Discussion PIC and inspector calibrated their food thermometers during
the inspection using the ice point method. Both thermometers read 32°F.
Discussion PIC to display current food permit, a copy of the certified
food protection manager (CFPM) certificate and a copy of the allergen
awareness training certificate all in public view.
**RE-INSPECTION** PIC TO DISPLAY APPLICABLE DOCUMENTS IN PUBLIC VIEW.
Verified 11/13/24 OBSERVATION: Observed multiple knives in wall mounted rack with
accumulation of food debris on the blades . PIC to ensure knives are
cleaned and sanitized prior to storage.
**RE-INSPECTION** OBSERVED STORED KNIVES IN CLEAN CONDITION.
4-601 .11 (A)-Pf REGULATION: Item 16 Food contact surface; cleaned and sanitized --
Cleanliness of FCS and Non-FCS
Verified 11/13/24 OBSERVATION: Observe labeled bucket holding wiping cloths in QAC sanitizer
measure 0 ppm via PIC's test strips. Container of QAC sanitizer attached
to dispenser appeared to be empty. PIC supplied new container of
sanitizer and made new sanitizer concentration that measured 200 ppm. PIC
to ensure QAC sanitizer is maintained at 150-400 ppm.
**RE-INSPECTION** OBSERVED QAC SANITIZER IN LABELED BUCKET MEASURE 200 PPM
VIA INSPECTORS TEST STRIPS.
4-501 .114-P REGULATION: Item 16 Food contact surface; cleaned and sanitized --

Verified 11/13/24 OBSERVATION: Observed two containers of beef barbacoa in meat walk in
refrigerator with a preparation date of 10/20/24. Upon reviewing
compliant date marking, PIC voluntarily discarded foods. PIC to re-train
staff on compliant date marking ensuring TCS foods are not held for
longer than 7 days.

Verified 11/13/24 OBSERVATION: Observed cooler holding corn on the cob stored directly on
the floor. PIC to ensure foods are stored at least 6 inches above the
floor. PIC relocated cooler to table during the inspection.

Verified 11/13/24 OBSERVATION: Observed employee wearing watch on their wrist while
preparing food. Reviewed with PIC that the only compliant jewelry to be
worn while preparing food is a plain banded ring. PIC to ensure employees
do not wear jewelry on their arms or hands while preparing food.

OBSERVATION: Allergen statement not observed on digital menu board or
paper menus. PIC to include statement, "Before placing your order, please
inform your server if a person in your party has a food allergy" . PIC to
ensure statement is easily visible on all menus. In lieu of placing the
notice on the menu board itself, the food establishment may post the
notice adjacent to the menu board or at each point of service where food
is ordered.
**RE-INSPECTION** ALLERGEN STATEMENT NOT OBSERVED ON PAPER MENUS OR MENU
BOARD. PIC TO INCLUDE ON ALL MENUS. WILL VERIFY AT NEXT ROUTINE.

Discussion Hood sticker dates 10/9/2024, in compliance.
In Compliance Observed low temperature dish machine dispense sanitizer at
50 ppm via PIC' s test strips. Observed rice at 141°F and chicken at 167OF
both in steam table; cooked peppers in drawer refrigerator below grill at
37°F; pico de gallo in top rail of in line refrigerator at 39°F; salsa
verde in low sliding refrigerator at 38°F; chicken tinga in meat walk in
refrigerator at 37°F; sour cream in dairy / produce walk in refrigerator
at 39°F.
Closing Violations marked "Verified" have been corrected. Violations not marked
"Verified" remain uncorrected. Uncorrected violations are to be corrected
immediately. Uncorrected violations may result in additional
Re-inspections and fees, fines and or administrative action including
