DESCRIPTION OF VIOLATION
Fail Code
Discussion PIC states dish machine is not in use and is in need of
repair. PIC states calling for repair service prior to inspection. PIC to
provide health office with copy of repair slip once completed.
OBSERVATION: Observed QAC sanitizer in two-compartment sink measure less
than 150 ppm via inspectors and PIC's test strips. PIC to call a repair
service for QAC sanitizer dispenser if needed and provide the health
office with a status or repair slip within 24 hours. PIC to manually mix
sanitizer until dispenser is in good repair and dispensing QAC sanitizer
at 150-400 ppm.
4-501 .114-P REGULATION: Item 16 Food contact surface; cleaned and sanitized --

OBSERVATION: PIC provided food thermometer with a temperature range of
140-190°F. Discussed with PIC to provide a food thermometer capable of
measuring temperatures of cold foods.

OBSERVATION: Observed refrigerator in "Toddler Room A" not supplied with
an ambient thermometer, PIC unable to verify. PIC to ensure all
refrigerators are supplied with an ambient thermometers.

Measuring Devices, Ambient Air and Water-Accuracy
OBSERVATION: Observed food employee in kitchen preparing foods while not
wearing a hair restraint. PIC to ensure all food employees wear a
compliant hair restraint such as a hat or hair net while preparing
exposed food, clean equipment, utensils and linens, and unwrapped
single-service and single-use articles.
2-402 .11 REGULATION: Item 40 Personal cleanliness -- Hair Restraints
Discussion Hood sticker indicates past due. PIC to correct immediately
and contact the fire department for compliance responsibilities.
Closing Correct Priority Item and Priority Foundation Item violations immediately;
Core Item violations within 10 days. Correct all violations in entirety
and maintain. Train and supervise staff. Failure to correct all
violations and maintain corrections may result in administrative action
