The North Andover Selectmen references this plan in a petition dated 1929 to abandon portions of Chestnut St., Willow St., Rea St., and Appelton St., Stourth Bradford St., and Liberty St. The plan clearly shows Stiles St. to be a Public Way as of the date of this plan. I have researched the town meeting warrants subsequent to this date with the assistance of the Town Clerk, Ms. Joyce Bradshaw, and found no articles petitioning the County Commissioners to "adjudicate that said way shall thereafter be a private way and that the town shall no longer be bound to keep the same in repair". The basis for this revision is comments received from the board at the public hearing held Tuesday 9/9/03. VHB would not recommend that the Board consider a waiver of the Stormwater Management requiremcnts for the roadway construction. II. GENERAL COMMENTS: 1. VHB recommends two separate plans sets be created;the first set showing the proposed driveway and the second showing the proposed roadway may have been a clearer way to indicate initial and future construction. 2. The Applicant should label the centerline geometry on plans as dictated in section 5.2.5.4 of the Subdivision Rules and Regulations. VHB does not believe a waiver is warranted. 3. The Applicant should provide an existing conditions plan. 4. Subdivision Street Typical Section: The hydrant is shown on left side of typical section. The plans show the hydrant on the right side. The Applicant's Engineer should revise the plans. 5. VHB recommends providing more information for the cape cod berm,such as dimensions and a construction detail. 6. Granite curb inlets should be proposed at all proposed catch basins. VHB recommends that the Applicant provide a detail showing how the cape cod berm will transition to and from the granite curb inlet. 7. VHB assumes that the object shown to the left of Station 3+20+is a proposed septic system. The system appears to be located within the new right of way. The proposed system should be moved so that it is completely located on private property. The applicant responded to this on 919 and stated that the system will be on private property but that the grading extends into the easement area 8. A stop line and possible crosswalk should be provided and the intersection of Stiles Street and Gray,Street. 9. VHB recommends the conservation commission and any other appropriate authority review the plans. This review is conducted in accordance with the Town of North Andover Zoning Bylaw,the North Andover Rules and Regulations Governing the Subdivision of Land,Massachusetts Department of Environmental Protection(DEP)Stormwater Management Policy and standard engineering practice. VHB has received the following drawings and documents for review: • Site Plans(3 sheets)dated August 8,2003 • Descriptive narrative(10 sheets)faxed to VHB on September 17,2003. VHB understands that the Applicant intends to initially construct a proposed driveway. It is not clear on the plan that certain waivers apply only to the proposed driveway. VHB further understands that future development may occur and that at that time the proposed roadway will be constructed. VHB recommends that the waivers that are listed on the plan be clearly divided for proposed driveway and proposed roadway. The Applicant is requesting numerous waivers from the requirements of a Definitive Subdivision Plan. As a result, much of the technical information that VHB typically reviews has not been included. The following comments note non-conformance with specific sections,constructability issues and questions/comments on the proposed design. I. RULES AND REGULATIONS GOVERNING THE SUBDIVISION OF LAND Section 5—Definitive Subdivision Plan 5.2.5.1 Cover Sheet The Applicant is requesting a waiver from this section. 5.2.5.2 Lot Layout Plan The plan provided is not clear. It is difficult to determine what is proposed and what the existing features are. VHB suggest that the Applicant clearly label existing and proposed right-of-way(ROW)lines. The Applicant should refer to Section 6.8.9 Intersections and General Comment No. 8. 5.2.5.5 Typical Sections Details and Notes The typical section does not show the location of a proposed sewer. Also the electric, cable,telephone utilities are not shown. Considering the possibility of future development,VHB recommends that these utilities and a proposed sewer be considered for this Definitive Subdivision plan. The width of the proposed cape cad berm is not shown. This information should be shown on the typical section. Where are the utility poles to be relocated? The Applicant is requesting a waiver from this section based on Section 5.2.5.4(2). 