PIC to contact health office regarding current food permit status and obtain physical copy. PIC to ensure current food permit in posted in public view adjacent to the certified food protection manager (CFPM) certificate. **RE-INSPECTION** OBSERVED CURRENT FOOD PERMIT POSTED IN COMPLIANCE. Verified 12/10/24 OBSERVATION: Observed two hand sinks in kitchen without cleanser. PIC to ensure all hand sinks are provided with a supply of hand cleaning liquid, powder, or bar soap. **RE-INSPECTION** OBSERVED ALL HAND SINKS SUPPLIED WITH HAND CLEANSER. 6-301 .11-Pf REGULATION: Item 10 Adequate handwashing sinks properly supplied and accessible -- Handwashing Cleanser, Availability Repeat OBSERVATION: Observed knives in mounted rack with slight accumulation of food debris. Observed deli slicer that PIC identified as used "a few days ago" with food debris on the blade. PIC to ensure equipment food contact surfaces are cleaned and sanitized after each use and prior to storage. **RE-INSPECTION** OBSERVED DELI SLICER IN CLEAN CONDITION. OBSERVED MULTIPLE STORED KNIVES IN MOUNTED RACK WITH ACCUMULATION OF FOOD DEBRIS ON THE BLADES. PIC TO ENSURE KNIVES ARE CLEANED AND SANITIZED PRIOR TO STORAGE. 4-601 .11 (A) -Pf REGULATION: Item 16 Food contact surface; cleaned and sanitized -- Cleanliness of FCS and Non-FCS Discussion Provided PIC with "Keep Commercial Deli Slicers Safe" handout from the FDA. OBSERVATION: Observed ice machine with slight accumulation of what appears to be mold in the interior. PIC to fully empty ice machine and clean and sanitize at a frequency specified by the manufacturer or as frequently needed to preclude accumulation of soil or mold. **RE-INSPECTION** OBSERVED ICE MACHINE IN KITCHEN WITH WHAT APPEARS TO BE MOLD IN THE INTERIOR. PIC TO CLEAN AND SANITIZE. 4-602 .11 REGULATION: Item 16 Food contact surface; cleaned and sanitized -- Ice Machine Cleaning Frequency Verified 12/10/24 OBSERVATION: Observed sausage patties held without temperature control adjacent to grill at 74°F. Reviewed hot holding requirements with PIC. PIC to rapidly reheat sausage patties and ensure TCS foods are maintained at 135°F in hot holding. PIC to re-train staff. **RE-INSPECTION** NO TCS FOODS HELD WITHOUT TEMPERATURE CONTROL OBSERVED. 3-501 .16 (A) (1) -P REGULATION: Item 21 Proper hot holding temperature -- Holding TCS Food, Hot Repeat OBSERVATION: Observed sliced ham at 53°F and sliced turkey at 47°F in top rail of in line refrigerator. Bags of deli meats observed not fully submerged in deli pans. Upon reviewing cold holding requirements, PIC voluntarily discarded deli meats. PIC to ensure TCS foods are stored in equipment in a way that maintains them at 41°F or below. PIC to re-train staff. **RE-INSPECTION** OBSERVED SLICED HAM AT 450F AND SLICED TURKEY AT 520F BOTH IN THE TOP RAIL OF THE IN LINE REFRIGERATOR. PIC TO DETERMINE SOURCE OF ELEVATED TEMPERATURES. REVIEWED COLD HOLDING REQUIREMENTS WITH PIC. Reviewed cold holding requirements with PIC. PIC to ensure eggs that have not been treated to destroy all viable Salmonellae are stored in refrigerated equipment that maintains an ambient air temperature of 450F or less. PIC to re-train staff. **RE-INSPECTION** NO TCS FOODS HELD WITHOUT TEMPERATURE CONTROL OBSERVED. 3-501 .16 (A) (2) (B) -P REGULATION: Item 22 Proper cold holding temperature -- Holding TCS Food, Cold OBSERVATION: Observed containers of tuna salad in cabinet of in line refrigerator at 530F; PIC identified tuna as recently prepared and is cooling. Reviewed with PIC to place cooling foods in the walk in refrigerator to rapidly cool. PIC to re-train staff on cooling methods. **RE-INSPECTION** EMPLOYEE STATES THEY PLACE COOLING FOODS INSIDE THE CABINET OF THE IN LINE REFRIGERATOR OR INSIDE THE WALK IN REFRIGERATOR. PIC TO RE-TRAIN STAFF TO PLACE COOLING FOODS IN RAPID COOLING EQUIPMENT. 3-501 .15-Pf REGULATION: Item 33 Proper cooling methods used; adequate equipment for -- Cooling Methods OBSERVATION: Observed an employee not wearing a hair restraint while preparing food. PIC to ensure all employees wear a compliant hair restraint such as a hat or hair net while handling food. **RE-INSPECTION** OBSERVED EMPLOYEES NOT WEARING HAIR RESTRAINTS WHILE PREPARING FOODS. REVIEWED COMPLIANCE RESPONSIBILITIES WITH PIC. 2-402 .11 REGULATION: Item 40 Personal cleanliness -- Hair Restraints OBSERVATION: Observed floors and walls on chef's line with accumulation of debris. PIC to ensure physical facilities are cleaned as often as necessary to keep them clean. **RE-INSPECTION** OBSERVED PORTIONS OF FLOORING ON CHEF'S LINE WITH ACCUMULATION OF SOIL. PIC TO CLEAN. 6-501 .12 REGULATION: Item 55 Physical facilities installed, maintained, and clean -- Cleaning Frequency, Restrictions Discussion Hood sticker indicates past due. PIC to correct immediately and contact the fire department for compliance responsibilities. **RE-INSPECTION** HOOD STICKER DATES 11/22/2024, IN COMPLIANCE. In Compliance Observed high temperature dish machine achieve a utensil surface temperature of 166.7°F via inspector's water-proof maximum registering thermometer; QAC sanitizer in three compartment sink measure 200 ppm via inspector's test strips. Observed final cooking temperature of fried egg at 1860F; rice in steam table at 1990F; chicken salad in cabinet of in line refrigerator at 390F; cut cantaloupe in top rail of salad in line refrigerator at 370F; cooked pasta in walk in refrigerator at 39°F. Closing Violations marked "Verified" have been corrected. Violations not marked "Verified" remain uncorrected. Uncorrected violations are to be corrected immediately. Uncorrected violations may result in additional Re-inspections and fees, fines and or administrative action including possible suspension of permit.
