conform � to ASTM designation C478 and shall meet the following additional requirements : the wall thickness shall not be less than five ( 5) inches ; section shall have tongue and groove joints with and approved bitumastic gasket ; pipe connections shall be of the rubber boot with stainless steel band or approved equal; manhole frames and covers shall be cast iron lettered "SEWER" and shall be eight ( 8) inches high. 55 . All sewers must pass low pressure air tests with a maximum allowable leakage rate of 0. 0015 cubic feet per minute per square foot of internal pipe surface .