6.8.9 Intersections This section in the North Andover Subdivision Rules and Regulations states that"street intersections shal I be designed with a flat grade(not greater than 3%)for a distance of at least 100 feet." Section 6.11—Sidewalks The Applicant is proposing a 4'bituminous sidewalk instead of the required 5' sidewalk. The Applicant is requesting a waiver for this section. Is there an existing sidewalk on Gray Street? Wheelchair ramps may be required if there is sidewalk on Gray Street. Also,the Applicant must meet the requirements of the Massachusetts Architectural Access Board(MAAB). Section 6.12 Monuments It does not appear that any monuments are proposed for this project. The Applicant should review this section and revise the plans as described in this section. VHB recommends that monuments be constructed. Section 6.13 Utilities—Basic Requirements It is unclear from the plans if any utilities other than water and drainage will be provided. The Applicant should clarify and revise the plans as required. Consideration should be made for future development. Section 6.14—Storm Drainage Partial information was provided for the proposed the drainage system. See General Comments below. At a minimum,the Applicant should provide pipe sizing calculations, flow calculations,identify where the system outlets and identify the need for drainage easements. As it is unclear how many lots will be developed in the future,the Applicant may need to meet the requirements of the Stormwater Management Guidelines at that time. Section 6.15 Water Facilities Where the proposed waterline connects to the existing waterline on Gray Street,VHB recommends installing 3 water gates,one on the proposed waterline and one on each side of the waterline on Gray Street. Section 6.16 Sewer It does not appear that a sewer system is proposed for this project. is there an existing sewer main on Gray Street? The North Andover Subdivision Rules and Regulations require the connection of all lots to the sewerage system if the option is available. The Applicant should consider a sewer design for the potential future development. Section 6.17 Electric and Communications Lines The Subdivision Rules and Regulations state that all electrical and communication lines shall be underground. The Applicant has indicated that all existing lines are overhead. The Applicant is requesting a waiver from this section. Again, it is unclear how many lots may be developed in the future. The Applicant should consider accommodation of future development. Section 6.18 Trees and Other Plantings The Applicant is requesting a waiver from this section. Section 6.20 Street Signs The plans do not seem to indicate that any signage will be provided for this project. VHB recommends placing a stop sign and the end of Stiles Street. VHB also recommends providing a detail for the stop sign and sign post. Section 6.21 Street Lighting The plans do not indicate any street lighting for this project. VHB could not review this section for compliance. Appendix l—Streets Construction Specifications A—Roadways (5)Gravel Sub base: The Applicant is requesting a waiver to provide 8 inches of gravel sub-base instead of the required 12 inches. It is not clear why the Applicant cannot meet this requirement. Appendix V—Rules and Regulations Governing Storm Water Management The Applicant is requesting a waiver from this requirement. if the Applicant's intent is to request this waiver for the proposed driveway construction,VHB would suggest that the Board consider the waiver request. DRAINAGE COMMENTS: 1. Drainage calculations(flow calculations)were provided on September 17,2003. These calculations were not provided with the original submission. Also provided were three plan sketches showing contour information. As these sketches were faxed and are at a small scale,VHB could not verify the drainage area delineations. Additional information should be provided in order to verify the adequacy of the proposed drainage design. 2. Drainage invert information was not provided. VHB recommends that inverts be provided by the Applicant. 3. Where does the proposed closed system outlet'? Is a drainage easement required for this outlet? It is not clear on the plan that certain waivers apply only to the proposed driveway. VHB further understands that future development may occur, and at that time,the proposed roadway will be constructed. VHB recommends that the waivers that are listed on the plan be clearly divided for proposed driveway and proposed roadway. I. INITIAL COMMENTS: The Applicant is requesting waivers from certain requirements of a Definitive Subdivision Plan. As a result,much of the technical information that VHB typically reviews has not been included. VHB would not typically recommend approval of these waiver requests. VHB would recommend that the Planning Board consider the following two options for Sections 5.3,5.4,6.14,Appendix V and Drainage Comment#3: • Consider requiring submission of technical information prior to Planning Board Approval so that an adequate review can occur or • Specifically condition the approval such that the submission of technical information must occur prior to construction. This information should be reviewed by the Town and its review consultant prior to construction. RULES AND REGULATIONS GOVERNING THE SUBDIVISION OF LAND Section 5—Definitive Subdivision Plan 5.2.5.1 Cover Sheet The Applicant is requesting a waiver from this section. The Applicant has indicated that the waiver has been approved by the North Andover Planning Board on 9/9/03. 5.2.5.2 Lot Layout Plan The plan provided is not clear. It is difficult to determine what is proposed and what the existing features are. VHB suggest that the Applicant clearly label existing and proposed right-of-way(ROW)lines. There are several locations along the north side of Stiles Street where the distance between the proposed roadway and the existing ROW line is approximately 1 to 3 feet. VHB would recommend that a minimum distance of 5 feet be provided to allow for utilities and future sidewalk, The Applicant has indicated that no fixture walls or utilities are proposed for on that side and a waiver has been approved by the North Andover Planning Board on 9/9/03. 