Form attached for your files. The contractor opted to install an effluent filter in addition to the gas baffle inside the septic tank outlet. As a result they need to have a manhole to finished grade over the effluent filter. The contractor was aware of this but had not yet installed the riser with manhole so you might wish to double check that when doing the final grade inspection. Also, somehow the owner should be made aware that there is an effluent filter with the need for annual maintenance which has been installed by their contractor. I could create a quick letter to them, or you could, if desired. Unfortunately,the plan cannot be approved until the following items are corrected. The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by this design follows each item where applicable. 1. Two Local Upgrade Approvals are being requested as noted on the design plan while only one is noted on Form 9A—Request for Local Upgrade Approval. Clarify this discrepancy 2. The design plan contains conflicting information regarding the design flow, listed as 330 gallons per day in some areas and 440 gallons per day in other areas. The submitted Form 9A—Request for Local Upgrade Approval is similarly conflicted. Clarify this discrepancy 3. Provide additional topographic information to the east of the proposed soil absorption system to confirm the breakout standards at 310 CMR 15.211 are met Please feel free to contact the office or Mill River Consulting at 978-282-0014 with any questions you may have.
