Site Inspection Date: May 5, 2006 Property Owner: H& S Realty Trust Bruce Hughe Affected Premises: Assessor's Map 103,Parcel 20 9 Winter Street,North Andover Decisions Requiring Reporting Conservation Order of Conditions: 242-1300(recorded bk. 9919,pg. 251) Conservation Order of Conditions: 242-1309 (recorded bk. 9874,pg. 254) Definitive Subdivision-approved 5/17/05 (recorded bk. 9874,pg.240) Watershed Special Permit(Dwelling)-approved 5/17/05 (recorded bk. 9874,pg. 235) Watershed Special Permit(Roadway)-approved 5/17/05 (recorded bk. 9874,pg. 249) On the date shown above the staff of Atlantic Engineering performed an inspection of the site per the requirements of the above listed decisions. The following information is a summary of what was found at the site. Any concerns or recommendations to the owner and/or the Conservation.Commission/Planning Department are italicized following the item. General 1. OoC Condition 41 -The required DEP signs exist,as required,at the site. Page I of 2 2. OoC Condition 45 -The wetlands line as approved was cliecked and remains visible. 3. OoC Condition 46 -The limit of work is clearly identified on the ground by the staked haybales/siltation fence. This limit is in good condition. The haybale/siltation fence was located by Atlantic to check its location relative to the approved location. It was fouled that the actual location encroaches into the limit of the 75' conservation zone. I have attached a sketch that shows the existing and proposed locations. Since the area has already been disturbed up to the actual line it is ]ny reconinnendation that the line be maintained as is and that the area between the existing and proposed litres be returned to its original condition ul,hen it is time for the haybales to be removed. The owner has been informed of this situation. 4. OoC Condition 47-The required extra haybales or on site as required. 5. OoC Condition 50 -No rainfall of 1 in or greater occurred during the week of this report. 6. OoC Condition S l -The required"no disturbance"markers/signs have not be placed as required by this condition. The site owner has been informed that this item needs to be complied with. 7. OoC Condition 60 -The proposed foundation location was staked on the ground prior to site excavation to insure installation in the correct location. Tine foundation location was re-staked after the initial excavation for the proposed footing locations. The footing are approximately half complete as of this date. 8. OoC Condition 61 -The foundation has not been completed as of the date of this inspection, 9. OoC Condition 63 -The required crushed stone entrance exists as required by this condition. 10. OoC Condition 64 -The erosion control devices were inspected and found to be in good condition. There was not siltation that required removing or cleaning. 11. OoC Condition 65 -No known de-watering has been done and there is no evidence of any sedimatation resulting fiom de-watering. 12. OoC Condition 66 -No fill has been brought onto the site as of this inspection. 13. OoC Condition 69 -No finish grading has been done. 14. OoC Condition 71 -No stockpiling of soil or other materials has been observed within 25 feet of any resource area. Miscellaneous Comments L The water line has been installed and backfilled. This was done prior to our engagement. 2. The sediment 7 oil separator has been installed. It is not active. No outfall pipe has been installed. 3. The roadway is under construction and is still a dirt access road. 4. Several digital pictures have been taken of the site to document the progression of the construction. These can be made available to you should you have any concerns regarding the above inspection comments. OoC Condition 51 - The required "no disturbance"markers/signs exist and are clearly visible. 7. OoC Condition 60 -No comment on this item. 8. OoC Condition 62 -The foundation decking has been completed and the house framing is being worked on.. 9. OoC Condition 63 -The required crushed stone entrance exists as required by this condition. 10. OoC Condition 64 -The erosion control devices were inspected and found to be in good condition with the following exceptions. A few areas of the cloth silt felOng are beginning to sag and will be fired. 11. OoC Condition 66 -No fill has been brought onto the site as of this inspection. 12. OoC Condition 69 -No finish grading has been done. 13. OoC Condition 71 -No stockpiling of soil or other materials has been observed within 25 feet of any resource area. 14. OoC Condition 74 - Some lunch and coffee break debris has been noted around the site and beyond the limit of proposed work, The owner has been instructed to clean this tip. Miscellaneous Comments l.
