The undersigned, being the applicant as defined under Chapter 44 Section
81—L, for approval of a proposed subdivision shown on a plan entitled

The undersigned hereby applies for the approval of said DEFINITIVE plan
by the Hoard, and in furtherance thereof hereby agrees to abide by the Boards
Rules and-Regulations. The undersigned hereby further covenants and agrees
with the Town of North Andover, upon approval of said DEFINITIVE plan by the
Board:

1. To install utilities in accordance with the rules and regulations of the
Planning Board, the Public Works Departmentt the highway Surveyor, the
Board of Health, and all general as well as zoning bylaws of said Town,
as are applicable to the installation of utilities within the limits of
ways and streets;
2. To complete and construct the streets or ways and other improvements shown
thereon in accordance with Sections Iv and V of the Rules and Regulations
of the Planning Board and the approved DEFINITIVE plan, profiles and cross
sections of the same. Said plan, profiles, cross sections and construction
specifications are specificall~gt by reference, incorporated herein and made
a part of this application. This application and the covenants and agree—
ments herein shell be binding upon all heirs, executorst administrators$
successors, grantees of the whole or part of said land, and assigns of the
undersigned; and

Plan does not affect any lots or ri ppurtenant thereto in such
subdivision which lot was conveyed or mortgaged in good faith and for
valuable consideration subsequent to the approval of the subdivision plan.

Notice to APPLICANT/TOWN CLIIIK and Certification of Action of Planning Board
on Definitive Subdivision Plan entitled: Wildwood Circle'
1.

The North Andover Planning Board has voted upon its own motion to modify its (.
approval of said plan (pursuant to M.G.L. Ch. 41, Sec. 81—W), subject to the I'
following conditions:
1. Acoept the bond amount of $23,000.
.2. The Planning Board of North Andover hereby modifies its approval of the
Wildwood Subdivision as a condition of its retaining the status of an
approved plan in accordanoe with M.G.L. Section 81--W with the following
conditions:
A slope easement drafted. by Mr. Paul. A,heaariz s attorney., which is �
agreeable to and signed by Mr. Richard Umlah shall be submitted to
the North Andover Planning Board by September 8, 1980.
b) All grading and slope work performed on Mr. Umlah's property shall
be supervised by the Highway Surveyor, Mr. William Cyr, and shall 'be
completed no later than November. 15, 1980.
o} Binder coat shall be completed no later than November 15, 1980.
d All improvements in said subdivision shall be completed no lator`' han
October 1, 1981.

In the event that no appeal shall have -been-taken from said approval within twenty
days from this date, the North Andover Planning Board will forthwith:
1. Record a certified copy of the vote of the Planning Board and a copy of the
plan as originally approved;

2. Endorse the action of the Planning Board on the plan originally approved as
recorded referring to 'such vote and where it is recorded;
3. Index the vote of the Planning Board in the grantor index under the names of
the owners of record of the land affected.