5.2.5.3 Grading,Drainage and Utilities Plan A Grading,Drainage and Utility plan was not provided. VHB could not review the plan for compliance. The Applicant has indicated that a waiver for this plan was granted on 9/9/03. 5.2.5.4 Street Plan and Profile Street Plan: A Street plan was not provided with this submission. VHB could not review the plan for compliance. Although a plan is now provide,much of the information listed in this section was not provided. Profile: The Applicant should provide the sight distance for each proposed vertical curve. The Applicant should refer to Section 6.8.9 intersections and General Comment No. 8. The vertical curves have been revised so all but the first curve meet the minimum K value for 30 mph roadways. The slope from Gray Street to Station . 1+43.50 still exceeds the minimum slope required in Section 6.8.9 Intersections. The Applicant has indicated that a waiver for this plan was granted on 9/9/03. 5.2.5.5 Typical Sections Details and Notes Tire typical section does not show the location of a proposed sewer. Also the electric,cable,telephone utilities are not shown. Considering the possibility of future development,VHB recommends that these utilities and a proposed sewer be considered for this Definitive Subdivision plan-'The Applicant has provided two typical roadway sections;one with sidewalk and one without sidewalk. The two typical sections do not agree with each other with respect to the locations of the gasline,electric,cable and telephone lines. The Applicant has indicated that no utilities will be constructed on,the right side of the roadway due to the limited right of way. The Applicant should clarify the location of all proposed utilities and revise the typical sections appropriately. The width of the proposed cape cad berm is not shown. This information should be shown oil the typical section. This comment has been addressed. Where are the utility poles to be relocated? The Applicant should consider and provide clarification. This comment has been addressed. 5.2.6 Test Data Test pits and percolation tests were taken. Only the test pit information is shown on the plans. Please note that the information for Test Pit#4 appears to be erroneous. The location of the test pits and percolation test should be shown on the plans. 5.3 Stormwater Management Re ort A Stormwater Management Report was not submitted as part of this package. The Applicant is requesting a waiver for this section. As it is unclear how many future lots may be developed,VHB suggests that the Applicant agree to prepare a complete stormwater management report at the time of the future development. VHB recommends the Planning Board not approve this waiver but snake the preparation,review and approval of the Stormwater Management Report a condition of the approval. See VHB's initial comments. 5.4 Environmental and Community impact Statement An Environm m ental and Community Impact Statement was not provided. The Applicant is requesting a waiver for this section. As it is unclear how many future lots may be developed,VHB suggests that the Applicant agree to prepare a complete Environmental and Community Impact Statement at the time of the future development. VHB recommends the Planning Board not approve this waiver but mare the preparation,review and approval of the Environmental and Community Impact Statement a condition of the approval. See VHB's initial comments. Section 6.8—Streets--Design Standards 6.8.1 Table IA The Applicant is requesting a waiver for the minimum right of way, the minimum pavement width, the minimum centerline curve radius,and the minimum tangent length between reverse curves requirements. The Applicant has indicated that a waiver for this plan was granted on 9/9/03. 6 8.4 Street Geomcta The plans do not provide the proposed street geometry. The Applicant is requesting a waiver frown this section based on Section 5.2.5.4(2). Although the Applicant has indicated that street geometry has been provided,it is not apparent on the plans. The bearing and distance for all tangents and radius and curve length should be provided for curves of the baseline. The curbline geometry should also be shown on the plans,including radius and length. VHB is concerned that the proposed geometry does not meet the minimum design requirements for a 30 mph roadway. 6.8.9 Intersections This section in the North Andover Subdivision Rules and Regulations states that"street intersections shall be designed with a flat grade(not greater than 3%) for a distance of at least 100 feet." The profile seems to indicate that the profile grade of Stiles Street coming into Gray Street is approximately 7.8%. VHB recommends that the Applicant revise the profile to meet the requirements of this section. The Applicant has revised the profile but the slope from Gray Street to Station 1+43,50 still exceeds the minimum slope required this section. The Applicant has indicated that a waiver for this plan was granted on 9/9/03. Section 6.9—Curbing Tile Applicant is proposing cape cod berm along the entire roadway. The Applicant is requesting a waiver from constructing granite edging and granite curbing. The Applicant has changed the proposed edge treatment to granite edging. This comnnent has been addressed. Section 6.11--Sidewalks The Applicant is proposing a 4' bituminous sidewalk instead of the required 5' sidewalk. Tine Applicant is requesting a waiver for this section. Is there an existing sidewalk on Gray Street? Wheelchair ramps may be required if there is sidewalk on Gray Street. Also,the Applicant must meet the requirements of the Massachusetts Architectural Access Board (MAAB). The Applicant has indicated that there are no existing sidewalks on Gray Street and the Planning Board has granted a waiver on 9/9/03 for the proposed 4'sidewalk. 