To Whom It May Concern:
The proposed wastewater system design plan for the above site dated May 5, 2017 with a final
revision date of August 14, 2017 and received on August 17, 2017 has been approved.
The design has been approved for use in the construction of a new on-site septic system for a five
(5) bedroom, eleven(11)total room residential dwelling utilizing gravity leaching trenches. This
design plan approval is valid until August 24, 2020.
During this time, a licensed septic system installer must obtain a permit and complete this work,
and a Certificate of Compliance be endorsed by the installer, designer and the Town of North
Andover.
This approval is also subject to the following conditions:
1. Prior to the issuance of the Disposal Works Construction Permit, the applicant must
submit a foundation as-built at the same scale as the approved plan.
2. Prior to the issuance of the Disposal Works Construction Permit,the applicant must
submit the floor plans of the proposed dwelling showing no greater than five(5)
bedrooms or eleven(11) total rooms.
3. Prior to construction,the designer must establish a benchmark within seventy-five
(75) feet of the proposed system. The location and elevation of the benchmark must
be submitted to the Health Department.

In response to Mr. Borgesi s comments I offer the following in the order the comments were made
1. Drainage calculations are attached
2. The culvert was on the plan but not labeled as such. The culvert allows runoff that flows in that direction to
flow towards the northwest property line as it does now. I have added_lab-els.and.ad.d-e-d rjp rap a.s.suggested-
Rip Rap sizing has been added to the detail sheet.
3. The elevations should be 117.0 and 117.5.The changes have been made to the plan
4. The culvert is labeled on the site under the common drive.
5. I have added driveway profiles to the plan set and modified the proposed grading somewhat.The average
ode on the common drive is
9%with the steepest grade of 14% occurring for only a length of 45 ft.The
driveway for Lot 3 has an average grade of 9%and a maximum grade of 15%for 48 ft.The driveway locations
and grades were driven to minimize the disturbance to the riverfront area.You will notice in the profile that for
both driveways the approach_ gtenni2 Salem Street rs lowered to 5%.Additionally on Lot 3 the grade at the top
of the drive is 3%.

Sorry for the delay in getting these to you -here are our comments:
1975 Salem Street:
1. Stormwater design calculations should be submitted and reviewed including calculations for the proposed infiltration
basins.
2. It is unclear what the proposed culvert that crosses the driveway to Lot 1 is. This should be identified on the site
plan including the inlet structure and outlet protection.
3. The same culvert is identified as having an inlet invert of 17.5 and outlet invert of 17.0 while the surrounding contours
are at an elevation of 120'. This discrepancy shall be corrected,
4. It is unclear where the 3'x5'box culvert included on the detail sheet is located on the site.
5. A portion of the driveway for Lot 1 is graded at almost a 17% slope. This is an extremely steep slope for a
residence. Regrading of this area should be considered.

6. There is a substantial amount of infiltration systems proposed along the driveways. In addition for the drainage to
operate properly it is critical that the final grading and final paving be constructed per design. We recommend requiring
that a third party engineer verify the construction of the driveways and infiltration systems and submit in writing a
certification that the driveways and infiltration systems are constructed per design.
7. Further detail shall be provided along the driveway and right of way interface to show that all stormwater runoff will be
contained on-site and not directed onto the Town right of way per the Town's driveway policy. The driveway policy states
the following: All driveway entrances in areas where the road drainage is "country drainage" shall leave the existing j
paved way with a negative pitch. The pavement elevation of all such driveways must be at least 4 inches below the
edge of the existing roadway's pavement at a distance of 6 feet into the roadway shoulder from that edge of
pavement. The Applicant's engineer may want to consider the installation of a swale or berm located at the end of the
driveway to direct any stormwaler away from the public right of way.

Please forward the attached to the Board members. It is the approved plan for the Salem woods. I added the red print to
show actual grades that were not marked on the plan. It should be noted that the approved "roadway" is effectively a
common drive that serves 3 lots. It is 16 ft wide, the same as what we propose for our common drive for two lots. The
grades on what was then the existing drive and is now part of the road is 15.85% for 78 feet, and 8.85% in what was
then the proposed roadway a grade of 8.85% for 101 ft. The 15.85% grade is steeper than what I proposed and for a
substantially longer distance.
It may be good if the members drove on that roadway to get a better feel for roadway grades and what has been
approved in the past.

Per our conversation I reviewed the aforementioned mentioned and agrees that the lot, other than what they are seeking
a permit for, complies with the bylaw.

Frontage Exception under sections 2,30.1, 7.2.1 7.22, 10.3 and 10.31 of the North Andover Zoning Bylaw. The

prior to that Hearing I would like to have confirmation that the Building Commissioner agrees that the lot, other than
what they are seeking a permit for, complies with the bylaw (lot width, etc.).
Attached is the plan. I have a larger planset we can review as well.
