Verified 12/04/24 OBSERVATION: Observed spoon stored in basin of hand sink adjacent to salad
in line refrigerator. PIC to ensure all hand sinks are maintained
accessible at all times and are used for handwashing only.
**RE-INSPECTION** OBSERVED ALL HAND SINKS MAINTAINED ACCESSIBLE.

Verified 12/04/24 OBSERVATION: Observed ice machine with accumulation of what appeared to be
mold in the interior. PIC to fully empty ice machine and clean and
sanitize at a frequency specified by the manufacturer or as frequently
needed to preclude accumulation of soil or mold.
**RE-INSPECTION** OBSERVED ICE MACHINE IN CLEAN CONDITION.
4-602 .11 REGULATION: Item 16 Food contact surface; cleaned and sanitized -- Ice
Machine Cleaning Frequency
Verified 12/04/24 OBSERVATION: Observed cooked potatoes at 48OF and cooked broccoli at 45°F
both in the top rail of the in line refrigerator across from the stove.
Observed cover unable to fully close due to cutting board being stored
directly below which could be attributing to elevated temperatures. Upon
reviewing cold holding requirements, PIC instructed employee to place
elevated foods in the walk in refrigerator. PIC to routinely monitor
temperatures with their food thermometer ensuring TCS foods are
maintained at 41°F or below.
**RE-INSPECTION** OBSERVED BUTTER AT 41OF AND COLESLAW AT 390F BOTH IN TOP
RAIL OF IN LINE REFRIGERATOR ACROSS FROM STOVE. PIC STATES THEY ARE NOW
USING A SMALLER CUTTING BOARD WHICH ALLOWS COVER TO FULLY CLOSE.

Verified 12/04/24 OBSERVATION: Observed PIC's "Sink and Surface Cleaner Sanitizer" test
strips with an expiration date of 05/2024 . Employee states contacting
distributor during the inspection. PIC to sully un-expired test strips.
**RE-INSPECTION** OBSERVED PIC'S TEST STRIPS WITH AN EXPIRATION DATE OF
09/2026.

Verified 12/04/24 OBSERVATION: Observed staff toilet room door maintained ajar and not
provided with a self-closing door. Reviewed with PIC that all toilet
rooms located on the premises are to be completely enclosed and provided
with a tightfitting and self-closing door.

In Compliance Observed high temperature dish machine achieve a utensil
surface temperature of 180OF via PIC's sensor stickers. Observed blue
cheese dressing in cabinet of salad in line refrigerator at 40°F; sword
fish in "True" single-door refrigerator at 38°F; cooked sausage in top
rail of pizza in line refrigerator at 390F; roasted vegetables in cabinet
of in line refrigerator across from stove at 390F; sliced tomatoes in top
rail of in line refrigerator across from pizza in line refrigerator at
41°F; sliced turkey in drawer refrigerator at 38°F; chicken salad in top
rail of take out in line refrigerator at 38°F; raw pork in walk in
refrigerator at 37°F.
