Dear Mr. LaGrasse,
Civil Design Consultants, Inc. (CDCI) is pleased to provide the attached revised plan in response to
comments received from Daniel Ottenheimer, PE, of Mill River Consulting, dated November 22, 2023.
The following revisions have been made to the plan set:
1. The tax map and lot number have been added to the plan.
2. The address of the owner is now provided under the notes on sheet C-1.
3. A locus is now provided on sheet C-1.
4. General Note 10 has been added to sheet D-1 to indicate the requirement for magnetic marking
tape.
5. Sheet C-1 indicates the leaching field to be 31.3'(20'min) from the cellar wall.
6. No drain pipes intercepting the water table are proposed, except for the foundation drain, which
is a minimum of 20'from the leaching field as required by 310 CMR 15.211.
7. General Note 11 has been added to sheet D-1 to specify the requested note.
8. The Septic Tank Detail on sheet D-1 has been revised to include the requested dimensions.
9. The Septic Tank Detail on sheet D-1 has been revised to clarify the outlet baffle.
10. The system profile on sheet C-1 has been revised to specify a manhole to within 6" of finished
grade.
11. The Typical Septic Tank and Distribution Box Detail on sheet D-1 has been revised to include
note 11 specifying the requirement for a watertight tank and distribution box.
12. The Typical Septic Tank and Distribution Box Detail on sheet D-1 has been revised to include
note 12 specifying the requirement for all distribution box outlets to be at the same elevation.
13. The Leaching Facility Cross-Section Detail on sheet D-1 has been revised to include a note
specifying the requested orifice size.
14. Sheet D-1 has been revised to include an Inspection Port Detail.
15. Sheet C-1 has been revised to indicate connected and vented trenches with a corresponding
detail on sheet D-1.
16. Sheet D-1:
a. All note references have been revised to indicate the Town of North Andover.
b. The wording of General Note 6 has been revised as requested.
c. The reference to a well has been removed from the General Construction Sequence.
d. The Landscaped Area Cross-Section has been revised to be in compliance with Title 5.
e. Sheet C-1 has been revised to indicated Sections A-A and B-B.
f. The Monolothic Septic Tank Section View has been revised to more clearly illustrate the
2-compartment tank and the piping connecting the two compartments.
g. The notation indicating an inspection plug has been removed.
h. A callout referencing note 6 has been added to the Monolithic Septic Tank Section View
to illustrate the top of the tank to be sealed.
In addition to the revisions listed above, the proposed building foundation has been revised. To
accommodate this revision, the proposed leaching field has been mirrored, and the septic tank is now
located on the opposite side of the building. The elevations of the septic tank inlet/outlet, and invert at
the cellar wall as well as pipe lengths have been updated accordingly. The parcel has also been assigned
an address. All references to'0 Boston Street'have been revised to refer to'531 Boston Street'.
