Verified 12/10/24 OBSERVATION: Observed multiple hand sinks supplied only with hand
sanitizer. PIC to ensure each hand sink is provided with a supply of hand
cleaning liquid, powder, or bar soap.

Verified 12/10/24 OBSERVATION: Observed ice machine with accumulation of what appears to be
mold in the interior. PIC to fully empty ice machine and clean and
sanitize at a frequency specified by the manufacturer or as frequently
needed to preclude accumulation of soil or mold.
**RE-INSPECTION** OBSERVED ICE MACHINE IN CLEAN CONDITION.

Machine Cleaning Frequency
Verified 12/10/24 OBSERVATION: Observed QAC sanitizer in labeled buckets measure less than
150 ppm via PIC's test strips. PIC to call a repair service for QAC
sanitizer dispenser if needed and provide health office with status or
repair slip within 24 hours. PIC to manually mix QAC sanitizer or use
dish machines for sanitizing until QAC sanitizer is dispensed and
maintained at 150-400 ppm.
**RE-INSPECTION** OBSERVED QAC SANITIZER IN LABELED BUCKET MEASURE 200 PPM
VIA PIC'S TEST STRIPS.

Verified 12/10/24 OBSERVATION: Observed a few employees wearing jewelry on their wrists
while preparing food. Reviewed with PIC that the only compliant jewelry
to be worn while preparing food is a plain banded ring. PIC to ensure
employees do not wear jewelry on their arms or hands while handling food.

Verified 12/10/24 OBSERVATION: Observed scoops stored in containers of flours and bread
crumbs with the handles making direct contact with the foods. PIC to
ensure if in-use utensils are stored in the containers of food that the
handles are facing up.

device. Reviewed examples with PIC, including 160°F sensor stickers, dish
plate thermometers, and water-proof maximum registering thermometers. PIC
to supply and routinely monitor utensil surface temperatures.

Verified 12/10/24 OBSERVATION: Observed gender neutral employee toilet room not supplied
with a covered receptacle. PIC to ensure all toilet rooms used by females
are supplied with a covered receptacle for sanitary napkins.

5-501 .17 REGULATION: Item 53 Toilet features; properly constructed, supplied,and
cleaned -- Toilet Room Receptacle, Womens, Covered
Discussion Observed piece of prosciutto sitting on deli slicer at 41°F
during the inspection that PIC states is maintained on the slicer during
service. PIC unable to provide Time as a Public Health Control (TPHC)
procedure or provide time logs. Reviewed compliance responsibilities with
PIC, including submitting a completed copy of the TPHC procedure to the
health office for review and maintaining a copy on site for review, and
developing a method to identify the time that is 4 hours past the point
in time when the foods were removed from temperature control. Reviewed
with PIC that unmarked foods are considered for consumption. Provided PIC
with TPHC sample procedure.
**RE-INSPECTION** NO PROSCIUTTO HELD WITHOUT TEMPERATURE CONTROL OBSERVED
DURING THE INSPECTION. PIC PROVIDED TIME LOGS, IN COMPLIANCE. PIC TO
COMPLETE TPHC PROCEDURE AND SUBMIT TO THE HEALTH OFFICE FOR REVIEW AND
ENSURE A COMPLETE COPY IS MAINTAINED ON SITE FOR FUTURE INSPECTIONS.
Discussion PIC provided digital copy of their allergen awareness
training certificate. PIC to obtain physical copy and display in public
view adjacent to the current food permit.
**RE-INSPECTION** OBSERVED ALLERGEN AWARENESS TRAINING CERTIFICATE
DISPLAYED IN COMPLIANCE.
Discussion Hood sticker dates 10/15/2024, in compliance.
In Compliance Observed high temperature dish machine achieve a utensil
surface temperature of 166°F via inspector's dish plate thermometer; low
temperature dish machine dispense sanitizer at 50 ppm via inspector's
test strips. Observed meatballs in table-top warmer at 138°F; grilled
chicken in top rail of grill in line refrigerator at 390F; butternut
squash ravioli in drawer refrigerator at 41°F; sliced tomatoes in top
rail of pantry in line refrigerator at 380F; tomato sauce in top rail of
pizza in line refrigerator at 39°F; pasta fagioli soup in walk in
refrigerator at 37°F.
