Dear Mr. Hodgson,
Upon review of the Plan entitled "Site Plan for 1975 Salem Street(Lot 1),"prepared for
Livingstone Development Corp,, dated October 16, 2019, The Staff Planner identified the
following inconsistencies associated with the Site Plan for Special Permit for Lots 1-3 Salem
Street dated October 3, 2016, revised to August 20, 2017:
• The plan depicts a modification to the foundation size and shape to the building erected at
1995 Salem Street.
• A new sedimentation control barrier and foundation drain are shown on the Plan.
All work at the site roust stop and the following information must be submitted and reviewed by
Planning:
• A revised plan showing the limit of work, the as-built site grading and all impervious
surfaces should be added to the Plan.
• The project engineer should prepare a letter that details the plan modifications,
• After construction Stormwater calculations
The site plan and stormwater calculations require Stormwater Peer Review. Please do not

Re: 1995 Salem Street—Civil Peer Review
Dear Ms. Gregoire,
On behalf of the Town of North Andover, TEC, Inc. reviewed documents as part of the civil engineering
peer review for the proposed development located at 1995 Salem Street in North Andover, MA.
Christiansen & Sergi submitted the following documents on behalf of Livingstone Development Corp,
which TEC reviewed for conformance with the approved drawings and generally accepted industry
standards:
• Site plans entitled "Site Plan for Special Permit for Lots 1-3 Salem Street" prepared by
Christiansen & Sergi dated October 3, 2015, last revised August 20, 2017;
• Interim As-built Plan for 1995 Salem Street, prepared by Christiansen & Sergi, dated October
29, 2019
• Cover letter, prepared by The Morin-Cameron Group, dated October 31, 2019;
In addition to review of the documents and plans, TEC performed a field visit to the project site on
Wednesday, November 27, 2019, to review current site conditions. At the time of the site visit, the
Contractor had been given a "stop work" order by the Town. Upon completion of the visit, TEC has
compiled the following comments for your consideration:
1. As indicated on the Interim As-built plan, it appears that the Contractor disregarded the
approved limit of work. Areas outside the limit of work have been cleared and planted with loam
& seed.
2. TEC concurs with CSI that the infiltration stone was installed with additional stone,
approximately 25 feet longer than the approved site plan.
3, TEC concurs with CSI that the house was constructed with a different, larger footprint than
shown on the approved plan.
4. Based on review of the approved site plan, and the interim as-built, the driveway has been
constructed wider than the approved plan, resulting in additional impervious area. CSI should
provide additional calculations to prove that the stormwater system can handle the additional
impervious area.
5. The edge of the driveway where the infiltration stone has been constructed has a 1-2' drop
between the infiltration stone and the rock wall. Based on the approved site plan, the grading
should slope in the other direction.

6. TEC recommends that the Town of North Andover retain a consultant to provide construction
observations on a weekly basis for the remainder of construction. TEC can provide additional
observations, if requested.

Dear Ms. Gregoire:
We have received a review letter from TEC, dated December 13, 2019, regarding the above-mentioned
property and we offer the following responses:
1. No response needed.
2. No response needed.
3. No response needed.
4. The area of the driveway shown on the Interim As-built plan shows the limit of the gravel base
for the driveway in place at the time of the "Stop Work" order. The actual paved driveway will
be smaller than the gravel base and will be consistent with what was approved on the design
plan. The remaining gravel base outside of the pavement will be filled, foamed and seeded,
The walkway shown on the as-built is also shown as the width of the gravel base and will be
much smaller than the base. At this point in the construction phase, we feel performing
drainage calculations is not necessary as the final pavement and walkway have not been
installed.
5. We have discussed this comment with the reviewer and have agreed that a small portion along
the left side of the infiltration stone will need to be filled to ensure the stormwater runoff will
reach the infiltration stone as designed.
6. The majority of the site work is complete for the site. The only remaining items are to install the
driveway,walkway, and foaming and seeding along the driveway and walkway. We feel it is not
necessary to have a weekly inspection of the site since the remaining items are minor and
Involve little or not additional earth disturbance. In our opinion, one inspection during the work
and one after would be adequate.

We have received you letter dated October 18, 2019,regarding the above-mentioned property. We
have performed an interim as-built and offer the following response:
1, An Interim As-built plan has been provided. The limit of work, site grading and impervious
surfaces are shown. As you know, the site was under construction at the time of the stop work
order and therefore some items are still incomplete. The walkway has not been installed and
the driveway has not been paved.
2. The following modifications were made to the site:
a. A different house footprint was built.
b. The approved dwelling was designed as a garage under, however the as-built dwelling
has an attached garage.
c. The infiltration stone along the left side of the driveway was extended an additional 26'.
d. A foundation drain was installed.
e. The Limit of Work line in front of the dwelling is different from the approved Limit of
Work.
3. At this point of the site construction, it would not be accurate to provide after construction
stormwater calculations as most of the impervious surfaces on the lot are not in place. We have
calculated the area of the approved house footprint and compared that to the as-built footprint,
A covered porch was also built. The area of the as-built dwelling and porch is 134 s.f. larger
than the proposed dwelling. The applicant extended the infiltration stone along the left side of
the driveway an additional 26'. The additional storage provided by the infiltration stone
provides an additional 187.2 c.f, of stormwater storage. The additional storage volume would
handle runoff from an additional 261 s.f. under a 100yr storm event. In our opinion, the
additional infiltration stone provided is capable of accepting, storing and infiltrating runoff from
the increased impervious area.

We have received you letter dated October 29, 2019, regarding the above-mentioned property. We have
performed an interim as-built and offer the following response:
1. The approved limit of Work and the as-built Limit of Work are shown on the Interim As-built Plan.
2. An Interim As-built plan has been provided. The limit of work, site grading and impervious surfaces are
shown. As you know,the site was under construction at the time of the stop work order and therefore

3. The as-built detention pond and infiltration stone have been shown on the interim as-built.
4. At this point of the site construction, it would not be accurate to provide post-construction stormwater
calculations as most of the impervious surfaces on the lot are not in place. We have calculated the area of
the approved house footprint and compared that to the as-built footprint, A covered porch was also
built. The area of the as-built dwelling and porch is 134 s.f. larger than the proposed dwelling. The
applicant extended the Infiltration stone along the left side of the driveway an additional 26'. The

5. Erosion control blankets were not installed. The site was graded in a way that reduced the slopes in the
area where the erosion control blankets were proposed. The site is currently stabilized with vegetation.

attached garage.
c. The infiltration stone along the left side of the driveway was extended an additional 55'.
d. A foundation drain was installed.
e. The limit of Work line in front of the dwelling is different from the approved limit of Work.
