Repeat COS OBSERVATION: Observed hand sink adjacent to point of sale without drying
provisions. PIC to ensure all hand sinks are supplied with drying
provisions. Employee provided paper towels during the inspection.

Repeat OBSERVATION: Observed carton of raw shelled eggs stored directly adjacent
to cooling rice and deli meats in walk in refrigerator. PIC to ensure raw
animal foods are properly stored in equipment to prevent cross
contamination.

OBSERVATION: Observed labeled spray bottle holding chlorine sanitizer
measure well above 200 ppm via inspector' s test strips. PIC to routinely
monitor sanitizer concentrations ensuring chlorine sanitizer is
maintained at 50-200 ppm.

OBSERVATION: Observed deli slicer in rear of store with accumulation of
food debris on the blade. PIC to ensure deli slicer is cleaned and
sanitized after each use.

Cleanliness of FCS and Non-FCS
Repeat OBSERVATION: Observed meatballs at 131°F and marinara sauce at 1130F both
in table top warmer. PIC to determine source of low temperatures. PIC to
rapidly reheat foods and ensure TCS foods are maintained at 135OF or
above in hot holding.

OBSERVATION: Observed diced tomatoes at 43°F; sliced tomatoes at 45°F;
sliced turkey at 48°F; sliced ham at 47°F all in top rail of sandwich in
line refrigerator. PIC to determine source of elevated temperatures. PIC
to rapidly cool foods and routinely monitor temperatures with food
thermometer ensuring TCS foods are maintained at 41°F or below.

Correct On Site OBSERVATION: Observed raw steak tips at 63-69OF and raw chicken at 65°F in
drawer refrigerator below grill. Ambient thermometer appears to be
displaying "0" . It appears refrigerator in not working properly. PIC
states intent to call repair service. PIC to provide health office with
status or repair slip within 24 hours. PIC states foods were recently
prepared. Upon reviewing cold holding requirements, PIC relocated
elevated foods to the walk in refrigerator. PIC to ensure TCS foods are
maintained at 41°F or below.

Correct On Site OBSERVATION: Observed cooked rice in cabinet of sandwich in line
refrigerator at 93°F, PIC identified rice as cooling. PIC to place
cooling foods in rapid cooling equipment ensuring TCS foods cool within
time and temperature parameters. PIC relocated rice to walk in
refrigerator.

PIC to supply food thermometer and ensure it is readily accessible for
monitoring temperatures of food.

OBSERVATION: Observed digital ambient thermometer of drawer refrigerators
appears to be reading "0" . PIC unable to provide other ambient
thermometer inside unit. PIC to ensure all cold holding and hot holding
equipment are supplied with easily readable and accurate ambient
thermometers.

OBSERVATION: Observed left knob of three compartment sink consistently
leaking water. PIC to ensure plumbing is maintained in good repair using
approved methods and materials.

OBSERVATION: Observed walls and floors throughout kitchen in poor state of
cleanliness. PIC to ensure physical facilities are cleaned as often as
necessary to keep them clean.
