compacted firm base
Cleanouts per plan
Bottom of tank hole has 6” stone base
Tank size and loading per plan
Monolithic tank construction
If monolithic, water tightness of tank has been
achieved by visual testing
If 2-piece, water tightness of tank has been
achieved by water / vacuum test (circle one)
Inlet tee installed, centered under access port
Outlet tee installed, centered under access port
Gas baffle installed
If effluent filter, riser to finished grade over filter
Riser with cover to within 6” of finish grade
installed over one access port
Manhole and riser minimum ___ inch diameter
Pipe penetrations appear watertight using
rubber boots / hydraulic cement (circle one)
Comments: 7/2/24 Pipe into tank needs hydraulic cement, inlet tee not beneath
