The Planning Board is in receipt and approval of the final inspection and reports and/or upon
otherwise determining that all work of the permit has been satisfactorily completed in accordance
with the approved plans, specifications, and conditions of the approved permit. Therefore, the
Board grants a Certificate of Completion to close out September 5,2017 Land Disturbance Permit.
CONDITIONS
This decision must be filed with the North Essex Registry of Deeds and a copy of the recording
submitted to the Town Planner.
MOTION
After a Planning Board meeting on the above date, and upon a motion by J. Simons and 2nd by S.
Kevlahan a vote was taken to grant a Certificate of Completion in order to close out the September
5, 2017 Land Disturbance Permit. The vote was 5-0,unanimous in favor of the certificate.

Dear Board Members:
In accordance with Condition 31 of the Land Disturbance Permit Special Conditions issued on
September 5, 2017, please find enclosed an As-Built Site Conditions plan for the above referenced
project.
hereby certify that the project was built in general conformance with the approved site plans
except for the following minor deviations:
• Lot 1 (1995 Salem Street): The total proposed impervious area was 6,063 SF. The total as-
built impervious area is 7,700 SF. An additional 1,690 SF of infiltration stone/drip edge
area where installed compared to the proposed plan. This additional pervious drainage
stone area results in a net impervious area decrease of 53 SF.
• Lot 2/3 (1975 & 1985 Salem Street): The total proposed impervious area was 17,816 SF.
The total as-built impervious area is 21,857 SF. An additional 4,854 SF of infiltration
stone/drip edge area where installed compared to the proposed plan. This additional
pervious drainage stone area results in a net impervious area decrease of 813 SF.
The above referenced changes should have no measurable impact on the resource areas or
abutters. At this time, we are requesting that the Planning Board issue a Certificate of compliance
