to obtain current food permit and display in public view adjacent to the
certified food protection manager (CFPM) certificate.
**RE-INSPECTION** PIC TO OBTAIN CURRENT FOOD PERMIT AND DISPLAY IN
COMPLIANCE.
Verified 09/17/24 OBSERVATION: Observed hand sink adjacent to point of sale with ice and
spoon stored in basin. PIC to ensure hand sinks are maintained accessible
at all times and are used for handwashing only.
**RE-INSPECTION** OBSERVED ALL HAND SINKS MAINTAINED ACCESSIBLE.

Verified 09/17/24 OBSERVATION: Observed packages of "Fruit by the Foot" bearing label "not
labeled for individual sale" . PIC to discontinue offering all packages
foods for retail sale bearing that label. PIC to ensure all packaged
foods for reail sale include the following components:
(1) The common name of the FOOD, or absent a common name, an adequately
descriptive identity statement; (2) If made from two or more ingredients,
a list of ingredients in descending order of predominance by weight,
in-cluding a declaration of artificial color or flavor and chemical
preservatives, if contained in the FOOD; (3) An accurate declaration of
the quantity of contents; (4) The name and place of business of the
manufacturer, packer, or distributor;
**RE-INSPECTION** OBSERVED PACKAGED FOODS FOR RETAIL SALE LABELED IN
COMPLIANCE.

OBSERVATION: Observed floor adjacent to high temperature dish machine with
accumulation of water. PIC states dish machine is leaking and a repair
service has been called prior to inspection. PIC to ensure all plumbing
systems are maintained in good repair using approved methods and
materials .
**RE-INSPECTION** OBSERVED FLOOR ADJACENT TO DISH MACHINE WITH SLIGHT
ACCUMULATION OF WATER, PIC STATES DISH MACHINE IS WAITING TO BE REPAIRED.
PIC TO REPAIR USING APPROVED METHODS AND MATERIALS. WILL VERIFY AT NEXT
ROUTINE INSPECTION.
5-205.15 REGULATION: Item 51 Plumbing installed; proper backflow devices -- Water
System Good Repair
In Compliance Observed high temperature dish machine achieve a utensil
surface temperature of 180OF via PIC's sensor stickers, wiping cloths
stored in QAC sanitizer measure 400 ppm via inspector's test strips.
Observed in-use utensil stored in water at 1420F; milk in top rail of in
line refrigerator at 38°F.