3 CAWINDOWSWEMMStiles Street-review 2.doe Section 6.12 Monuments It does not appear that any monuments are proposed for this project. The Applicant should review this section and revise the plans as described in this section. VHB recommends that monuments be constructed. The plans now show that two bounds are proposed at the lot corners. Monuments should be installed at all street intersections,at all points of change in direction or at curvature of streets,at property corners of all new lots, etc as shown in the Subdivision Rules and Regulations. The Applicant should revise the plans or request a waiver. Section 6.13 Utilities—Basic Requirements It is unclear from the plans if any utilities other than water and drainage will be provided. The Applicant should clarify and revise the plans as required. Consideration should be made for future development. See comment for Section 5.2.5.5 Typical Sections,Details and Notes. Section 6.14—Storm Drainage Partial information was provided for the proposed the drainage system. See General Comments below. At a minimum,the Applicant should provide pipe sizing calculations, flow calculations, identify where the system outlets and identify the need for drainage easements. As it is unclear how many lots will he developed in the future, the Applicant may need to meet the requirements of the Stormwater Management Guidelines at that time. VHB recommends the Planning Board make the preparation,review and approval of the drainage calculations as described above(Stormwater Management Report)a condition of the approval. See VHB's initial comments. Section 6.15 Water Facilities Where the proposed waterline connects to the existing waterline on Gray Street,VHB recommends installing 3 water gates,one on the proposed waterline and one on each side of the waterline on Gray Street. This comment has been addressed. Section 6.16 Sewer It does not appear that a sewer system is proposed for this project. Is there an existing sewer main on Gray Street? The North Andover Subdivision Rules and Regulations require the connection of all lots to the sewerage system if the option is available. The Applicant should consider a sewer design for the potential future development. The Applicant has indicated that there is no sewer available to this lot. This comment has been addressed. Section 6.17 Electric and Communications Lines The Subdivision Rules and Regulations state that all electrical and communication lines shall be underground. The Applicant has indicated that all existing lines are overhead. The Applicant is requesting a waiver from this section, Again, it is unclear how many lots may be developed in the future. The Applicant should consider accommodation of future development. The Applicant has changed the design so that all utilities are underground. This continent has been addressed. Section 6.18 Trees and Other Plantings The Applicant is requesting a waiver from this section. The Applicant is no longer requesting a waiver from this section. Although landscaping is now shown on the plans,the species and tree size are not shown. This section could not be reviewed for compliance. Section 6.20 Street Signs The plans do not seem to indicate that any signage will be provided for this project. VHB recommends placing a stop sign and the end of Stiles Street. VHB also recommends providing a detail for the stop sign and sign post. VHB could not review this section for compliance. A note has been added to the plans stating that lighting shall be located as directed by the North Andover Planning Department. VHB is not able to review this section for compliance. Appendix I—Streets Construction Specifications A—Roadways (5)Gravel Sub base. The Applicant is requesting a waiver to provide S inches of gravel sub-base instead of the required 12 inches. It is not clear why the Applicant cannot meet this requirement, If this waiver request is intended for driveway construction only,VHB would suggest that the Board consider the waiver request. This comment has been addressed. (7)Pavement: The Applicant is requesting a waiver from this section. Again,assuming that the Applicant is requesting this waiver for the proposed driveway,VHB would suggest that the Board consider the waiver request. This continent has been addressed. C-Sidewalks (2)Sidewalk Pavement: The plans do not indicate the sidewalk pavement structure. The Applicant's Engineer should place this information on the plans. The plans now show that the sidewalk is proposed to be bituminous concrete but the pavement notes for the sidewalk are not shown. F—Street Name Signs The Subdivision Rules and Regulations state that street name signs be installed at all street intersections. The,Applicant's Engineer should revise the plans to show the sign. This comment has been addressed. Appendix III—Specifications for Installation of Electrical Services Given the potential future development,the Applicant should consider this in the roadway design. This comment has been addressed. Appendix