PIC to ensure foods are stored at least 6 inches above the floor. **RE-INSPECTION** OBSERVED MULTIPLE CONTAINERS OF FOOD STORED DIRECTLY ON THE FLOOR OF THE WALK IN FREEZER. PIC TO ENSURE FOODS ARE STORED IN COMPLIANCE. 3-305.11 REGULATION: Item 39 Contamination prevented during food preparation, storage and display -- Storage, Food-Preventing Contamination from the Premises Verified 12/04/24 OBSERVATION: Observed a few food employees not wearing hair restraints while preparing foods. PIC to ensure all food employees wear a compliant hair restraint such as a hat or hair net while handling / preparing foods. **RE-INSPECTION** OBSERVED EMPLOYEES WEARING COMPLIANT HAIR RESTRAINTS WHILE PREPARING FOOD. 2-402 .11 REGULATION: Item 40 Personal cleanliness -- Hair Restraints Verified 12/04/24 OBSERVATION: Observed PIC's QAC sanitizer test strips with an expiration date of 7/15/2024. PIC to supply un-expired test strips. **RE-INSPECTION** OBSERVED PIC'S NEW QAC SANITIZER TEST STRIPS READILY AVAILABLE. 4-302 .14-Pf REGULATION: Item 48 Warewashing facilities : installed, maintained, and used; test strips -- Sanitizing Solutions, Testing Devices Repeat OBSERVATION: Observed flooring throughout kitchen and mop sink in poor repair. PIC to ensure flooring is smooth, easily cleanable, and nonabsorbent. PIC to repair using approved methods and materials. **RE-INSPECTION** OBSERVED FLOORING IN SAME STATE OF REPAIR. PIC STATES THEY ARE IN PROCESS OF REPAIRING FLOOR. WILL VERIFY AT NEXT ROUTINE INSPECTION. 6-501 .11 REGULATION: Item 55 Physical facilities installed, maintained, and clean -- Repairing-Premises, Structures, Attachments, and Fixtures-Methods Verified 12/04/24 OBSERVATION: Observed receiving door at dock with gap in bottom. Reviewed with PIC that gap in door could be potential entry point for pests. PIC to ensure outer openings are protected against entry of pests. PIC to repair using approved methods and materials. **RE-INSPECTION** OBSERVED RECEIVING DOOR AT DOCK IN GOOD REPAIR. 6-202 .15 REGULATION: Item 38 Insects, rodents, and animals not present -- Pests, Outer Openings, Protected Discussion Observed room holding chemicals and containing mop sink with over-head light not turning on when light switch is turned on. PIC to ensure adequate lighting is available. **RE-INSPECTION** OBSERVED ADEQUATE LIGHTING AVAILABLE IN ROOM CONTAINING MOP SINK. Discussion Hood sticker dates 10/22/2024, in compliance. Observed chicken fingers in cabinet of pizza in line refrigerator at 40°F; sliced turkey in top rail of sandwich in line refrigerator at 38°F; hamburger patties in drawer refrigerator below grill at 370F; cooked pasta in "Coca-Cola" refrigerator at 41°F; grilled chicken in walk in refrigerator at 38°F. Closing Violations marked "Verified" have been corrected. Violations not marked "Verified" remain uncorrected. Uncorrected violations are to be corrected immediately. Uncorrected violations may result in additional Re-inspections and fees, fines and or administrative action including possible suspension of permit.