IV—Tree Planting Specifications The Applicant is not providing any landscaping as part of this project. The Applicant is requesting a waiver from this requirement. The Applicant is no longer requesting a waiver from this section. Although landscaping is now shown on the plans,the species and tree size are not shown. This section could not be reviewed for compliance. Appendix V—Rules and Regulations Governing Storm Water Management The Applicant is requesting a waiver from this requirement. If the Applicant's intent is to request this waiver for the proposed driveway construction,VHB would suggest that the Board consider the waiver request. VHB would not recommend that the Board consider a waiver of the Stormwater Management requirements for the roadway construction. See continent for Section 5.3 Stormwater Management Report. See VHB's initial comments. III. GENERAL COMMENTS: 1. Plans are not clear and as a result difficult to review. The Applicant should provide labels of existing and proposed features on the plans. VHB recommends two separate plans sets be created; the first set showing the proposed driveway and the second showing the proposed roadway may have been a clearer way to indicate initial and future construction. Although additional labels have been added,the plans are still unclear. 2. The Applicant should label the centerline geometry on plans as dictated in section 5.2.54 of the Subdivision Rules and Regulations. A waiver request does not appear warranted. Subdivision Street Typical Section: The hydrant is shown on left side of typical section. The plans show the hydrant oil the right side. The Applicant's Engineer should revise the plans. This comment has been addressed. 5. VHB recommends providing more information for the cape cod berm,such as dimensions and a construction detail. This comment has been addressed. 6. Granite curb inlets should be proposed at all proposed catch basins, VHB recommends that the Applicant provide a detail showing how the cape cod berm will transition to and from the granite curb inlet. This comment has been addressed. 7. VHB assumes that tire object shown to the left of Station 3+20±is a proposed septic system, The system appears to be located within the new right of way. The proposed system should be moved so that it is completely located on private property. This comment has been addressed. 8. Profile: a. Profile grade of the proposed roadway at its intersection with Gray Street does not meet the requirements of the Rules and Regulations. See Section 6.8.9 Intersections. b. The K values for the proposed vertical curves should be increased to meet the requirements for a 30mph design speed. See Comments for Section 5.2.5.4 Street Plan and Profile and Section 6.8.9 Intersections. 9. Are there any retaining walls proposed on this project? If so,they should be shown on the plans. This comment has been addressed. 10. A stop line and possible crosswalk should be provided and the intersection of Stiles Street and Gray Street. This comment has been addressed. IL It is noted that the proposed well is located within the 50' buffer to the wetlands. VHB recommends the conservation commission and any other appropriate authority review the plans. This Comment has been addressed. 12. The plans indicate a house will be demolished at the eastern corner of Stiles Street and Gray Street. The plans also show a proposed easement from this property. VHB assumes that the Applicant has obtained permission from the property owner to do this work. The Applicant has indicated that permission has been granted by the abutter. IV. DRAINAGE COMMENTS: I. Drainage calculations(flow calculations)were provided on September 17,2003. These calculations were not provided with the original submission. Also provided were three plan sketches showing contour information, As these sketches were faxed and are at a small scale,VHB could not verify the drainage area delineations. Additional information should be provided in order to verify the adequacy of the proposed drainage design, Pipe flow calculations were provided. This comment has been addressed. 2. Drainage invert information was not provided. VHB recommends that inverts be provided by the Applicant. The invert information has been provided. 3. Where does the proposed closed system outlet? Is a drainage easement required for this outlet? The plans show that the proposed system outlet to a flared end/level spreader on the owner's property. An easement will be required to the Town for future maintenance. The location of the proposed outlet generates many questions and concerns. What is the topography in the area of outfall? Is there a depressed area? Are any wetlands located near this outlet? Initial Comments The option#2 is not a problem and agreed to. H. Rules & Regulations 5.2.5.4 Street Plan&Profile 1. A waiver for minimum K value first vent. Curve was discussed and agreed to at the continued meeting 2. A waiver for maximum slope at the intersection of Stiles and Gray St. was discussed and agreed to at the continued meeting 5.2.5.5 Typical Sections 1. Only one roadway section is now provided on revised plans. 5.2.6 Test Data 1. locations of tests have been added to revised plans 5.3 Storm water management 1. Is not required for 1 lot. Will do report for full roadway build out which agrees with VHB initial comments and first review. 5.4 Environmental& Community Impact 1. Is not required for 1 lot.
